:00:58. > :01:00.calling for a charge of corporate manslaughter to be brought `gainst
:01:01. > :01:03.an NHS Trust she holds responsible for the death of her teenagd son.
:01:04. > :01:07.Connor Sparrowhawk died at Slade House in July last year. In her
:01:08. > :01:11.first television interview, his mother Sara Ryan has spoken of her
:01:12. > :01:16.campaign to improve care for vulnerable adults. Peter Cooke has
:01:17. > :01:25.been to meet her. It's enorlously painful. The sadness didn't go away
:01:26. > :01:29.at all. It is something you have got to live with. Sara Ryan says she's
:01:30. > :01:33.still seeking justice for hdr son Connor Sparrowhawk. They had a duty
:01:34. > :01:38.of care to look after him and keep him alive. And they didn't. The
:01:39. > :01:43.18`year`old drowned in a bath at Slade House in Headington l`st July.
:01:44. > :01:46.A report found the unit for people with learning difficulties had
:01:47. > :01:49."failed significantly" in hhs care, and left him unsupervised ddspite
:01:50. > :01:52.his epilepsy. Southern Health NHS Foundation Trust, which runs the
:01:53. > :02:01.unit, closed it in November pending a review. But does Sarah thhnk
:02:02. > :02:04.that's enough? The chief exdcutive needs to take responsibilitx for
:02:05. > :02:09.what's happened. You shouldn't have that level of disarray and chaos on
:02:10. > :02:19.your watch. What would you like to see happen? Hopefully shame them to
:02:20. > :02:23.the point that other trusts think, we need to look at our own house
:02:24. > :02:27.here and make sure that thex aren't falling into the same traps that
:02:28. > :02:31.they did. Today she was at ` radio station near Wantage which supports
:02:32. > :02:34.groups with learning diffictlties. Sarah is also launching a c`mpaign
:02:35. > :02:38.this week to highlight poor care provision. She thinks Southdrn
:02:39. > :02:49.Health should face corporatd manslaughter charges. But to what
:02:50. > :02:58.end? Some visible, tangible change. I think Winterborne throughout all
:02:59. > :02:59.sorts of outrage in shock and two years on, there has not been any
:03:00. > :03:18.change at all. Sarah Ryan talking to Peter Cooke.
:03:19. > :03:21.Sue Harriman is the deputy Chief Executive of Southern Health NHS
:03:22. > :03:24.Foundation Trust ` which runs Slade House. I asked what her response was
:03:25. > :03:30.to a possible corporate manslaughter charge. Firstly what I would like to
:03:31. > :03:35.do is repeat the unreserved apology that we gave to the family of Connor
:03:36. > :03:40.Sparrowhawk. We let him and his family down and for that, wd
:03:41. > :03:42.apologise. We fully accept the findings of the independent
:03:43. > :03:47.investigation. We have made significant changes since that time.
:03:48. > :03:54.What is important to us is that we share the learning and we are keen
:03:55. > :03:58.to open a dialogue with Sar`h Ryan. We have tried to open dialogue from
:03:59. > :04:05.the beginning. We appreciatd for her, that the time only to be
:04:06. > :04:12.right, but when she wants to talk to us, we are keen to engage. What
:04:13. > :04:18.changes have been made? We did a 72 hour investigation after his death,
:04:19. > :04:20.and then it became clear th`t we needed to make improvements in the
:04:21. > :04:26.epilepsy risk management and protocols. Action was put in place
:04:27. > :04:32.not just in the unit where Connor Sparrowhawk died but across all of
:04:33. > :04:34.our services. He said that xou let Connor Sparrowhawk down and his
:04:35. > :04:43.family down, so has anyone been disciplined? A number of individuals
:04:44. > :04:50.are subject to HR disciplin`ry proceedings and we will know the
:04:51. > :04:55.outcomes of that in some tile. How confident can people be abott the
:04:56. > :04:59.standard of care and homes that you run? It is important that wd have
:05:00. > :05:03.the confidence of the local population. Southern Health has been
:05:04. > :05:07.running these services for just over a year. There is much work to do,
:05:08. > :05:11.but significant progress has been made, and we believe that wd are
:05:12. > :05:16.really making those improvelents, and we will be, we want to have
:05:17. > :05:30.services that we are excepthonally proud of. A court has heard how a
:05:31. > :05:33.type of forklift truck was wrongly used on a building site where an
:05:34. > :05:36.Oxfordshire man died. 41`ye`r`old Mark Williams from Nuneham Courtenay
:05:37. > :05:40.died in the summer of 2011 when the truck he was using to lift tiles
:05:41. > :05:43.onto a roof at the Parkway development in Newbury fell on him.
:05:44. > :05:46.Two building companies are on trial for breaching health and safety
:05:47. > :05:52.laws. Ben Moore was at Readhng Crown Court. Expert operator Mich`el Dixon
:05:53. > :05:57.told the court that a mobild crane or similar machine should h`ve been
:05:58. > :06:00.used by Mr Williams instead as he worked on the Parkway Development.`
:06:01. > :06:03.The court has already heard how Mr Williams complained to site managers
:06:04. > :06:09.about the tight space he had to work in the day before he died.
:06:10. > :06:14.Mr Williams was 41 when he died father of two. The court had heard
:06:15. > :06:21.how Mr William 's complaint site managers about the tight sp`cing had
:06:22. > :06:24.to work in, the day before he died. Mr Dixon said the risks of tsing
:06:25. > :06:33.telehandlers should have bedn identified as the work was being
:06:34. > :06:36.planned. Mr William's mother was in court, as the defence said Lr
:06:37. > :06:40.Williams was using an unauthorised loading bay. The defence cross
:06:41. > :06:43.examined Michael Dixon about whether stabilising legs should havd been
:06:44. > :06:47.used with the telehandler. The defence say the loading could have
:06:48. > :06:49.been done safely but the re`son the telehandler tipped was that the
:06:50. > :06:58.loaded boom was raised and stabilisers not deployed. Asked if
:06:59. > :07:05.using it in this was dangerous, Mr Dixon replied, "Yes". The ddveloper
:07:06. > :07:13.Costain is expected to take the stand tomorrow. It along with
:07:14. > :07:18.Attlee's Roofing plead not guilty to breaching health and safety laws.
:07:19. > :07:22.Two men have been arrested following an armed robbery at a bookm`ker s in

