0:07:02 > 0:07:05the kidneys are functioning is looking at a urine sample.

0:08:16 > 0:08:18and then we'll try and pop a needle in
0:08:18 > 0:08:20to get a sample from her bladder directly, OK?
0:08:20 > 0:08:23The ultrasound machine will help the team see inside Isla
0:08:23 > 0:08:26so they can locate her bladder
0:08:26 > 0:08:28and then collect a urine sample using a syringe.
0:08:28 > 0:08:32Do you want to come round, then, and we'll start clipping her tummy.
0:08:32 > 0:08:35This may seem strange but it's the easiest way of getting the sample
0:08:35 > 0:08:39that might help us discover what's wrong with Isla's kidneys.
0:08:40 > 0:08:45- Good girl.- So we want to clip all of this here, right up to here. OK?
0:08:49 > 0:08:51Isla will be sedated but awake for this.
0:08:51 > 0:08:55She's a bit nervous, so Jamie does his best to keep her calm.

0:09:18 > 0:09:20It's full of - you guessed it - donkeys!
0:09:20 > 0:09:25Nearly 80 of them, all arrived here because they needed a good home.
0:09:26 > 0:09:29Rescued donkey Rebecca has a new arrival herself.
0:09:35 > 0:09:38Six days ago, she gave birth to daughter Poppy.
0:09:43 > 0:09:46And Junior Vets Josh and Molly have the pleasure of being
0:09:46 > 0:09:50the first ever vets to treat proud Rebecca's tiny foal.
0:09:50 > 0:09:52- Ah!- Oh, hello.
0:09:52 > 0:09:55Hello, hello.
0:09:55 > 0:09:58You like me? I'm taking this one home.
0:09:58 > 0:10:01It's loving me to bits.
0:10:04 > 0:10:07Newborn foals and their mums can easily become ill
0:10:07 > 0:10:13so my Junior Vets are joining vet Morven from Greenside Vets
0:10:13 > 0:10:17to make sure mum and daughter are fit and healthy.
0:10:17 > 0:10:20They are quite stressed when they're foaling
0:10:20 > 0:10:23and they can be prone to getting infections.
0:10:23 > 0:10:26So, we want to just make sure that everything's back to normal
0:10:26 > 0:10:28and everything is ticking over OK.
0:10:30 > 0:10:33An infection could be a big problem for Rebecca but it would be worse
0:10:33 > 0:10:38for little Poppy, who needs milk to survive and grow strong.
0:10:38 > 0:10:40- Now, can you hear her heartbeat? - Yeah.
0:10:40 > 0:10:44- Should hear a nice "lub-dub, lub-dub".- Yeah.
0:10:44 > 0:10:47Basically, it just sounded like, really, it's a healthy donkey.
0:10:47 > 0:10:50We're going to have a listen to her guts as well.
0:10:52 > 0:10:56And what you want to hear is a good gurgle.

0:10:59 > 0:11:01Sometimes it just goes "gurgle, gurgle"
0:11:01 > 0:11:03and then you might get a big flush, like a toilet.

0:11:06 > 0:11:10I heard really loud sounds coming from Rebecca's stomach,
0:11:10 > 0:11:12which meant that she was healthy.
0:11:12 > 0:11:16Another one! That's a really loud one.
0:11:16 > 0:11:18That's what we want.
0:11:20 > 0:11:24Mum Rebecca is getting a little bit grouchy and Josh has worked out why.
0:11:24 > 0:11:27- Gosh!- Whoa, whoa, whoa.- I think she just wants to see her baby.
0:11:27 > 0:11:31She just moves around fast so she can just keep an eye on it.
0:11:31 > 0:11:33That's exactly right, Josh, yeah.

