:00:00. > :00:00.It's goodbye from me. Welcome to BBC Points West with

:00:00. > :00:07.Alex Lovell and David Garmston. The children who died

:00:08. > :00:10.on Heart Ward 32. Were the patients let down

:00:11. > :00:15.by internal failures? An inquiry is launched into the

:00:16. > :00:17.ward's failings, but some f`milies I'll be examining their concerns

:00:18. > :00:22.and asking is this inquiry fit A series of fires around Bristol `

:00:44. > :00:49.the Queen's Baton arrives, on Grieving families of some

:00:50. > :01:03.of the children who've died on Ward 32 in Bristol are claiming that

:01:04. > :01:06.the inquiry into their deaths The youngsters were being treated

:01:07. > :01:10.on the controversial ward at Bristol Children's Hospital, which

:01:11. > :01:12.has received some serious criticism Today details have emerged

:01:13. > :01:18.about the scope of the inquhry. Here's our health correspondent

:01:19. > :01:21.Matthew Hill. Jason Banham still has unanswered

:01:22. > :01:23.questions about the death Jasmine was a very sick babx and

:01:24. > :01:47.died shortly after a heart operation Y`word she sent up to Bristol

:01:48. > :01:53.hospital, `` Y`word she sent up to Bristol hospital as an emergency and

:01:54. > :01:56.then not operated on the two weeks. The operation was cancelled again

:01:57. > :01:58.and again. She had severe deficiencies which they did not

:01:59. > :02:00.realise until they operated. But Jasmine's death in 2009 won t be

:02:01. > :02:04.looked at by the enquiry set up That's because it will only include

:02:05. > :02:09.families whose children died One of the families who do fall

:02:10. > :02:13.into that timescale is the Turners from Warminster, who lost

:02:14. > :02:16.their son Sean in 2012 . A coroner ruled there were lost

:02:17. > :02:24.opportunities in his care. We have at least seven families who

:02:25. > :02:30.are from before 2010 who ard feeling are from before 2010 who ard feeling

:02:31. > :02:35.excluded from the review and they are seeking answers for what they

:02:36. > :02:39.have lost their child. One of our strong concerns as they are only

:02:40. > :02:43.going to look at the clinical notes, as we know from Sean's case from the

:02:44. > :02:45.inquest, a lot of the notes are missing and it just won't work.

:02:46. > :02:50.There are test results and notes that we note do not take place,

:02:51. > :02:56.there are too many retrospective notes and Sean's 24`hour critical

:02:57. > :02:59.nursing notes are missing from before his cardiac arrest. Ht

:03:00. > :02:59.nursing notes are missing from before his cardiac arrest. It is

:03:00. > :03:02.turning into a farce, reallx. But it's not just about the care

:03:03. > :03:15.of individual patients that this They will be looking at staffing

:03:16. > :03:18.levels, care, the working environment. Then there is

:03:19. > :03:21.communication, how open the trust has been with dealing with

:03:22. > :03:26.families. And compassion, particularly when children have

:03:27. > :03:29.died. Then finally, the culture of trust and hope the hospital reported

:03:30. > :03:32.information to outside bodids and information to outside bodies and

:03:33. > :03:39.have any concerns, particul`rly about staffing levels, were

:03:40. > :03:42.responded to. The hospital says Jasmine's parents have not contacted

:03:43. > :03:46.them about concerns before but have offered to meet them. Nothing short

:03:47. > :03:49.of a full public enquiry will satisfy the Turners, though, who are

:03:50. > :04:05.leading this campaign for answers. Professor Kennedy, who is leading

:04:06. > :04:10.this enquiry has not been given interviews today. He has bedn

:04:11. > :04:10.this enquiry has not been ghven interviews today. He has been told

:04:11. > :04:16.interviews today. He has bedn told to hit the ground running. He needs

:04:17. > :04:20.to see the evidence thoroughly but I think at the back of his mind is if

:04:21. > :04:21.you go back beyond the four years, people's memories can fade and

