0:01:31 > 0:01:33There is a theme there in terms of out-of-hours care.

0:01:33 > 0:01:36In a new report, he highlights a dozen examples stretching back

0:01:37 > 0:01:40He says he found it entirely unacceptable that one patient,

0:01:40 > 0:01:42Mr T, an elderly man with motor neurone disease,

0:01:42 > 0:01:46had to wait more than 40 hours over a weekend before doctors gave him

0:01:46 > 0:01:47a feeding tube.

0:01:47 > 0:01:49He later died.

0:01:49 > 0:01:52On another occasion, a patient, Mrs K, was discharged from hospital

0:01:52 > 0:01:54twice by junior doctors, even though the ombudsman found

0:01:54 > 0:01:57on both occasions it had not been medically safe to do so.

0:01:57 > 0:02:02She later died of a blood clot.

0:02:02 > 0:02:05He also says junior doctors can sometimes be afraid to ask for help

0:02:05 > 0:02:09from senior colleagues, and says standards of care can vary

0:02:09 > 0:02:14even between wards in the same hospital.

0:02:14 > 0:02:17Russell Hopkins has concerns, but not only because of his

0:02:17 > 0:02:21experiences as a patient.

0:02:21 > 0:02:23He's a former surgeon and hospital manager.

0:02:23 > 0:02:26In 2011, he was left with bladder and nerve damage after surgery.

0:02:26 > 0:02:30He says it was four days before he was seen by a consultant.

0:02:30 > 0:02:34On a particular weekend, you may have three or four separate

0:02:34 > 0:02:38junior doctors, not knowing the patient.

0:02:38 > 0:02:41Because if they haven't been with the patient and they haven't

0:02:41 > 0:02:43seen the early treatment, looking in the notes,

0:02:43 > 0:02:47which may be pretty awful anyway, may mean that they don't understand

0:02:47 > 0:02:51what's going wrong with the patient.

0:02:51 > 0:02:54Opposition parties here at the Senedd argue the ombudsman's

0:02:54 > 0:02:57concerns echo their own.

0:02:57 > 0:03:00They say it reflects a failing on the part of the Welsh government.

0:03:00 > 0:03:04But the Welsh government argues improvements are underway

0:03:04 > 0:03:07in delivering better care 24/7.

0:03:07 > 0:03:10I've never been attracted to some of the calls for large

0:03:10 > 0:03:12ombudsman sort of enquiries.

0:03:12 > 0:03:15I'll take advice on this proposition, and if we think that

0:03:15 > 0:03:17there's something good to be derived from it,

0:03:17 > 0:03:19then, of course, we want to do it.

0:03:19 > 0:03:21But concerns about 24/7 care aren't new, and it's also an issue

0:03:21 > 0:03:23across the border.

0:03:23 > 0:03:26The UK Government wants to impose a new contract on junior doctors

0:03:26 > 0:03:31in England, which they argue will mean better 24/7 care.

0:03:31 > 0:03:33Doctors there, who go on strike again tomorrow,

0:03:33 > 0:03:35say it will mean longer hours and poorer care.

0:03:35 > 0:03:38But the Welsh government wants to avoid a similar dispute here.

0:03:40 > 0:03:43delivered by working alongside front line staff.

0:05:36 > 0:05:40in Riverside found her remains wrapped in an off cut of carpet,

0:05:40 > 0:05:42bound with electrical wire.

0:05:42 > 0:05:46Her hands were tied behind her back, and a plastic bag had been placed

0:05:46 > 0:05:49over her head.

0:05:49 > 0:05:52As part of the efforts to identify her, a clay model

0:05:52 > 0:05:56was created using ground-breaking techniques.

0:05:56 > 0:05:58And it wasn't long before two social workers identified her as

0:05:58 > 0:06:01Karen Price.

0:06:01 > 0:06:04In 1991, two pimps were jailed for her murder.

0:06:04 > 0:06:08Alan Charlton and Idris Ali were sentenced to life in prison.

0:06:08 > 0:06:11A subsequent appeal by Charlton failed,

0:06:11 > 0:06:14but Ali's conviction was quashed.

0:06:14 > 0:06:18He admitted manslaughter and was released from prison in 1994.

0:06:18 > 0:06:21Today at a brief hearing at the Court of Appeal,

0:06:21 > 0:06:25another attempt to have their convictions quashed was rejected.

0:06:25 > 0:06:28Lawyers for the men had argued that evidence from the key witness

0:06:28 > 0:06:31in the trial, a girl from the same children's home as Karen,

0:06:31 > 0:06:35could not be relied upon.

0:06:35 > 0:06:40And Idris Ali claims that he only confessed to manslaughter in order

0:06:40 > 0:06:41to get out of prison.

0:06:41 > 0:06:45For the reasons given in the judgment handed down today,

0:06:45 > 0:06:47the appeals against conviction of Alan Charlton and Idris Ali

0:06:47 > 0:06:49are dismissed.

0:06:49 > 0:06:51This street which now faces the Principality Stadium has been

0:06:51 > 0:06:54completely transformed since the 1980s, when it was at the heart

0:06:54 > 0:06:57of Cardiff's red light district.

0:06:57 > 0:07:00It's been 27 years since Karen Price's body was found here.

0:07:04 > 0:07:07tonight, there are still many unanswered questions about how this
