:00:56. > :00:57.in the interim when we wait for a government in denial to grant one

:00:58. > :01:02.they must be the removal of the Chief Executive.

:01:03. > :01:06.At the Princess of Wales Hospital in Bridgend, failings have been found.

:01:07. > :01:09.I'll be asking the Chairman of the Health Board what went wrong

:01:10. > :01:28.A neo natal centre for north Wales will be sited at Ysbyty Glan Clwyd

:01:29. > :01:31.but the sickest babies will be treated in England.

:01:32. > :01:35.The green energy schemes that can help communities make money.

:01:36. > :01:38.But some are concerned there's too much red tape.

:01:39. > :01:42.And Wales names the boxing squad for the Commonwealth Games,

:01:43. > :01:52.including the first ever female Welsh boxers to compete.

:01:53. > :02:03.Patients left without food, water, or the correct medication and told

:02:04. > :02:08.Welcome to the Princess of Wales Hospital in Bridgend,

:02:09. > :02:11.which tonight finds itself at the centre of a storm about how

:02:12. > :02:17.An independent review found serious failings here,

:02:18. > :02:23.Tonight, the First Minister and The Health Minister have apologised

:02:24. > :02:27.to families of patients and have ordered a Wales wide review of

:02:28. > :02:39.Here's our health correspondent Owain Clarke.

:02:40. > :02:48.The government cannot hide behind this report. All of these families

:02:49. > :02:50.claim the Swansea health board had left them and their loved ones down.

:02:51. > :02:58.They each other story about that care. They saw today one of the most

:02:59. > :03:04.damning reports on care standards in Wales in recent years. They have not

:03:05. > :03:08.taken any consideration of surgery. With them was Gareth Williams from

:03:09. > :03:13.Porthcawl. An investigation last summer revealed his mother suffered

:03:14. > :03:18.serious neglect during the last two years of her life. This video by the

:03:19. > :03:23.family shows medicine scattered around the hospital bed. She had

:03:24. > :03:27.been sedated against their wishes. The health board ultimately

:03:28. > :03:32.apologised but ever since Gareth Williams has led a campaign to

:03:33. > :03:34.expose other examples of poor care which party prompted the

:03:35. > :03:40.investigation that was published today. He told us apologies aren't

:03:41. > :03:44.enough. It is a public disgrace. There must be a public enquiry and

:03:45. > :03:48.in the interim while we wait for a government in denial to grant one

:03:49. > :03:53.they must be the removal of the Chief Executive and the management

:03:54. > :03:57.team. The team of experts brought in by the Welsh Government spent months

:03:58. > :04:04.talking to staff, patients and their families. Some of what they had

:04:05. > :04:08.caused them real concern. Some patients were not getting the water

:04:09. > :04:13.they needed, the food they needed, the medication they needed. These

:04:14. > :04:19.were largely exceptional events but they should not happen. There were

:04:20. > :04:24.several other feelings for example older patients in bed instructed to

:04:25. > :04:30.go to tile at Waverley. They were dismayed that staff tolerated

:04:31. > :04:33.hazardous practices which meant medicines were recorded as given

:04:34. > :04:38.when sometimes they were not. It notes a failure of adequate staffing

:04:39. > :04:44.20 47 and managers being distracted trying to cut costs at the expense

:04:45. > :04:48.of quality and patient safety. Concerns were also raised about the

:04:49. > :04:55.effectiveness of health care Inspectorate Wales in not picking up

:04:56. > :04:58.some of the issues. All in all there are 18 specific recommendations for

:04:59. > :05:03.the local health board and Welsh Government. Both say they accept

:05:04. > :05:06.them all and this afternoon the Health Minister went further

:05:07. > :05:12.announcing spot checks would be held within months at all Welsh

:05:13. > :05:17.hospitals. As from today, I will give health boards and NHS trusts in

:05:18. > :05:20.Wales a four-week period to absorb the findings of the report and

