0:00:54 > 0:00:59texting her, in breach of a harassment warning.

0:00:59 > 0:01:02But officers didn't go to her home, and hours later, Bowen

0:01:02 > 0:01:03stabbed her multiple times.

0:01:24 > 0:01:27The case prompted a review by the Independent Police Complaints

0:01:29 > 0:01:31amongst officers around risk assessments and some failures

0:01:31 > 0:01:40to follow force policies and procedures.

0:01:40 > 0:01:44From the start, from the time Ms Evans contacted the control

0:01:49 > 0:01:52in which all involved in Ms Evans dealt with her, they failed

0:02:22 > 0:02:25by her esranged husband near her home in Newport in 2013.

0:02:31 > 0:02:34that the force really needed to go back to the beginning,