0:11:39 > 0:11:42So, the last and most important thing to check with the mare

0:11:50 > 0:11:53Hm, a job involving a thermometer and a donkey's bum.
0:11:53 > 0:11:58Who's going to be up for a job like this? Ah, well volunteered, Josh!
0:11:58 > 0:12:01Just pop it up her bum. Just in there.
0:12:01 > 0:12:05I found it funny because I had to just check the stomach
0:12:05 > 0:12:08whilst Josh had to literally really go in.
0:12:08 > 0:12:12This check will give Josh Rebecca's body temperature -
0:12:12 > 0:12:15too high and it might mean the new mum has an infection.
0:12:18 > 0:12:21I just found it really smelly and horrible.
0:12:21 > 0:12:24Like, smells really horrible.
0:12:24 > 0:12:27What is the average level for a donkey?
0:12:27 > 0:12:32It's usually sort of between 37.5 and 39.
0:12:32 > 0:12:35- 36.7.- Okey-doke. Which is probably a wee bit low, actually.
0:12:35 > 0:12:37Low is all right.
0:12:37 > 0:12:41She's obviously doing a good job at keeping herself cool.
0:12:42 > 0:12:46So, no nasty infections but there are still important tests
0:12:46 > 0:12:50to carry out on mum and her foal before both get an all clear.
0:12:51 > 0:12:55Later, donkey work gets all scientific as Josh
0:12:55 > 0:12:58and Molly go hunting for nasty parasites.
0:12:58 > 0:13:01It's like a milkshake, really. I don't think it will taste nice.
0:13:04 > 0:13:07Back at the small animal hospital, Jamie and Nyatepe
0:13:07 > 0:13:12are investigating three-year-old Isla's worrying kidney problem.
0:13:12 > 0:13:15They need to take a pee sample right from Isla's bladder.
0:13:15 > 0:13:17You just want to hold the probe right over there
0:13:17 > 0:13:21and press down slightly, so we can see where the bladder is, OK?
0:13:21 > 0:13:23Nyatepe positions the probe
0:13:23 > 0:13:27so Scott can carefully insert the syringe into Isla's bladder.
0:13:29 > 0:13:33- And then we very gently take the needle out. OK.- Wow!- So, that's us.
0:13:35 > 0:13:37While Isla has a well-earned rest,
0:13:37 > 0:13:42the team head down to the lab to complete Isla's tests.
0:13:42 > 0:13:47One of the things that we look for is how concentrated her urine is.
0:13:47 > 0:13:50Your kidneys should be able to produce concentrated urine
0:13:50 > 0:13:52and if they don't, then that is an indication
0:13:52 > 0:13:54that the kidneys are not working properly.
0:13:56 > 0:13:58First, Jamie uses a gadget called a refractometer
0:13:58 > 0:14:02- to check Isla's wee is the right concentration.- OK?
0:14:02 > 0:14:05Nyatepe then completes further checks...
0:14:05 > 0:14:07Quite a few of them are changing colours.
0:14:07 > 0:14:10- That confirm Isla's wee is a healthy colour.- Great.
0:14:10 > 0:14:14That is a concentrated, normally concentrated urine sample.
0:14:14 > 0:14:17Does that mean that she's good,
0:14:17 > 0:14:20that she can be sent home, or could there be a big shock or...
0:14:20 > 0:14:24It's so difficult because we...you just never know
0:14:24 > 0:14:29and we still have to be prepared for surprises as there always can be.
0:14:29 > 0:14:31After all the tests we did,
0:14:31 > 0:14:33we didn't know what was wrong with her, did we?
0:14:33 > 0:14:36So we decided to put her under an ultrasound scan
0:14:36 > 0:14:40to see what's wrong with her kidneys.
0:14:42 > 0:14:45This scan could give Isla the all clear or uncover a serious problem.
0:14:45 > 0:14:49Again, just comfort her head in there.
0:14:49 > 0:14:53- Make sure she thinks she's OK. - It's OK. Don't worry.
0:14:54 > 0:14:57So, are you OK to turn the lights off? Thanks.
0:14:58 > 0:15:01Jamie thinks he may have spotted Isla's kidney.
0:15:01 > 0:15:03You can sort of, like,
0:15:03 > 0:15:07- see a little circle with something light in the middle.- Very good.
0:15:07 > 0:15:10And actually, that circle that you're seeing in the middle
0:15:10 > 0:15:14of the screen is actually the kidney that we're looking at.
0:15:14 > 0:15:19So, can you notice anything abnormal in the kidney area?
0:15:19 > 0:15:24Scott's expert eye can tell Isla's kidney does not look right.
0:15:24 > 0:15:28It's not exactly as we would expect the kidney to look.
0:15:28 > 0:15:31A healthy kidney would look light on the screen
0:15:31 > 0:15:34but Isla's looks really dark and it's a funny shape.
0:15:34 > 0:15:37Our investigations were all looking very positive
0:15:37 > 0:15:40but the ultrasound, at the moment,
0:15:40 > 0:15:44potentially indicates that this isn't actually 100% normal.
0:15:44 > 0:15:48The scan suggests Isla has kidney disease and sadly,
0:15:48 > 0:15:50this will almost certainly shorten her life.
0:15:52 > 0:15:54After all their efforts,
0:15:54 > 0:15:57it's not the news the team were wanting to hear.
0:15:57 > 0:15:59Does this mean we might have to keep her in for a bit longer?
0:15:59 > 0:16:02I don't think we're going to have to keep her in today,
0:16:02 > 0:16:05but it does mean we are not going to be able to necessarily give
0:16:05 > 0:16:07her the clean bill of health that we wanted to.
0:16:07 > 0:16:11I think this does show the benefit of doing what we've done today.
0:16:15 > 0:16:19- Now then, Isla. Is it all right to stroke her?- Yes, of course.
0:16:19 > 0:16:21- How is she?- It was, like, really positive...
0:16:21 > 0:16:24- When we first did the wee. - ..before we did the ultrasound.
0:16:24 > 0:16:28- But then we did the ultrasound... - And we discovered something wrong.
0:16:30 > 0:16:33Part of vet work is obviously,
0:16:33 > 0:16:37you know, things aren't going as well as expected.
0:16:37 > 0:16:40- At least you picked this up when the dog was still healthy.- Exactly.
0:16:40 > 0:16:43I think it just shows you've got to always keep your mind open
0:16:43 > 0:16:46and make sure you're being as thorough as possible.
0:16:46 > 0:16:48We just have to do the best we can
0:16:48 > 0:16:51and just hope she stays well for as long as possible.
0:16:52 > 0:16:54It's been a challenging day for everyone.
0:16:54 > 0:16:57How did my Junior Vets cope, Scott?
0:16:57 > 0:16:59I think they did really well, actually.
0:16:59 > 0:17:02I think having to deal with the slightly more negative news
0:17:02 > 0:17:04towards the end of the day is really difficult
0:17:04 > 0:17:06and I still struggle with that.
0:17:06 > 0:17:09And I think, for them, being at the stage they're at,
0:17:09 > 0:17:12I think they coped with it very well.
0:17:12 > 0:17:16If you had to pick one, which would you pick as the best of the day?
0:17:16 > 0:17:19I would say Jamie was, for me, a standout.
0:17:19 > 0:17:21I can see him being a really good little communicator,
0:17:21 > 0:17:26so I think if he did, you know, decide to go and be a vet,
0:17:26 > 0:17:29I think he would be great from that point of view.
0:17:33 > 0:17:36At the donkey sanctuary,
0:17:36 > 0:17:39Josh and Molly have been getting all vetty with new mum Rebecca.
0:17:39 > 0:17:42Now they're about to complete her six-day-old foal's
0:17:42 > 0:17:44first ever vet check.
0:17:48 > 0:17:51With it being newborn, it's not that used to being held as well,
0:17:51 > 0:17:54- so it might be a bit of a wriggler for us.- Aw!