:05:21. > :05:25.satisfy themselves that such departures from basic professional

:05:26. > :05:31.standards are not present in their organisations. Immediately after

:05:32. > :05:37.that, each district general hospital in Wales will be visited by a small

:05:38. > :05:41.group of independent experts. But others argue this report findings

:05:42. > :05:48.contains ominous echoes of the mid Staffordshire neglects candle in

:05:49. > :05:51.England. He may not think that this is a mid-Staffs crisis and I don't

:05:52. > :05:56.think the report authors can conclude that given the narrow focus

:05:57. > :06:03.of their work. But I have this one final thing to say. If it looks

:06:04. > :06:07.like, smells like and moves like a mid-Staffs crisis, it's time for a

:06:08. > :06:12.major enquiry into care standards in the Welsh NHS. The elderly care

:06:13. > :06:17.expert who led the review begs to differ. There is a huge difference

:06:18. > :06:21.between the catastrophic organisational failure that happened

:06:22. > :06:26.in mid-Staffs and the situation here. In mid-Staffs the Danish

:06:27. > :06:35.health service had no idea that it had a problem at all. Here, by the

:06:36. > :06:42.time the Minister was aware, you had asked us to review it and the health

:06:43. > :06:47.board knew there were issues. The report does give some cause to be

:06:48. > :06:55.hopeful claiming that with immediate effort things can be turned around.

:06:56. > :06:57.That with ten nurses suspended from duty and police investigation is

:06:58. > :07:00.ongoing these families are still concerned. They consider themselves

:07:01. > :07:05.to be victims of the care system that didn't care enough.

:07:06. > :07:07.Let's talk to the chair of the Health Board,

:07:08. > :07:11.One patient told the review team "I am in hell".

:07:12. > :07:26.I was ashamed and I told the Minister that. We accept all the

:07:27. > :07:31.recommendations of the report and we apologise for any poor care that we

:07:32. > :07:39.have provided in recent years. Nurses claiming that patients would

:07:40. > :07:43.not their responsibility. Management not doing much about complaints from

:07:44. > :07:50.families. It does not get much worse does it? Absolutely unacceptable. We

:07:51. > :07:53.have been talking today with members of staff here and we will do that

:07:54. > :07:59.over the coming weeks. We have said clearly that that type of care is

:08:00. > :08:08.unacceptable. If we become aware of it people should be made -- making

:08:09. > :08:11.us aware of it. When I arrived here tonight I was met by a dozen

:08:12. > :08:16.families who told me they were making complaints going back years.

:08:17. > :08:21.Nothing was done and nobody was listened to. They are calling for

:08:22. > :08:27.resignations tonight. Why has nobody resigned? The report said clearly

:08:28. > :08:31.they have confidence in the leadership of the board to put right

:08:32. > :08:39.what has been happening over many years. This has happened on

:08:40. > :08:46.Baywatch. We are not a mid Staffordshire. The difference

:08:47. > :08:53.between... We started to take action. You did not recognise the

:08:54. > :09:04.deficiencies early. The families were telling the hospital this years

:09:05. > :09:11.ago and nobody listened. I took up this office in January 2013. Serious

:09:12. > :09:16.concerns came to light in March and we have moved very quickly. We have

:09:17. > :09:23.changed the decision-making and we have challenged the bad practice. We

:09:24. > :09:26.have changed management locally here. We are making wider changes

:09:27. > :09:38.across the health board. So heads will roll? People have not performed

:09:39. > :09:42.and they are no longer in position and we have appointed new people. We

:09:43. > :09:46.have strengthened the local management and we are strengthening

:09:47. > :09:51.the management in the health board. We accept the changes have not been

:09:52. > :09:54.fast enough but that is why we welcomed the report. We think we can

:09:55. > :10:21.produce something very special here and across the health board.

