question is whether the Games will deliver a lasting change

:00:00.:00:13.

Good evening. A report into the death of a three year old boy from

:00:14.:00:18.

Somerset has found the introduction of a new computer system may have

:00:19.:00:22.

played a part in his death. Samuel Starr, who was born with a heart

:00:23.:00:26.

condition, died in September 20 2. His parents say delays caused by the

:00:27.:00:30.

new system meant he hadn't had a cardiac check`up for more than 0

:00:31.:00:34.

months. As our health correspondent, Matthew Hill, reports, Samuel was

:00:35.:00:37.

not the only child whose appointment was delayed.

:00:38.:00:46.

Samuel was born with a heart condition when he was three he had a

:00:47.:00:53.

cardiac operation. But his parents were concerned that it took longer

:00:54.:01:04.

than expected. We spoke to Samuel, reading him stories, and he passed

:01:05.:01:10.

away in our arms. Despite the many questions they had about his

:01:11.:01:14.

operation in Bristol, their biggest concern was whether his death might

:01:15.:01:17.

have been prevented if there were not huge delays to his regular

:01:18.:01:23.

checkups when he was alive. Samuel's local hospital was in Bath.

:01:24.:01:31.

Sam's condition meant he was supposed to have regular scans to

:01:32.:01:36.

make sure his condition didn't worsen. Following successful

:01:37.:01:41.

surgery, his heart was scanned but that turned out to be the last scan

:01:42.:01:48.

he had for 20 months. Inside out West has discovered a key factor in

:01:49.:01:53.

these delays was the introduction of a new IT system for hospital

:01:54.:01:58.

appointments. I am disgusted that it was recognised that no action was

:01:59.:02:06.

taken on it. In a review of his death, Bristol 's children Hospital

:02:07.:02:10.

concluded these delays may have played a part. We asked how many

:02:11.:02:15.

appointments were overdue or delayed because of the new computer system.

:02:16.:02:21.

They told us there were 63 overdue appointments with some delays of up

:02:22.:02:25.

to two years before they were discovered. It's very serious. You

:02:26.:02:32.

are dealing with people's lives and it's essential if you use computer

:02:33.:02:37.

systems that you have computer systems that work. Inside out West

:02:38.:02:44.

has also discovered there were 3 overdue appointments because of the

:02:45.:02:47.

introduction of the computer system, with some taking nearly two

:02:48.:02:59.

years to discover. We have offered our sincere condolences to the

:03:00.:03:04.

family of Samuel. The inquest will be held soon and we hope this will

:03:05.:03:09.

provide both Samuel's family and everybody involved in his care with

:03:10.:03:13.

a clear indication of the circumstances surrounding his death.

:03:14.:03:18.

And you can see Matthew's film on the BBC iPlayer. A jury has heard

:03:19.:03:22.

how a Bristol lorry driver is alleged to have driven through

:03:23.:03:24.

changing traffic lights seconds before colliding with a pedestrian.

:03:25.:03:28.

Teacher Jake Thompson died of head injuries after being hit by the 13.5

:03:29.:03:32.

tonne truck at the tree lamps junction on the A4 at Totterdown.

:03:33.:03:36.

He'd stepped into the road two seconds before the red man changed

:03:37.:03:41.

to green. Paul Vowles, here in the middle, denies causing death by

:03:42.:03:44.

careless driving. The most radical reform of the

:03:45.:03:47.

benefits system to take place in decades has arrived in the West

:03:48.:03:50.

From today, single, unemployed job`seekers in Bath and North East

:03:51.:03:55.

Somerset can claim Universal Credit. The authority is one of the pilot

:03:56.:03:59.

areas for the new system which rolls six benefits into one.

:04:00.:04:04.

Winter Olympians from the West are on the final leg of their journeys

:04:05.:04:08.

home after flying in from Sochi today. Bobsleigh couple Paula Walker

:04:09.:04:10.

and John Jackson from Trowbridge joined skeleton gold medallist

:04:11.:04:13.

