0:00:02 > 0:00:07- Ysbyty Gwynedd has been caught up - in Hurricane Ophelia's high winds.

0:00:11 > 0:00:13- But even in the roughest weather...

0:00:14 > 0:00:15- It's windy!

0:00:16 > 0:00:18- ..the nurses cope.

0:00:18 > 0:00:22- Many have had to go home - because of the wind...

0:00:22 > 0:00:24- ..and the bridge is closed.

0:00:24 > 0:00:26- We shared a car, didn't we?

0:00:26 > 0:00:27- We shared a car, didn't we?- - Yes.

0:00:27 > 0:00:31- I was traumatised after sitting - in your passenger seat!

0:00:32 > 0:00:37- Despite everything, the patients - continue to arrive on Dewi Ward.

0:00:38 > 0:00:42- A three-month-old boy with breathing - problems is coming straight here.

0:00:43 > 0:00:46- These tonsils are massive.

0:00:51 > 0:00:54- I felt it straight away - and heard it break as well.

0:01:03 > 0:01:07- Every morning on Dewi Ward - tends to be the same.

0:01:08 > 0:01:10- The night shift comes to an end.

0:01:10 > 0:01:12- See you later. Ta-ta.

0:01:13 > 0:01:17- The day shift takes over - and the work starts again.

0:01:20 > 0:01:23- We've been quite busy overnight.

0:01:23 > 0:01:26- We've had a lot of admissions - but most have gone home...

0:01:26 > 0:01:28- ..after seeing the doctor.

0:01:28 > 0:01:32- I think we only have about - two new patients this morning.

0:01:32 > 0:01:35- So it's not too bad - for the day staff.

0:01:35 > 0:01:38- And I'm looking forward - to seeing my bed!

0:01:44 > 0:01:48- It's nine in the morning. - This is the assessment unit.

0:01:48 > 0:01:53- Every child that is referred to us - from their GP or from A&E...

0:01:53 > 0:01:57- ..comes in here first - to be seen by a doctor.

0:02:01 > 0:02:04- This unit's been here - for about a year.

0:02:04 > 0:02:06- It's been a great help for the ward.

0:02:06 > 0:02:10- It stops the ward - from getting full of children...

0:02:11 > 0:02:15- ..who don't need admission - but need time to get better...

0:02:15 > 0:02:20- ..or time to decide whether - they're well enough to go home...

0:02:20 > 0:02:22- ..or if they need more treatment.

0:02:23 > 0:02:25- But with Hurricane Ophelia...

0:02:25 > 0:02:28- ..and the highest winds - in a century on the way...

0:02:29 > 0:02:32- ..this is not a normal day - for Ysbyty Gwynedd.

0:02:35 > 0:02:39- We might have fewer children - coming in...

0:02:39 > 0:02:41- ..because they may not go - to the surgery.

0:02:41 > 0:02:45- The biggest challenge for us - will be staff needing to go home.

0:02:47 > 0:02:51- But one little boy has managed - to arrive before the storm.

0:02:52 > 0:02:56- Ophelia can't come between - Calum and his tonsils.

0:02:58 > 0:03:03- He's been in and out to see - the doctor for about five years.

0:03:03 > 0:03:08- His tonsils touch each other. - He always has a secondary infection.

0:03:08 > 0:03:13- The first time, it was in the knee. - The second time, the hip.

0:03:13 > 0:03:16- Last week, he had a secondary - infection on the bowel.

0:03:17 > 0:03:19- That created some problems for him.

0:03:19 > 0:03:22- So we're happy - that he's having them taken out.

0:03:22 > 0:03:25- My tonsils are massive.

0:03:26 > 0:03:28- But the storm is even bigger.

0:03:28 > 0:03:32- She has her eye on the toys - in the Dewi Ward play area.

0:03:36 > 0:03:40- We'll put all the toys in the corner - before they go out of the gate.

0:03:54 > 0:03:57- It's windy!

0:04:00 > 0:04:03- But Ophelia - is only just getting started.

0:04:04 > 0:04:07- I've had a text message - from the school...

0:04:07 > 0:04:09- ..to say - they're closing at one o'clock.

0:04:10 > 0:04:13- Unfortunately, I can't hold - of their dad or their gran...

0:04:13 > 0:04:17- ..so I'll have to pick up the girls, - drop them off at their gran's...

0:04:17 > 0:04:19- ..and then come back to work.

0:04:26 > 0:04:29- Other members of staff - can't come in...

0:04:29 > 0:04:33- ..because they haven't got anyone - to look after their children.

0:04:33 > 0:04:35- So we're quite short-staffed.

0:04:38 > 0:04:42- This nurse has just enough time - to take Calum to theatre...

0:04:42 > 0:04:46- ..before having to pick up - her own child from school.

0:04:46 > 0:04:49- But on a day like today, - everyone understands.