:10:22. > :10:24.Many will hope this trial is a line under a row that goes back two years

:10:25. > :10:31.or more to a decision by the Betsy Cadwallader health board to move the

:10:32. > :10:35.treatment of seriously ill babies to a hospital over the border. John

:10:36. > :10:40.Hewitt, you campaigned against that initial decision and you campaigned

:10:41. > :10:45.for the regional centre to be tastier. Wrexham were hoping they

:10:46. > :10:49.would get it. That is disappointment as well from some people who are

:10:50. > :10:55.concerned that the most seriously ill babies will still be treated

:10:56. > :11:00.over the border. I say to the people in Wrexham, two years ago there was

:11:01. > :11:04.not going to be any investment in a new mental -- neonatal service in

:11:05. > :11:15.North Wales. They were going to ship everybody over to London. To your

:11:16. > :11:19.second point, I would say there has always been a case for seriously ill

:11:20. > :11:27.babies. And you have benefited from that in your family? We have. Our

:11:28. > :11:31.baby was born here in this unit and she would not be alive today if it

:11:32. > :11:35.wasn't for the fantastic perfectionism of the staff here. The

:11:36. > :11:40.first minister says this decision comes with risks and challenges.

:11:41. > :11:49.There are concerns that the health board will not be able to deliver on

:11:50. > :11:52.this side of it. They will have to. That is quite clear. There is a

:11:53. > :11:58.change of the higher management now in Betsy Cadwallader and as a result

:11:59. > :12:03.there needs to be a higher change so they're going to have two get their

:12:04. > :12:10.ideas are straight. So a big announcement today. Acta you, Jamie.

:12:14. > :12:16.The First Minister and The Health Minister have both apologised to the

:12:17. > :12:21.families of hospital patients after an investigation found "unacceptable

:12:22. > :12:27.More on this from our health correspondent Owain Clarke later.

:12:28. > :12:38.A judge at Mold Crown Court has jailed a scout master

:12:39. > :12:42.for 12 years for a series of sex offences against young teenagers

:12:43. > :12:47.Martyn Tucker, who's 68 and from Chester, had admitted the offences.

:12:48. > :12:50.The court heard the scout movement knew of the allegations,

:12:51. > :12:55.Some of the sexual assaults took place on outdoor activity trips

:12:56. > :13:02.A mines inspector has told the jury at the Gleision trial that

:13:03. > :13:04.he wasn't notified of any plans to make contact with

:13:05. > :13:08.a disused part of the mine, contrary to industry regulations.

:13:09. > :13:11.Four men drowned at the Swansea Valley pit nearly three years ago.

:13:12. > :13:15.Under cross examination, Tony Forster said he held

:13:16. > :13:18.the mine manager, Malcolm Fyfield, in high regard.

:13:19. > :13:21.Mr Fyfield denies four counts of manslaughter through gross

:13:22. > :13:24.negligence and the mine operators deny four

:13:25. > :13:31.A charity which gives grants to low income families with disabled or

:13:32. > :13:35.seriously ill children has told BBC Wales it's had to stop

:13:36. > :13:38.allocating money in Wales because of a delay in its funding.

:13:39. > :13:42.The Family Fund normally receives around ?2.6 million a year

:13:43. > :13:49.But it hasn't been told yet whether it will get any money this year.

:13:50. > :13:51.The Government has admitted the delay is 'not desirable' but says

:13:52. > :14:05.We have over 550 families waiting for the fund support. They are

:14:06. > :14:12.waiting for washing machines, fridge freezers, clothing, bedding. The

:14:13. > :14:13.reason we are not able to help them is because of the delay in the

:14:14. > :14:20.funding from the Welsh Government. The Welsh Liberal Democrats have

:14:21. > :14:23.launched their manifesto for the European elections

:14:24. > :14:27.by saying that while they're big supporters of the European Union,

:14:28. > :14:29.they're not afraid of reform. The launch took place

:14:30. > :14:32.in the technology firm I.Q.E in Cardiff

:14:33. > :14:34.which has just announced a new deal Our Political Editor Nick Servini

