Good evening. A report into the death of a three year old boy from

Somerset has found the introduction of a new computer system may have

played a part in his death. Samuel Starr, who was born with a heart

condition, died in September 20 2. His parents say delays caused by the

new system meant he hadn't had a cardiac check`up for more than 0

months. As our health correspondent, Matthew Hill, reports, Samuel was

not the only child whose appointment was delayed.

Samuel was born with a heart condition when he was three he had a

cardiac operation. But his parents were concerned that it took longer

than expected. We spoke to Samuel, reading him stories, and he passed

away in our arms. Despite the many questions they had about his

operation in Bristol, their biggest concern was whether his death might

have been prevented if there were not huge delays to his regular

checkups when he was alive. Samuel's local hospital was in Bath.

Sam's condition meant he was supposed to have regular scans to

make sure his condition didn't worsen. Following successful

surgery, his heart was scanned but that turned out to be the last scan

he had for 20 months. Inside out West has discovered a key factor in

these delays was the introduction of a new IT system for hospital

appointments. I am disgusted that it was recognised that no action was

taken on it. In a review of his death, Bristol 's children Hospital

concluded these delays may have played a part. We asked how many

appointments were overdue or delayed because of the new computer system.

They told us there were 63 overdue appointments with some delays of up

to two years before they were discovered. It's very serious. You

are dealing with people's lives and it's essential if you use computer

systems that you have computer systems that work. Inside out West

has also discovered there were 3 overdue appointments because of the

introduction of the computer system, with some taking nearly two

years to discover. We have offered our sincere condolences to the

family of Samuel. The inquest will be held soon and we hope this will

provide both Samuel's family and everybody involved in his care with

a clear indication of the circumstances surrounding his death.

Winter Olympians from the West are on the final leg of their journeys

home after flying in from Sochi today. Bobsleigh couple Paula Walker

and John Jackson from Trowbridge joined skeleton gold medallist

Lizzie Yarnold, who lives and works in Bath. They touched down at

Heathrow earlier this evening. Well, bronze medal winner Jenny Jones

arrived home last week. Alistair Durden was with her as she returned

to Downend, where she grew up. In Downend, everyone has wanted to

pass on their congratulations. Hazlitt convinced you to go

snowboarding? Not quite! Soon, word was out that she was back home. And

everyone wanted an autograph. It's funny, coming back here. I remember

being hit when I was a youngster. `` being here. I didn't think people

would recognise me or would know about the Olympics but it has been

quite a big thing back here. It s two weeks since Jenny won her

bronze, the first this country has ever won on snow, but her journey

started in Somerset, where she first got on a snowboard 16 years ago

It's not as steep as I remember Maybe that's because I was learning

and it was all really scary at the beginning. She has already appeared

on chat shows and has become a face people recognise worldwide. I can

still cycle to the gym and back without getting recognised. It's

nice to be back and be amongst Bristolians. And she will be seeing

a lot more of them when the open top bus tour takes place next Monday.

A slightly more settled picture than most of the weather we have enjoyed

this winter. Blustery showers and strong winds but better than of

late. We will also see some decent spells of sunshine, especially

midweek. Rain overnight in the form of this frontal system which pushes

and windy weather working across the British Isles. Today brought many of

us some decent spells of sunshine and it brought us some decent

is an area of low pressure that is sweeping wind and rain across us.

It's pushed into the west already and is now making rapid progress

eastwards, moving into eastern England through the small hours

Strong, gusty winds around the Irish Sea coasts. The weather front will

weaken as it gets into eastern areas. Behind the weather front

things are a good deal clearer and that is the way we will start

tomorrow. Our wet and windy weather, the worst of it is out of the way as

we look at Tuesday. There will be some sunshine around, but there will