0:04:50 > 0:04:52- The staff have been really good.

0:04:52 > 0:04:55- They've had problems - with the weather.

0:04:55 > 0:04:58- Many have had to go home - because of the wind...

0:04:58 > 0:05:00- ..and the bridge is closed.

0:05:00 > 0:05:04- But everyone who's here - has been fantastic.

0:05:07 > 0:05:12- The GP has sent Andrew in because - he suspects he has appendicitis.

0:05:12 > 0:05:17- The poor lad brings a bucket with - him because he can't stop vomiting.

0:05:22 > 0:05:25- Claire brings Andrew to a bed.

0:05:25 > 0:05:29- She doesn't want an old paint bucket - on her ward.

0:05:30 > 0:05:31- Which nurse would?!

0:05:32 > 0:05:36- But she can't get rid of it - because Andrew's dad...

0:05:36 > 0:05:39- ..says the bucket - is a family heirloom.

0:05:39 > 0:05:41- Hmm!

0:05:41 > 0:05:43- There you go. OK?

0:05:43 > 0:05:45- Shall I grab that for you?

0:05:45 > 0:05:48- They've used it all their lives - as a vomit bowl...

0:05:48 > 0:05:51- ..but you could get rid - of the contents for us.

0:05:51 > 0:05:54- Be careful with that bucket, Claire.

0:05:55 > 0:05:57- I thought I just had a bug.

0:05:58 > 0:06:05- I have a little bit of pain - and I feel a bit sick as well.

0:06:06 > 0:06:07- There we are.

0:06:12 > 0:06:16- Calum is feeling a bit light-headed - after having his tonsils out.

0:06:16 > 0:06:19- But moving beds - can make the world spin.

0:06:22 > 0:06:23- Aah!

0:06:26 > 0:06:27- Aah!

0:06:29 > 0:06:31- My lips are so dry.

0:06:31 > 0:06:33- My lips are so dry.- - Are you OK to come across here?

0:06:34 > 0:06:36- I think so. Waa!

0:06:36 > 0:06:37- Whoa.

0:06:38 > 0:06:40- Nice and careful there.

0:06:40 > 0:06:42- Dizzy?!

0:06:42 > 0:06:44- Well done.

0:06:45 > 0:06:47- You have a lie-down now.

0:06:47 > 0:06:49- All the best now.

0:06:49 > 0:06:52- Thank you.

0:06:55 > 0:06:57- I don't want to...

0:06:57 > 0:06:58- Try and go back to sleep.

0:06:59 > 0:06:59- I want to stay with you.

0:06:59 > 0:07:02- I want to stay with you.- - I'm staying here. I'm staying here.

0:07:05 > 0:07:08- The night shift's - come in early to help...

0:07:08 > 0:07:11- ..because so many nurses - had to go home.

0:07:12 > 0:07:15- Everyone has their story.

0:07:15 > 0:07:19- A Tesco Bag for Life - hit my windscreen.

0:07:19 > 0:07:21- That's it really.

0:07:21 > 0:07:24- That's the only traumatic thing - that happened.

0:07:24 > 0:07:28- The electricity in the house went - off about an hour before I came in.

0:07:29 > 0:07:33- My dinner was in the oven and I - wanted a shower before coming in.

0:07:33 > 0:07:36- So I was a bit behind - and it was a bit of a rush.

0:07:37 > 0:07:39- Then it was windy as well.

0:07:39 > 0:07:43- I haven't been driving for long - so I was a bit nervous driving in.

0:07:43 > 0:07:45- What did you have to eat?

0:07:45 > 0:07:48- I had salmon in the oven.

0:07:49 > 0:07:52- And the new potatoes - were a little bit on the raw side.

0:07:54 > 0:07:57- So it was a bit of a rush - to come in.

0:07:57 > 0:07:58- We shared a car, didn't we?

0:07:58 > 0:07:59- We shared a car, didn't we?- - Yes.

0:08:00 > 0:08:04- I was traumatised after sitting - in your passenger seat.

0:08:04 > 0:08:06- No!

0:08:07 > 0:08:12- The wind has stopped people from - travelling and seeing their GP...

0:08:12 > 0:08:14- ..so it's quiet on Dewi Ward.

0:08:19 > 0:08:21- But Andrew is still here.

0:08:21 > 0:08:26- The surgeons have seen him and they - agree he must have his appendix out.

0:08:27 > 0:08:30- There's a toilet here.

0:08:31 > 0:08:34- He came in - about three o'clock this afternoon.

0:08:34 > 0:08:39- He's had blood taken - and he's been nil by mouth.

0:08:40 > 0:08:43- They've given him IV antibiotics - already.

0:08:43 > 0:08:45- He's been in a bit of pain.

0:08:45 > 0:08:48- So he's had some painkillers.