Lizzie Yarnold, who lives and works in Bath. They touched down at

:04:14.:04:20.

Heathrow earlier this evening. Well, bronze medal winner Jenny Jones

:04:21.:04:24.

arrived home last week. Alistair Durden was with her as she returned

:04:25.:04:41.

to Downend, where she grew up. In Downend, everyone has wanted to

:04:42.:04:46.

pass on their congratulations. Hazlitt convinced you to go

:04:47.:04:53.

snowboarding? Not quite! Soon, word was out that she was back home. And

:04:54.:05:05.

everyone wanted an autograph. It's funny, coming back here. I remember

:05:06.:05:13.

being hit when I was a youngster. `` being here. I didn't think people

:05:14.:05:20.

would recognise me or would know about the Olympics but it has been

:05:21.:05:28.

quite a big thing back here. It s two weeks since Jenny won her

:05:29.:05:32.

bronze, the first this country has ever won on snow, but her journey

:05:33.:05:39.

started in Somerset, where she first got on a snowboard 16 years ago

:05:40.:05:45.

It's not as steep as I remember Maybe that's because I was learning

:05:46.:05:48.

and it was all really scary at the beginning. She has already appeared

:05:49.:05:54.

on chat shows and has become a face people recognise worldwide. I can

:05:55.:06:02.

still cycle to the gym and back without getting recognised. It's

:06:03.:06:08.

nice to be back and be amongst Bristolians. And she will be seeing

:06:09.:06:15.

a lot more of them when the open top bus tour takes place next Monday.

:06:16.:06:18.

There's more news on the BBC Website. Also, don't forget, as part

:06:19.:06:22.

of our World War One At Home series, all this week, your local radio

:06:23.:06:25.

stations are revealing the impact of the war where you live. Tune in at

:06:26.:06:29.

8:15am for that. Now time for the weather.

:06:30.:06:38.

A slightly more settled picture than most of the weather we have enjoyed

:06:39.:06:45.

this winter. Blustery showers and strong winds but better than of

:06:46.:06:52.

late. We will also see some decent spells of sunshine, especially

:06:53.:06:56.

midweek. Rain overnight in the form of this frontal system which pushes

:06:57.:07:01.

through overnight. In its wake, scattered showers for tomorrow.

:07:02.:07:09.

Those winds will be gusting up to 50 mph. Inland as well that is a

:07:10.:07:16.

blustery story. With that cold air, and the wind, even with sunny

:07:17.:07:22.

spells, temperatures will struggle to feel ten and 11. I will leave you

:07:23.:07:26.

with the outlook for the rest of the week. And I'm settled picture.

:07:27.:07:29.

Accord night tomorrow or so. will be more rain overnight into

:07:30.:07:33.

Thursday. Good evening. We have a spell of wet

:07:34.:07:42.

and windy weather working across the British Isles. Today brought many of

:07:43.:07:45.

us some decent spells of sunshine and it brought us some decent

:07:46.:07:50.

temperatures. In fact, our warmest day of the year so far.

:07:51.:07:57.

At the moment, though, things have changed. This is the gap in the

:07:58.:08:03.

satellite picture. Now, this curl of cloud looks like a snail's shell, it

:08:04.:08:07.

is an area of low pressure that is sweeping wind and rain across us.

:08:08.:08:10.

It's pushed into the west already and is now making rapid progress

:08:11.:08:14.

eastwards, moving into eastern England through the small hours

:08:15.:08:21.

Strong, gusty winds around the Irish Sea coasts. The weather front will

:08:22.:08:25.

weaken as it gets into eastern areas. Behind the weather front

:08:26.:08:29.

things are a good deal clearer and that is the way we will start

:08:30.:08:34.

tomorrow. Our wet and windy weather, the worst of it is out of the way as

:08:35.:08:38.

we look at Tuesday. There will be some sunshine around, but there will

:08:39.:08:42.

be some hefty showers. First thing, a lot of sunshine. It is the showers

:08:43.:08:47.

that pack

:08:48.:08:49.
