:01:16. > :01:20.In 2012, the safeguarding board was established. Made up of
:01:21. > :01:24.representatives including the police, health trusts and social
:01:25. > :01:26.services, it aimed to establish greater communication between
:01:27. > :01:32.different groups sought to better protect children. However, an
:01:33. > :01:35.independent review led by Professor Alexis Jay said the board failed in
:01:36. > :01:39.what it was asked to do. It is a highly critical report which I led
:01:40. > :01:43.to the board spent too much time on wrong issues, that tensions existed
:01:44. > :01:49.between those at the top and a lack of structure prevented vital
:01:50. > :01:51.information being shared. On that board, if the medical profession
:01:52. > :01:54.agreed to share with trusts, with everybody else, for example the
:01:55. > :01:58.mental health well-being of the parents and then that parent becomes
:01:59. > :02:01.unwell, that can share that information with the social worker
:02:02. > :02:04.who can name, for example, take preventative action. It is that
:02:05. > :02:07.critical sharing of information which makes is able to work more
:02:08. > :02:11.preventative late, rather than waiting until the incident happens.
:02:12. > :02:17.While the review does not say that children were harmed as a result of
:02:18. > :02:20.poor practice, it does say that the proper mechanisms were not in place
:02:21. > :02:24.for properly recording children's deaths. What she says in this report
:02:25. > :02:29.is that one, the child death overview panel had not been set up
:02:30. > :02:32.but secondly there are too many processes and a lot of confusion
:02:33. > :02:35.about who gets what information when a child dies, particularly when it
:02:36. > :02:41.is unexpected. That has to be streamlined and it has to be clearer
:02:42. > :02:45.what happens. The review makes 11 recommendations, including that the
:02:46. > :02:47.board is notified of all deaths and children who have been significantly
:02:48. > :02:54.harmed. Child protection must be clearly prioritised. A review of
:02:55. > :02:57.senior staff roles and grades should be undertaken immediately. And
:02:58. > :03:02.learning must be disseminated more quickly. The Department of Health
:03:03. > :03:03.says it has asked for all the recommendations to be fermented
:03:04. > :03:08.without delay. -- to be implemented. There has been a mixed reaction
:03:09. > :03:11.to the news that the Treasury will commit to guarantee direct