:04:22. > :04:21.you go back beyond the four years, people's memories can fade `nd it

:04:22. > :04:27.people's memories can fade and it might just blur the focus of his

:04:28. > :04:29.enquiry so I think that is why they are keen to focus on more rdcent

:04:30. > :04:32.are keen to focus on more recent events. But nevertheless, that is

:04:33. > :04:38.something that has been very frustrating to many families. And

:04:39. > :04:43.the timescale? Within a year, I would hope. We know it is not a

:04:44. > :04:46.public enquiry, but the outcome will be? Of course, the outcome will be

:04:47. > :04:49.fully public. 70 football fans in the Avon

:04:50. > :04:52.and Somerset area are among those having to hand their passports

:04:53. > :04:55.in ahead of this month's World Cup. All of those required to do

:04:56. > :04:59.so have been caught up in violence or hooliganism at matches,

:05:00. > :05:01.including the Bristol Rovers The fans have until tomorrow to

:05:02. > :05:05.give police their passports. If they fail to do so,

:05:06. > :05:08.they face being arrested again. An MP has called

:05:09. > :05:12.on the police to be more opdn about their investigations into a string

:05:13. > :05:15.of arson attacks in the west. In the last four years,

:05:16. > :05:16.50 buildings, cars and radio masts

:05:17. > :05:19.have been targeted. A group of anarchists have claimed

:05:20. > :05:22.responsibility for the fires, including the blaze

:05:23. > :05:25.which caused millions of pounds' of damage to the new police

:05:26. > :05:29.firearms centre at Portishead. Here's our Home Affairs

:05:30. > :05:35.Correspondent, Steve Brodie. It was last August

:05:36. > :05:37.when the police firearms It took four days before

:05:38. > :05:45.the blaze was finally put out. The multi`million`pound centre,

:05:46. > :05:47.which was to provide training for forces across the west,

:05:48. > :05:52.had never opened and it's still closed nearlx

:05:53. > :05:55.one year on. Responsibility for the attack which

:05:56. > :05:58.caused ?16 million of damage was claimed by a Bristol`based `narchist

:05:59. > :06:01.group who gave precise details of They also claimed they set fire to

:06:02. > :06:06.a G4S courier van This was the biggest in a string

:06:07. > :06:12.of arson attacks including radio But so far,

:06:13. > :06:17.no convictions have been made. Today, the MP for Bristol North West

:06:18. > :06:20.called on the police to tell the public how their investigations

:06:21. > :06:33.are getting on. I Lehmann onlooker would look at the

:06:34. > :06:38.situation and say, yes, I think they have been slow `` and Lehman. Having

:06:41. > :06:40.said that, it is easy to crhticise from outside and hindsight is a good

:06:41. > :06:44.from outside and hindsight hs a good thing but I do think more effort

:06:45. > :06:47.needs to be taken now to link these and start to get to the bottom of

:06:48. > :06:52.what looks very much like an idea logically based series of criminal

:06:53. > :06:59.attacks. And the reason I think it is so important is they are not

:07:00. > :07:00.acting alone, what other situation surrounding it and is it going to be

:07:01. > :07:02.surrounding it and is it gohng to be contagious?

:07:03. > :07:05.Today, police appealed to the public for help

:07:06. > :07:09.They deny that they have bedn slow with their investigations

:07:10. > :07:15.and say they have looked at every possible angle and, indeed,

:07:16. > :07:24.We have a whole range of experts examining what has been claimed on

:07:25. > :07:27.each individual offence and looking at the possible connection between

:07:28. > :07:31.them all, so, yes, we are looking at the links and doing so very

:07:32. > :07:35.seriously. If you are asking me can I link them definitively, h`ve I got

:07:36. > :07:37.evidence they are linked? Then, no, I haven't.

:07:38. > :07:39.Anarchists also claimed thex were behind another arson attack `

:07:40. > :07:41.this time on the North Avon Magistrates Court
