away in our arms. Despite the many questions they had about his | :01:11. | :01:14. | |
operation in Bristol, their biggest concern was whether his death might | :01:15. | :01:17. | |
have been prevented if there were not huge delays to his regular | :01:18. | :01:23. | |
checkups when he was alive. Samuel's local hospital was in Bath. | :01:24. | :01:31. | |
Sam's condition meant he was supposed to have regular scans to | :01:32. | :01:36. | |
make sure his condition didn't worsen. Following successful | :01:37. | :01:41. | |
surgery, his heart was scanned but that turned out to be the last scan | :01:42. | :01:48. | |
he had for 20 months. Inside out West has discovered a key factor in | :01:49. | :01:53. | |
these delays was the introduction of a new IT system for hospital | :01:54. | :01:58. | |
appointments. I am disgusted that it was recognised that no action was | :01:59. | :02:06. | |
taken on it. In a review of his death, Bristol 's children Hospital | :02:07. | :02:10. | |
concluded these delays may have played a part. We asked how many | :02:11. | :02:15. | |
appointments were overdue or delayed because of the new computer system. | :02:16. | :02:21. | |
They told us there were 63 overdue appointments with some delays of up | :02:22. | :02:25. | |
to two years before they were discovered. It's very serious. You | :02:26. | :02:32. | |
are dealing with people's lives and it's essential if you use computer | :02:33. | :02:37. | |
systems that you have computer systems that work. Inside out West | :02:38. | :02:44. | |
has also discovered there were 3 overdue appointments because of the | :02:45. | :02:47. | |
introduction of the computer system, with some taking nearly two | :02:48. | :02:59. | |