0:18:04 > 0:18:07Occasionally they get a bit of pigment on the inside of their eyes.

0:18:11 > 0:18:14Now, one more important check to do
0:18:14 > 0:18:16and this one can't be done by examining Poppy or her mum.
0:18:16 > 0:18:19A clue - it's a bit of a stinker.
0:18:19 > 0:18:25Now, this is a nice, fresh poo sample and what we're going to do
0:18:25 > 0:18:29is take a little bit of it to see if there's any worm eggs in there.
0:18:29 > 0:18:32It stinks.
0:18:32 > 0:18:35If they find worm eggs in it, it means mum Rebecca has tiny
0:18:35 > 0:18:39parasites living inside her which can make her feel very ill.
0:18:40 > 0:18:42Oh, my gosh! Squidgy!
0:18:44 > 0:18:47Worms can be life-threatening for an adult donkey and they can
0:18:47 > 0:18:51affect the quality of her milk and that would harm her foal, Poppy.
0:18:51 > 0:18:54I don't think being a vet is very glamorous.
0:18:54 > 0:18:57- I don't think I could do this every day.- I could.
0:19:02 > 0:19:05It stinked really, really bad.
0:19:05 > 0:19:07Okey-doke, guys. That's us got enough poo.
0:19:07 > 0:19:10And we'll have a look at this under the microscope and see