0:08:48 > 0:08:51- He's also been having IV fluids.

0:08:51 > 0:08:54- I think they'd planned to take him - to theatre this afternoon.

0:08:55 > 0:08:59- But it was about ten o'clock tonight - when the doctor came to see him.

0:08:59 > 0:09:02- They're hoping to take him - to theatre tonight.

0:09:03 > 0:09:06- For those children in real need...

0:09:06 > 0:09:09- ..there's always a way - of getting to Dewi Ward.

0:09:10 > 0:09:11- It's 10.55pm.

0:09:11 > 0:09:15- We've just had a phone call - about a three-month-old baby...

0:09:15 > 0:09:17- ..with breathing difficulties.

0:09:17 > 0:09:21- They're coming straight to the ward - in the ambulance.

0:09:21 > 0:09:24- That's all we know at the moment - so we'll see.

0:09:26 > 0:09:29- This little man is Logan.

0:09:29 > 0:09:33- He's given his mother a fright - and he's only 12 weeks old.

0:09:39 > 0:09:41- And he didn't improve at all?

0:09:41 > 0:09:44- No. He's been the same all day.

0:09:48 > 0:09:51- He started to get ill last night.

0:09:52 > 0:09:56- I waited until this morning and then - I took him in. He had a high temp.

0:09:57 > 0:09:58- He was crying in pain.

0:09:58 > 0:10:01- I had a lift up this morning.

0:10:04 > 0:10:08- I was home until about 4.30pm.

0:10:09 > 0:10:12- Then he started getting worse.

0:10:14 > 0:10:18- I've assessed him. He looks OK.

0:10:19 > 0:10:23- Mam was a bit worried - because he wasn't drinking much...

0:10:23 > 0:10:25- ..was a bit snuffly and had a cough.

0:10:25 > 0:10:29- He's got progressively worse - all day.

0:10:29 > 0:10:33- So we're just waiting now - for the doctor to assess him...

0:10:33 > 0:10:36- ..and we'll see then - what the next step will be.

0:10:38 > 0:10:43- While Logan waits to see the doctor, - and as the clock passes midnight...

0:10:43 > 0:10:47- ..Andrew is on his way to theatre - to have his appendix out.

0:10:47 > 0:10:51- They need to wrap up warm - to protect from Ophelia.

0:10:56 > 0:10:56- .

0:10:59 > 0:10:59- Subtitles

0:10:59 > 0:11:01- Subtitles- - Subtitles

0:11:05 > 0:11:10- Two days after Hurricane Ophelia - and Andrew is still in hospital.

0:11:15 > 0:11:20- They found that the end - of the appendix was gangrenous.

0:11:21 > 0:11:24- He was there for about two hours - in total.

0:11:25 > 0:11:29- He came back and now he has to be - on antibiotics for 48 hours...

0:11:29 > 0:11:32- ..to make sure - he doesn't have an infection.

0:11:35 > 0:11:39- He's finding it difficult - to eat at the moment.

0:11:39 > 0:11:43- During the treatment, - they handled his bowels...

0:11:43 > 0:11:48- ..so it takes a while for everything - to get back to normal.

0:11:50 > 0:11:52- There we go.

0:11:55 > 0:11:59- Someone else who's been having - trouble eating is Seren.

0:11:59 > 0:12:01- But for completely - different reasons.

0:12:02 > 0:12:06- She's been feeling light-headed - and vomiting since first thing.

0:12:06 > 0:12:09- It's because her diabetes - is out of control.

0:12:13 > 0:12:16- Her blood sugar levels are too high.

0:12:16 > 0:12:19- Her pancreas - doesn't produce insulin...

0:12:19 > 0:12:23- ..so her body can't use the sugar - to produce energy.

0:12:23 > 0:12:27- Instead, the cells use fat...

0:12:27 > 0:12:31- ..and that creates ketones - which poison her body.

0:12:33 > 0:12:37- One of the first tests we do - is what we call blood gas.

0:12:38 > 0:12:43- We can then see how acidotic - or alkalotic the blood is.

0:12:43 > 0:12:47- Seren's blood had started to - creep up to be more acidotic.

0:12:48 > 0:12:50- It's normally neutral.

0:12:53 > 0:12:58- She was breathing a bit differently. - She was sort of panting.

0:12:58 > 0:13:02- That means that the level of C02 - in her blood is raised.

0:13:02 > 0:13:05- Her body's way of trying to - get rid of the C02...

0:13:06 > 0:13:08- ..is to make her breathe like that.

0:13:08 > 0:13:11- The ketones - that make the blood acidotic...

0:13:11 > 0:13:14- ..affect many organs - and systems in the body.

0:13:15 > 0:13:21- Seren has diabetes type 1, - an incurable genetic condition.

0:13:21 > 0:13:24- Her family use - the latest technology...