:14:35. > :15:04.was there. A highly technical briefing was

:15:05. > :15:07.given to the team. The workings of the European Parliament can be just

:15:08. > :15:12.as baffling. It's one of the challenges the Lib Dems face trying

:15:13. > :15:15.to sell the EU to the public. Coming to companies like this is a way of

:15:16. > :15:22.highlighting the opportunities in trade. The party as set out its

:15:23. > :15:27.stall as the party of in which it insists is markedly different from

:15:28. > :15:33.the others. If you want to ensure that Wales and the UK continues to

:15:34. > :15:39.punch above its weight and to trade across the European Union there is

:15:40. > :15:42.only one party you can vote for. This company works in the

:15:43. > :15:47.communications industry and makes the kind of equipment you find in

:15:48. > :15:49.virtually every mobile phone. The challenge for the Liberal Democrats

:15:50. > :15:53.is about communication as well and getting their message across that

:15:54. > :15:58.they are different to the other pro-European parties because they

:15:59. > :16:05.claim to put EU membership at the heart of everything they do.

:16:15. > :16:17.Some community groups say they're having problems getting energy

:16:18. > :16:20.projects built because there's too much red tape.

:16:21. > :16:24.In a bid to find new sources of income, many are turning to

:16:25. > :16:27.small hydro-electric, wind and solar schemes to generate money by

:16:28. > :16:38.Our environment correspondent Iolo ap Dafydd reports from Snowdonia.

:16:39. > :16:44.This is just inside Snowdonia 's national park. Villagers have formed

:16:45. > :16:48.a regeneration company which turned this mill into a community centre

:16:49. > :16:54.and now has planning approval for a 270 kilowatts hydroelectric scheme.

:16:55. > :17:03.This project will generate powerful in the National Grid -- power for

:17:04. > :17:08.the National Grid. But the process has been slow. I think it is too

:17:09. > :17:21.bureaucratic. The landowners, the National Trust and the Forestry

:17:22. > :17:23.Commission, they are dealing with us as the first community they have

:17:24. > :17:27.negotiated with and they are worried about setting precedents so they are

:17:28. > :17:32.being cautious. In an area of high rainfall hydra scheme benefits from

:17:33. > :17:37.an extra tariff paid by the UK Government to generate electricity

:17:38. > :17:43.from renewable energy. Generating clean energy is still attractive to

:17:44. > :17:47.landowners. This proposal to abstract route -- water from this

:17:48. > :17:52.river was refused by Natural Resources Wales and a private scheme

:17:53. > :17:59.was approved on the grounds it had slightly less environmental impact.

:18:00. > :18:02.It did cost is ?28,000 to get to the point of getting the licence for the

:18:03. > :18:15.water extraction. It was at that point they said no it has been

:18:16. > :18:18.allocated to another company. The National Trust is adapting its

:18:19. > :18:22.properties in Wales with over 100 renewable energy schemes including

:18:23. > :18:28.two new hydroelectric schemes at this farm. The six and 40 kilowatts

:18:29. > :18:35.hydro cost ?1.6 million but will pave itself in seven years. You've

:18:36. > :18:40.got 100 years of sustainable income. The process especially if you'll

:18:41. > :18:46.only ever build one hydro is not to be taken lightly. It is years of

:18:47. > :18:50.hard slog. If it was easy everybody would be doing it. The trust is

:18:51. > :18:55.pushing ahead with larger renewable energy plans to cut its power bills

:18:56. > :19:01.but smaller community problems are slowly becoming less affordable as

:19:02. > :19:02.cats in the West generate -- generous tariffs to produce green

:19:03. > :19:07.energy are being accelerated. Wales have named an initial

:19:08. > :19:10.10-strong boxing squad for the 2014 It includes three women set to make

:19:11. > :19:14.history as the first ever female Welsh

:19:15. > :19:18.boxers to compete in the Games. They're being described as Wales'