:07:30. > :07:32.shop last Thursday. The pair arrested are both 18, and are being

:07:40. > :07:42.under threat because the ch`rity that funds it has had a big drop in
:07:43. > :07:45.donations. Action on Hearing Loss currently has a ?2.5 million
:07:46. > :07:49.shortfall. It's now launching an appeal for help, so it can continue
:07:50. > :07:54.its community work as well `s its specialist research projects.

:08:04. > :08:08.they are working properly. But the charity that runs this servhce is
:08:09. > :08:15.struggling. I would be very upset if it disappeared. Before, I h`d to go
:08:16. > :08:21.to Stoke Mandeville Hospital. It is much better here. There are lots of
:08:22. > :08:27.people in pain with the samd problem, hearing problems. Three has
:08:28. > :08:30.had a ?2.5 million shortfall this year because fewer people and let
:08:31. > :08:36.them money and their will. They have launched a big appeal. Mich`el is
:08:37. > :08:40.from the charity. His words are spoken by an interpreter. Anyone
:08:41. > :08:44.with hearing loss needs ongoing support. You do not just get a pair
:08:45. > :08:48.of glasses, you have to get on with your life. People with hearhng loss
:08:49. > :08:52.have hearing aids and need support the batteries and tubing, you need
:08:53. > :08:56.to have the hearing aid functioning as effectively as possible. At
:08:57. > :09:02.Oxford University the charity is funding ground`breaking resdarch
:09:03. > :09:07.into tin of this which can `ffect one in 100 people. There is
:09:08. > :09:11.currently no cure. Hearing loss is a neglected area of medical rdsearch.
:09:12. > :09:18.And then it is, which is part of hearing loss research, is even more
:09:19. > :09:25.fragile and underfunded. So, it is vital that we increase the funding
:09:26. > :09:29.that we have for important research. The charity has donated over half ?1
:09:30. > :09:33.million to Oxford Universitx for hearing loss research, but future
:09:34. > :09:42.studies like this could be `t risk, if more money cannot be found, soon.
:09:43. > :09:46.Plans to reduce speed limits to 60mph on part of the M4 are being
:09:47. > :09:49.criticised by the AA. The Hhghways Agency is proposing variabld speed
:09:50. > :09:51.limits and driving on the h`rd shoulder through Berkshire. The
:09:52. > :09:54.Agency says the so`called slart motorway will increase capacity
:09:55. > :09:57.There will be new safe placds for vehicles which break down, but the
:09:58. > :10:03.AA believes they'll be too far apart. With the smart motorway we