0:13:24 > 0:13:26- ..to keep it under control.

0:13:26 > 0:13:30- She has an insulin pump - so instead of injections...

0:13:30 > 0:13:34- ..she wears this pump which gives - a constant infusion of insulin.

0:13:35 > 0:13:36- She has to manage it.

0:13:36 > 0:13:39- She also has another machine - on her body...

0:13:39 > 0:13:42- ..called continuous glucose monitor.

0:13:42 > 0:13:46- It's a sensor under the skin - which measures the blood sugar.

0:13:46 > 0:13:49- The reading from that - goes to her phone.

0:13:49 > 0:13:52- Seren is responsible for this - - not me.

0:13:52 > 0:13:56- She has to work out all the time - what she needs to do...

0:13:56 > 0:14:00- ..and whether she needs more - insulin, more sugar, less insulin.

0:14:00 > 0:14:03- She has to work out - the carb content of her food.

0:14:03 > 0:14:05- As you can imagine...

0:14:05 > 0:14:10- ..it's quite a big responsibility - for a young woman.

0:14:10 > 0:14:14- But she does a fantastic job - and we're very proud of her...

0:14:14 > 0:14:16- ..for being so responsible.

0:14:18 > 0:14:21- It's supposed to be between 4 and 8.

0:14:21 > 0:14:23- So let's check Seren's phone.

0:14:24 > 0:14:27- As you can see, it's 18.7.

0:14:27 > 0:14:29- This can be read on any phone...

0:14:29 > 0:14:32- ..so when my husband's - working in Botswana...

0:14:32 > 0:14:36- ..he can check Seren's - blood sugar levels.

0:14:36 > 0:14:39- When you come home - with a child who has diabetes...

0:14:39 > 0:14:42- ..it's like coming home - with a new baby.

0:14:42 > 0:14:46- Diabetes type 1 is completely - different from type 2.

0:14:46 > 0:14:49- You can't cure type 1 diabetes, - just manage it.

0:14:50 > 0:14:52- Peer support is so important.

0:14:52 > 0:14:56- We're part of a group for parents - and carers in North Wales.

0:14:56 > 0:15:00- There are around 100 children - under 18 who have diabetes type 1.

0:15:00 > 0:15:04- We meet up every month, - we help each other.

0:15:04 > 0:15:09- I'm going on a girls' night out - with about ten other mothers...

0:15:09 > 0:15:12- ..this weekend - - if Seren is well enough, of course!

0:15:12 > 0:15:15- It's an important part of our life.

0:15:16 > 0:15:19- Rugby is what's important - to Gethin.

0:15:19 > 0:15:22- That's why he's here - for the second time.

0:15:26 > 0:15:29- Last April, I broke my leg - in a rugby match.

0:15:29 > 0:15:33- I've just gone back to playing rugby - about two or three weeks ago.

0:15:33 > 0:15:36- I've just broken it again - in training last night.

0:15:36 > 0:15:40- I felt it straight away - and heard it break as well.

0:15:40 > 0:15:42- I've broken both bones now.

0:15:42 > 0:15:45- I knew straight away I'd broken it.

0:15:48 > 0:15:53- Seren's father also knows straight - away if his daughter's in trouble.

0:15:53 > 0:15:55- Thanks to her mobile phone.

0:15:55 > 0:16:01- Once you go below the red line, 4, - you go into hypoglycaemia.

0:16:01 > 0:16:07- You need to aim for it to be - between the red line and number 10.

0:16:07 > 0:16:10- You don't want it to go much over 10 - in the long term.

0:16:11 > 0:16:16- People like us who don't - have diabetes are around 7.

0:16:16 > 0:16:19- But for someone - with type 1 diabetes...

0:16:19 > 0:16:22- ..the blood can be - all over the place.

0:16:22 > 0:16:26- It can be too low, hypoglycaemia, - or too high, hyperglycaemia.

0:16:26 > 0:16:30- So now, you can see - that her blood is at 13.

0:16:30 > 0:16:33- So if we input that information - on the machine...

0:16:34 > 0:16:37- ..it will work out - whether she needs a correction dose.

0:16:37 > 0:16:40- Yes. It says 1.40 units.

0:16:40 > 0:16:42- Say yes, Seren.

0:16:43 > 0:16:44- Confirm.

0:16:44 > 0:16:49- So the machine now automatically - tells the insulin pump...

0:16:49 > 0:16:52- ..that she needs 1.40 units - of insulin.

0:16:52 > 0:16:55- They explain that type 1 diabetes...

0:16:55 > 0:16:59- ..is like walking along a tightrope - with a blindfold...

0:16:59 > 0:17:02- ..with a spoonful of sugar - in front of you.

0:17:02 > 0:17:04- That's the balancing act required...

0:17:04 > 0:17:07- ..to make sure your sugar levels - don't go too high...
