:00:23. > :00:26.played a part in his death. Samuel Starr, who was born with a heart
:00:27. > :00:30.condition, died in September 20 2. His parents say delays caused by the
:00:31. > :00:34.new system meant he hadn't had a cardiac check`up for more than 0
:00:35. > :00:37.months. As our health correspondent, Matthew Hill, reports, Samuel was
:00:38. > :00:46.not the only child whose appointment was delayed.
:00:47. > :00:53.Samuel was born with a heart condition when he was three he had a
:00:54. > :01:04.cardiac operation. But his parents were concerned that it took longer
:01:05. > :01:10.than expected. We spoke to Samuel, reading him stories, and he passed
:01:11. > :01:14.away in our arms. Despite the many questions they had about his
:01:15. > :01:17.operation in Bristol, their biggest concern was whether his death might
:01:18. > :01:23.have been prevented if there were not huge delays to his regular
:01:24. > :01:31.checkups when he was alive. Samuel's local hospital was in Bath.
:01:32. > :01:36.Sam's condition meant he was supposed to have regular scans to
:01:37. > :01:41.make sure his condition didn't worsen. Following successful
:01:42. > :01:48.surgery, his heart was scanned but that turned out to be the last scan
:01:49. > :01:53.he had for 20 months. Inside out West has discovered a key factor in
:01:54. > :01:58.these delays was the introduction of a new IT system for hospital
:01:59. > :02:06.appointments. I am disgusted that it was recognised that no action was
:02:07. > :02:10.taken on it. In a review of his death, Bristol 's children Hospital
:02:11. > :02:15.concluded these delays may have played a part. We asked how many
:02:16. > :02:21.appointments were overdue or delayed because of the new computer system.
:02:22. > :02:25.They told us there were 63 overdue appointments with some delays of up
:02:26. > :02:32.to two years before they were discovered. It's very serious. You
:02:33. > :02:37.are dealing with people's lives and it's essential if you use computer
:02:38. > :02:44.systems that you have computer systems that work. Inside out West
:02:45. > :02:47.has also discovered there were 3 overdue appointments because of the
:02:48. > :02:59.introduction of the computer system, with some taking nearly two
:03:00. > :03:04.years to discover. We have offered our sincere condolences to the
:03:05. > :03:09.family of Samuel. The inquest will be held soon and we hope this will
:03:10. > :03:13.provide both Samuel's family and everybody involved in his care with
:03:14. > :03:18.a clear indication of the circumstances surrounding his death.