:19:19. > :19:23.strongest-ever boxing squad. These are just some of the ten

:19:24. > :19:27.boxers gearing up for ten weeks of intense training before the

:19:28. > :19:30.Commonwealth Games gets underway. Taking their experience from London

:19:31. > :19:33.2012 to Glasgow are Olympic silver medallist Fred Evans and

:19:34. > :19:37.European Champion Andrew Selby, alongside reigning Commonwealth

:19:38. > :19:52.gold medallist Sean McGoldrick. It is a real tough tournament. The

:19:53. > :19:54.world number two and three and six are in my category so it can be

:19:55. > :19:58.tough. Ashley Brace is one of three women

:19:59. > :20:01.set to make history as the first ever Welsh female boxers

:20:02. > :20:05.to compete at a Commonwealth Games. Since Nicola Adams won the first

:20:06. > :20:08.women's boxing gold at the Olympics Ashley is now out to follow

:20:09. > :20:20.in Nicola's footsteps She is a good boxer and somebody I

:20:21. > :20:25.would like to fight and obviously beat. I don't go in there to lose.

:20:26. > :20:26.To me it would be the greatest achievement to beat the Olympic

:20:27. > :20:32.champion. Fred Evans first visited a boxing

:20:33. > :20:35.club at the age of four and hasn't The pinnacle

:20:36. > :20:39.of his career was London 2012. Alongside Andrew Selby they

:20:40. > :20:52.represented Team GB at the Olympics. An Olympic silver medal is a great

:20:53. > :20:57.achievement and everybody is saying it will be good now. But it is a

:20:58. > :21:01.tough competition but I do have my sights on the gold.

:21:02. > :21:13.So 10 boxers all with one goal and for one country.

:22:38. > :22:45.There's to be an All-Wales hospital review into elderly patient care.

:22:46. > :22:48.It follows a damning report into Abertawe Bro Morgannwg Health Board,

:22:49. > :22:55.The independent review found failings both at Neath Port Talbot

:22:56. > :22:58.Hospital and the Princess of Wales Hospital in Bridgend,

:22:59. > :23:10.An immediate review has been launched by the Health Minister.

:23:11. > :23:14.This report raises a number of "serious concerns" about

:23:15. > :23:19.the quality of care in the NHS and how elderly people are cared for.

:23:20. > :23:23.It all began following a family's complaint about the

:23:24. > :23:32.Let's talk to our Health Correspondent, Owain Clarke.

:23:33. > :23:40.Explain to us more about the Welsh Government's response.

:23:41. > :23:49.I think this report is so significant and the concerns in it

:23:50. > :23:53.so significant that the government had no option but to come up with a

:23:54. > :23:56.response that at least can be interpreted as robust. I am told

:23:57. > :24:02.this morning every health board boss in Wales was ordered to look at this

:24:03. > :24:08.report and measure their services for elderly patients against it.

:24:09. > :24:12.After a month, inspectors come experts will go into every hospital

:24:13. > :24:18.in Wales and judge whether or not elderly care is up to scratch. The

:24:19. > :24:27.person in charge of that process is the author of this report. RU

:24:28. > :24:33.surprised that this enquiry did not call for heads to roll? Yes.

:24:34. > :24:37.Apologies to campaigners are hollow without resignations but I was

:24:38. > :24:44.talking about this issue with the author of this report today and she

:24:45. > :24:50.came to the view that if heads had rolled, that would have left a power

:24:51. > :24:55.void at the top of the health board at the very time improvements needed

:24:56. > :25:01.to be made. The view was that would not have been good for patients.

:25:02. > :25:04.Visiting time is about to begin as hundreds of families come here

:25:05. > :25:09.The managers, politicians and NHS staff responsible for providing

:25:10. > :25:13.the best of patient care have been found wanting tonight.

:25:14. > :25:20.Tomorrow, the job of rebuilding trust and reputation begins.
