:00:15. > :00:22.involved in the accident on the outskirts of Cookstown. Conor

:00:23. > :00:28.Macauley reports. The driver of the car lost control,

:00:29. > :00:34.his Citroen crashed through two Gardens. It came to rest on its

:00:35. > :00:39.side. The driver and passenger, both men in their 20s, were killed. They

:00:40. > :00:46.have been named as a 23-year-old, Grant archer and a 20 H old Robert

:00:47. > :00:51.Wilkinson. Before four o'clock this morning, police received an

:00:52. > :00:55.emergency call and the emergency services were dispatched and the

:00:56. > :00:56.Fire Service and ambulance. Officers were first on the scene and

:00:57. > :01:02.Fire Service and ambulance. Officers by a scene of carnage.

:01:03. > :01:11.The double death has shocked the town. It is a tragedy to befall this

:01:12. > :01:16.town, particularly on Mother's Day. Our prayers and thoughts are with

:01:17. > :01:19.the family. The road has been reopened to traffic. Police are

:01:20. > :01:25.appealing for any witnesses to come forward.

:01:26. > :01:28.The family of one of the 11 patients whose deaths were investigated by

:01:29. > :01:30.the Northern Health Trust has accused the organisation of

:01:31. > :01:37.deliberately misleading them about the details surrounding their

:01:38. > :01:46.father's death. Neil Cormican, who was 81, died in April 2010 at Antrim

:01:47. > :01:48.Area Hospital. Speaking to the BBC, the family said there was a

:01:49. > :01:51.catalogue of errors including the family said there was a

:01:52. > :01:53.hospital not reporting the case to the coroner. Our health

:01:54. > :01:59.correspondent Marie-Louise Connolly reports.

:02:00. > :02:03.On Friday, it emerged 11 deaths are under investigation at the Northern

:02:04. > :02:08.Health Trust, among the cases are five babies. According to a review,

:02:09. > :02:14.care was below standard. The BBC has been contacted by one of the

:02:15. > :02:18.families. In 2010 Neil Cormican was given the wrong medication which

:02:19. > :02:23.triggered a massive heart attack. Neil Cormican died but his death was

:02:24. > :02:29.not the ported to the coroner. As far as we are concerned, there has

:02:30. > :02:35.been no openness, no transparency, any information we have got we have

:02:36. > :02:42.had to pursue them for it. We have had to go after them, nothing has

:02:43. > :02:45.been forthcoming. At being quest, it emerged the death had not been

:02:46. > :02:51.immediately reported as a serious adverse incidents and it emerged the

:02:52. > :02:56.medical director had advised the family against having a postmortem.

:02:57. > :03:05.There is a systematic problem in how we are delivering health which is

:03:06. > :03:09.having negative on patient safety. According to the Northern Health

:03:10. > :03:12.Trust they recorded the death as an advert incident but it was escalated

:03:13. > :03:19.to a Serious Adverse Incidents six months later. More on that story

:03:20. > :03:22.tomorrow morning. The police have released CCTV

:03:23. > :03:27.footage of a West Belfast pizza delivery driver, minutes before he

:03:28. > :03:31.was murdered a year ago. It's part of a new appeal about the killing of

:03:32. > :03:39.Kieran McManus outside Domino's Pizza on Kennedy Way. Kevin Sharkey

:03:40. > :03:45.reports. One of the final act in the life of

:03:46. > :03:49.Kieran McManus, 26 years old, a father and minutes from being

:03:50. > :03:54.murdered. Shot a number of times at his place of work. The killing was

:03:55. > :03:59.witnessed by many people including others who worked here but the

:04:00. > :04:05.gunman and the driver wearing masks. They were never traced. The new

:04:06. > :04:10.appeal about the maroon Vauxhall Astra. Did you see the vehicle? Did

:04:11. > :04:15.you see it in the days and weeks prior to the murder, did you see it

:04:16. > :04:17.being set on fire? These are among the first few steps of Kieran

:04:18. > :04:24.McManus leaving the shop I'm going to his death.

:04:25. > :04:29.Now just time for the weather forecast with Cecilia Daly. Hello,

:04:30. > :04:33.hopefully you have had a nice weekend. It looks like the night

:04:34. > :04:39.will be quiet, dry in most places, quite clear at times but fairly

:04:40. > :04:44.mild. It will be rather misty and hazy by Monday morning. The start of

:04:45. > :04:47.the week, the last day of March, not too bad but there will be a growing

:04:48. > :04:53.risk of heavy showers especially later in the day. When the morning

:04:54. > :04:59.mist lifts, hazy sunshine, one of two showers developing in the second

:05:00. > :05:04.half of the morning but some places they dry. The showers edge

:05:05. > :05:08.northwards across Northern Ireland, extending into north-west England

:05:09. > :05:12.and the Midlands. Another area of showers over the south-west of

:05:13. > :05:17.Britain moving across the Irish Sea into southern islands. Many are the

:05:18. > :05:26.places they dry, the North Sea coast will be cold, cloudy and misty. In

:05:27. > :05:30.the afternoon, showers leave the north coast, drier weather for a

:05:31. > :05:37.time, quite mild in the sunshine, 14 or 15. This area of rain looks like

:05:38. > :05:43.it could be heavy and sundry in places for the evening tomorrow.

:05:44. > :05:47.Some standing water in the night. It clears away by Tuesday morning and

:05:48. > :05:51.Tuesday looks like being dry. Overall this week, unsettled,

:05:52. > :05:56.further heavy rain to come and breezy at times, especially in

:05:57. > :06:00.East. Many parts of Britain and Ireland have a dry day, this rain

:06:01. > :06:04.approaching southern island will move northwards across most of

:06:05. > :06:09.Ireland on Wednesday affecting western parts of Britain. A split,

:06:10. > :06:13.further east it will be dry and warm, further rain in the west on

:06:14. > :06:19.further east it will be dry and Thursday. The North Sea coast will

:06:20. > :06:23.be cold and damp. For Northern Ireland, driest day on Tuesday,

:06:24. > :06:25.further rain on Wednesday and Thursday.

:06:26. > :06:37.Our next BBC Newsline bulletin is at 6:20am in the morning.
