:00:17. > :00:41.Dylan Seabridge was eight years old when he died of scurvy in 2011

:00:42. > :00:44.and yet, his inquest heard, he saw no doctors, dentists

:00:45. > :00:49.or teachers from when he was just 13 months old until his death.

:00:50. > :00:52.Now a BBC Wales investigation has found that concerns were raised

:00:53. > :00:56.to the authorities more than a year before he died.

:00:57. > :00:59.More than four years on and nothing has been published about

:01:00. > :01:02.whether more could have been done to prevent it.

:01:03. > :01:11.India Pollock has this special report.

:01:12. > :01:18.Dylan was invisible to local councils, according to a draft

:01:19. > :01:22.serious case review. The author of the report knew so little about

:01:23. > :01:27.Dylan that it was impossible to draw a picture of him, she said. The

:01:28. > :01:31.inquest into his death heard that Dylan did not see a doctor or

:01:32. > :01:37.dentist from the age of 13 months until he died of scurvy at the age

:01:38. > :01:43.of eight. Scurvy is caused by a lack of vitamins C. Dylan's inquest heard

:01:44. > :01:52.that it isn't easily preventable and treatable disease. Some of the

:01:53. > :01:57.critical features -- clinical features would be very typical of

:01:58. > :02:00.scurvy, the abnormal garments, the rash on the lead, the swelling of

:02:01. > :02:08.the lower limbs, the discolouration or bruising on the lower legs.

:02:09. > :02:12.Dylan's parents don't believe he died of scurvy. Or that he was

:02:13. > :02:18.invisible to the outside world. They were charged with neglect but those

:02:19. > :02:25.charges were dropped in 2014. Dylan lived here in rural Pembrokeshire

:02:26. > :02:31.but his mother worked as a teacher in neighbouring carrot again. It was

:02:32. > :02:36.during an employment tribunal the true professionals became aware she

:02:37. > :02:40.was suffering mental ill health. They contacted social services and

:02:41. > :02:44.told them that he was home educated. He died a year and a half later.

:02:45. > :02:50.After his death, a serious case review was commissioned to look at

:02:51. > :02:54.the involvement of any agencies. It should've been completed quickly so

:02:55. > :02:58.that any lessons can be learnt and shared. The draft review we have

:02:59. > :03:05.seen has never been published and poor years after Dylan died, nothing

:03:06. > :03:09.else has been published either. Mark Dyson is a former solicitor and was

:03:10. > :03:14.one of the people who contacted social services. He also told senior

:03:15. > :03:18.officials that he was anxious about the investigation they were carrying

:03:19. > :03:21.out after Dylan's death. He said he was not interviewed properly about

:03:22. > :03:25.his involvement and a key documents were not shared with the author of

:03:26. > :03:30.the review. He also says the delay in publication is worrying. The

:03:31. > :03:36.whole purpose of serious case reviews is to learn lessons. They

:03:37. > :03:45.need to be approached with urgency because if lessons are not learnt,

:03:46. > :03:50.these things will happen again. It has been inadequate. There doesn't

:03:51. > :03:55.seem to have been any fully co-ordinated response or sense of

:03:56. > :04:07.urgency, no focus on the child and the fact a child is dead. The

:04:08. > :04:11.council did and why Mr Dyson's claims. Pembrokeshire Council says

:04:12. > :04:14.the delay has been caused by external factors. They had to wait

:04:15. > :04:18.into the criminal investigation was completed and for the inquest which

:04:19. > :04:22.took place last year in January. They also said the process was very

:04:23. > :04:26.complex because of the change in government guidance and changing the

:04:27. > :04:33.structure of Pembrokeshire safeguarding board. They say the new

:04:34. > :04:38.review will be published soon Dylan was home educated. 1500 children in

:04:39. > :04:42.Wales are. There is no legal obligation to registered your

:04:43. > :04:47.teaching children at home. The children's commissioner of says that

:04:48. > :04:52.needs to change. I don't think Dylan is the only child who is under the

:04:53. > :04:57.radar in Britain so we should be learning what we can very clearly

:04:58. > :05:01.from this case and as quickly as possible. The Welsh government did

:05:02. > :05:04.not want to be interviewed but we asked them a number of questions.

:05:05. > :05:09.They told us they couldn't comment on an individual case but said the

:05:10. > :05:14.new non-statutory guidance on home education would be published soon.

