:00:46. > :00:48.with learning and physical disabilities. Care Quality
:00:49. > :00:54.Commission inspectors visitdd the home in January. If found lots of
:00:55. > :00:56.good care by staff but also became aware of serious problems whth
:00:57. > :01:01.management and staffing levdls which compromised the care and safety of
:01:02. > :01:07.patients. What were the major concerns?
:01:08. > :01:10.On multiple occasions inspectors were told of a disconnect bdtween
:01:11. > :01:13.management and staff which leant concerns and issues were not
:01:14. > :01:15.followed up. Records were not updated or properly maintained.
:01:16. > :01:18.There were three incidents which had not been properly investigated.
:01:19. > :01:20.Including one person who had been able to leave the building tnseen
:01:21. > :01:23.and unsupervised. This isn't the first time this
:01:24. > :01:27.organisation has had problels. No, this report comes just over a
:01:28. > :01:30.week after Southern Health NHS Foundation Trust, which runs the
:01:31. > :01:37.site was, criticised for thd way it is running Slade House in
:01:38. > :01:40.Headington. Again, a home for people with learning disabilities ` it was
:01:41. > :01:42.found a lack of effective management, leadership and staff
:01:43. > :01:45.training contributed to the death of 18`year`old patient Connor
:01:46. > :01:50.Sparrowhawk which could havd been prevented.
:01:51. > :01:53.What next? Piggy Lane has failed its inspection
:01:54. > :01:57.and a formal warning has bedn issued against the Trust. They now have
:01:58. > :02:00.until next week to detail how they're going to improve. In a
:02:01. > :02:03.statement they said they fotnd the outcome of the report unaccdptable `
