criticised by the Care Quality Commission after a number of | :01:14. | :01:17. | |
complaints. Now NHS England is investigating Vicky's death. Our | :01:18. | :01:20. | |

it had returned. I had to watch my daughter tell her | :01:43. | :01:51. | |
son that she was dying within two weeks. Then I had got to go and tell | :01:52. | :01:58. | |
my grandson that his mother had died. | :01:59. | :02:05. | |
This video was taken in the days before Becky did. She can be seen | :02:06. | :02:10. | |
talking about the preparations for her funeral. | :02:11. | :02:18. | |
I want to say goodbye and everything. | :02:19. | :02:26. | |
Becky was diagnosed in 2011 but was given the all clear in 2012. `` | :02:27. | :02:30. | |

checkup in January that it became clear the cancer had returned. | :02:47. | :02:53. | |
The GP should have spotted that things were seriously wrong. They | :02:54. | :02:59. | |

Last year, the GP practice was criticised by the Care Quality | :03:07. | :03:08. | |
Commission about the standards of care. | :03:09. | :03:13. | |
I am very concerned about the care that people in Staveley are | :03:14. | :03:19. | |
receiving and this is the most serious in a number of examples. I | :03:20. | :03:27. | |
want to see services improving in Staveley and the wider NHS community | :03:28. | :03:36. | |
improving. The medical group were Vicky went | :03:37. | :03:41. | |
say they cannot comment at the moment. The hospital that treated | :03:42. | :03:47. | |
her seedy treated her symptoms and referred her back to her GP. | :03:48. | :03:56. | |

The results of that approach have worked but many of our region's | :04:54. | :04:59. | |

region does poorly compared with other areas of the country. It is a | :05:36. | :05:40. | |
big challenge but there are tremendous is going on. We have some | :05:41. | :05:46. | |
tremendous teachers and the aim is to use that expertise by moving | :05:47. | :05:52. | |
knowledge around. We want to make the best practice available to as | :05:53. | :05:56. | |
many young people as possible. While there are no easy solutions, | :05:57. | :06:02. | |

very easy to get caught up with people who are really intent on | :12:54. | :13:00. | |

knowledge is about how to keep safe. It is not just one person's | :13:12. | :13:17. | |
responsibility. The people around young people have responsibility. | :13:18. | :13:25. | |
What is safe Internet use? Talking to your children or your | :13:26. | :13:30. | |
grandchildren about what they have been doing online since the minute | :13:31. | :13:35. | |
they have started. Just as when my children were younger and went to | :13:36. | :13:41. | |
the cinema or the theatre I would ask them how it was. You start the | :13:42. | :13:46. | |
dialogue as soon as possible. How common is it for it to spiral | :13:47. | :13:56. | |