:05:15. > :05:19.As for Mr Dyson, he is now retired. He gave up his career after 20 years

:05:20. > :05:22.in public services and says the fight for a full and robust

:05:23. > :05:28.investigation has had a profound impact on him. You start to feel as

:05:29. > :05:35.though either the thread of reason has broken or that there is

:05:36. > :05:48.something so profoundly wrong that you can't put it right. It exhausts

:05:49. > :05:52.you. It goes on and on and on because he was just trying to find

:05:53. > :05:56.somebody who can listen fairly to what you are saying, not necessarily

:05:57. > :06:00.agree with you, but listen fairly to what you are saying, and all the

:06:01. > :06:06.time saying is a child is dead. Where are we in terms of learning

:06:07. > :06:08.lessons from that? Who is asking the questions and shining a light on the

:06:09. > :06:11.situation? The answer is nobody. Former solicitor Mark Dyson ending

:06:12. > :06:14.that report by India Pollock. Two men from Wiltshire have appeared

:06:15. > :06:17.in court to face charges of death by dangerous driving

:06:18. > :06:19.after the tipper truck they were in killed four people,

:06:20. > :06:23.including three men from South Robert Parker from Cwmbran,

:07:21. > :07:22.in Wales on landfill has fallen by nearly a quarter in the last four

:07:23. > :07:26.years according to figures obtained The annual cost has dropped

:07:27. > :07:33.from more than ?62 million to more than ?47 million but that's

:07:34. > :07:40.still a bill of ?130,000 every day. And the ongoing cost of landfill has

:07:41. > :07:43.driven one local authority, Rhondda Cynon Taff, to become

:07:44. > :08:03.the latest to commit to stop All the stuff we didn't want, didn't

:08:04. > :08:07.recycle and Thruway, crushed up. 3500 tonnes of that processed every

:08:08. > :08:14.week, enough room for another 20 years worth. It has become so

:08:15. > :08:19.expensive to get rid of what we don't want in big holes in the

:08:20. > :08:25.ground like this between the UK Government's landfill tax and the

:08:26. > :08:29.Welsh government's finds. The council have become the latest

:08:30. > :08:33.authority in Wales to say enough is enough, no more landfill for

:08:34. > :08:41.household waste, no more of what we don't want in places like this. From

:08:42. > :08:44.April, black bag waste will be burnt to generate electricity instead.

:08:58. > :09:01.bury rubbish so it makes sense. Most local authorities have cut their

:09:02. > :09:06.landfill costs over the past four gears. Two of wine has seen the

:09:12. > :09:17.costs remain flat. Anglesey pretty much zero. Conwy has seen the

:09:18. > :09:21.biggest increase, 26% more than 4-mac rig years ago. It says it is

:09:22. > :09:30.down to the increase in landfill tax not that it is put in more waste in

:09:31. > :09:36.the ground. Recyclable. Tall pine say in proving recycling rates is up

:09:37. > :09:42.to reduce costs. These are your crisp bags or bread wrappers. They

:09:43. > :09:47.cannot be recycled. Material like this goes to heat treatment but that

:09:48. > :09:54.does not come cheap. Budgets are still as tight as ever. We don't

:09:55. > :09:59.make money on recycling. The money we earn on recycling goes into

:10:00. > :10:05.running the service. ?130,000 a day is still spent in Wales in landfill

:10:06. > :10:07.charges. Here in Denbighshire, recycling is higher than anywhere

:10:08. > :10:13.else in the country but they are keen to improve further. With our

:10:14. > :10:18.current disposal costs, it could save the council half ?1 million a

:10:19. > :10:23.year if that last little bit of recycling could be put on the right

:10:24. > :10:28.containers. But of course changes to our recycling does not always go

:10:29. > :10:36.down well. Protests in Cardiff over new Wii burns that some that were

:10:37. > :10:40.unnecessary. Anger over new recycling boxes in Gwent which some

:10:41. > :10:44.find difficult to use an impractical. The Welsh government

:10:45. > :10:49.wants a zero waste Wales in 2050. The methods used to get their is

:10:50. > :10:52.still leaving some deeply sceptical. Plans for a ?19.5 million renovation

:10:53. > :10:54.of one of Aberystwyth's most iconic buildings have been discussed
