chaos. Crews are urging people who live in the area to stay indoors | :01:25. | :01:30. | |
and a building has been evacuated. The blaze is at the former Mettoys | :01:30. | :01:34. | |
site, near to the city's park-and- ride at Fforestfach. Our reporter | :01:34. | :01:38. | |

currently 75 firefighters tackling the blaze. I have just come back | :02:06. | :02:10. | |
from the industrial estate. I can tell you of that it is very smoky | :02:11. | :02:15. | |
it down there. I have been speaking to people of poor work there were | :02:15. | :02:21. | |
told me it is not as bad as it was. At lunchtime, it was really bad. | :02:21. | :02:27. | |
There was a thick black smoke hanging over the industrial estate. | :02:27. | :02:32. | |
They say you could not see five feet in front of you. The problem | :02:32. | :02:37. | |
is that the fire has broken out at an old warehouse. People are caught | :02:37. | :02:41. | |
work there told me the old warehouse was used to store tyres | :02:41. | :02:46. | |
and they could be up to eight tens of shredded tyres there. The | :02:46. | :02:51. | |
problem with tackling a blaze like this is it is tackling a rubber | :02:51. | :02:58. | |
fire. You cannot use water. As you can see, read that fires create a | :02:58. | :03:03. | |
lot of smoke. The concern is that sometimes they cannot be in four | :03:03. | :03:07. | |
days. I have been speaking to people caught work on the | :03:07. | :03:14. | |
industrial estate. At about 1 o'clock, they say you could not see | :03:14. | :03:19. | |
anything around Fforestfach. I saw it at 2 o'clock. It was not too bad. | :03:19. | :03:25. | |
It looks like it is getting worse. It was like fog on the main road. | :03:25. | :03:29. | |
It is toxic smoke. They have told the neighbours to keep their | :03:29. | :03:35. | |
windows closed. I could not get in. The roads were blocked. There are | :03:35. | :03:41. | |
police on the bottom, their row police on at the top. | :03:41. | :03:45. | |
We saw a shot of killing cars. Has it caused lot of problems on the | :03:45. | :03:52. | |
roads? Yes. It has caused chaos on the roads. It has been gridlock | :03:52. | :03:57. | |
around Swansea. When I was trying to get close to the fire, I had to | :03:57. | :04:03. | |
walk. There were tailbacks of up to four Myers all down Carmarthen Road. | :04:03. | :04:09. | |
The majority of Carmarthen Road has been shattered. It remains closed. | :04:09. | :04:14. | |
There are no cars coming down here at the moment. It will remain shut | :04:14. | :04:19. | |
until the smoke begins to clear. The main advice for people living | :04:19. | :04:25. | |
in the area is to keep all windows and doors shut. The main concern | :04:25. | :04:30. | |
sure is that even though no one has been injured, the problem is that | :04:30. | :04:37. | |

They include guidance on issues like blood transfusions and some | :04:56. | :05:01. | |

This report, due to be published tomorrow morning, is by Action | :05:06. | :05:11. | |
Against Medical Accidents. It shows how well or not Welsh house -- | :05:11. | :05:17. | |
health board's response to safety alerts. The figures show not a | :05:17. | :05:23. | |
single health board in Wales was 100% compliant with these alerts, | :05:23. | :05:27. | |
as they are meant to be. Some health board complied with less | :05:27. | :05:32. | |
than 50%. The Welsh government requires alerts to be dealt with in | :05:32. | :05:40. | |
a timely way, but in one case, the report was five years over do. | :05:40. | :05:44. | |
Safety is a cornerstone of the modern NHS and when things go wrong, | :05:44. | :05:49. | |
alerts are sent out to help other hospitals avoid making the same | :05:49. | :05:54. | |
mistakes. But information supplied by the Welsh government shows | :05:54. | :05:58. | |
health boards in Wales are routinely slow to respond. All | :05:58. | :06:02. | |
seven have failed to fully implement and alert on the use of | :06:02. | :06:06. | |
oxygen cylinders which should have been completed over one year ago. | :06:06. | :06:10. | |
Guidance on reducing harm from delayed and admitted that medicine | :06:10. | :06:15. | |
has not been fully implemented by six out of seven. Likewise, an | :06:15. | :06:19. | |
alert on emergency blood transfusions had only been fully | :06:19. | :06:23. | |
implemented in one part of Wales. All of the patient safety alerts | :06:23. | :06:28. | |
are vitally important. There are life and death issues. They are | :06:28. | :06:32. | |
only issued about the most important issues, which repeatedly | :06:32. | :06:39. | |
caused harm or even death in the NHS. Not complying by the deadline | :06:39. | :06:43. | |
with ace patient safety and it is putting patients at an unnecessary | :06:43. | :06:47. | |
risk. They are vitally important. The Health Minister Lesley | :06:47. | :06:51. | |
Griffiths was on the wards in Merthyr Tydfil today, an area with | :06:51. | :06:56. | |
one of the lowest compliance rate in Wales. She refused a request to | :06:56. | :07:01. | |
