:01:08. > :01:09.He was locked up indefinitely under the Mental Health Act.
:01:10. > :01:13.He injured 20 people and killed this woman, leaving her
:01:14. > :01:23.Her sister believes if he had been monitored more closely the
:01:24. > :01:39.It makes me feel that it was unnecessary.
:01:40. > :01:42.I truly believe, if he was monitored and had been
:01:43. > :01:46.taking his medication, my sister would still be alive today.
:01:47. > :01:51.It is two years since she was killed.
:01:52. > :01:56.This report was ordered as Matthew Triddon had been a user of
:01:57. > :02:03.It concludes the homicide could not have been predicted
:02:04. > :02:07.and it was difficult to see how it could have been prevented.
:02:08. > :02:13.However, it highlighted shortcomings.
:02:14. > :02:14.The report said there was poor communication between
:02:15. > :02:28.He had not taken his medication for almost a year
:02:29. > :02:30.before the incident, but there was no system to monitor whether
:02:31. > :02:34.The workload for the consultant psychiatrist was excessive, with
:02:35. > :02:38.There was no single care coordinator to guide him through the system,
:02:39. > :02:43.but instead was seen by nine separate GPs.
:02:44. > :02:45.The view was taken that so good was the progress that this
:02:46. > :02:48.gentleman was making was that he was a pathway to no medication
:02:49. > :02:58.It is important to say that our goal is to enable people to
:02:59. > :03:07.One of the saddest things from this case is that Matthew
:03:08. > :03:16.Triddon felt there was a stigma attached to mental health services.
:03:17. > :03:20.The judge accepted that Triddon was having hallucinations as part
:03:21. > :03:32.The report today highlights earlier occasions where concerns were raised
:03:33. > :03:40.Gutted because in my eyes this was preventable.
:03:41. > :03:49.Now my nieces have to live their lives without a mother.
:03:50. > :03:50.Police from South Wales have confirmed
:03:51. > :03:52.they're assisting colleagues in London in their search for
:03:53. > :04:05.A reconstruction of her last known movements has been staged today.

:05:57. > :05:59.According to the schools inspectorate, vulnerable pupils
:06:00. > :06:05.are most affected if schools don't do enough to tackle absenteeism.
:06:06. > :06:10.Those eligible for free school meals are nearly twice as likely to miss
