:00:34. > :00:38.The NHS in Wales needs to make "radical changes" in order to cope

:00:39. > :00:43.That's according to the new director of the Welsh NHS Confederation -

:00:44. > :00:46.the body which represents all the Health Boards here.

:00:47. > :00:49.Vanessa Young says there needs to be a "shift in focus" and wants

:00:50. > :00:52.the next Welsh Government to set out a "10-year vision" for the Health

:00:53. > :01:02.Here's our Health Correspondent, Owain Clarke.

:01:03. > :01:08.The politicians argue about it. We all depend on it. It's no surprise

:01:09. > :01:15.that the NHS is a top election issue. But whichever way you look at

:01:16. > :01:20.it, a clear picture emerges and that is of a service under intense

:01:21. > :01:26.pressure. And that has not gone unnoticed among those enjoying the

:01:27. > :01:30.sunshine. There are not the phones these days to pay for it sets a

:01:31. > :01:36.question of prioritising. I feel the nurses and doctors are under great

:01:37. > :01:41.strain. Anyway, the NHS as a victim of its own success. Since it it was

:01:42. > :01:45.established, the panties -- advantages in medicine means we are

:01:46. > :01:49.living longer. It also means the NHS has to work harder than ever before.

:01:50. > :01:53.A much more radical changes now needed so it is about spreading that

:01:54. > :01:58.good practice and doing it on a much larger scale. The danger is, if we

:01:59. > :02:02.don't do that, we will not be able to have an NHS that is sustainable

:02:03. > :02:06.and fit for purpose in the future. She also says more resources need to

:02:07. > :02:10.be moved from hospitals to community services and more staff need to be

:02:11. > :02:14.recruited to areas of shortages like a large parts of the North Wales.

:02:15. > :02:19.But crucially she argues the public should be at the heart of the debate

:02:20. > :02:24.about the NHS's future. But that does not always happen and you don't

:02:25. > :02:30.have to rewind the clock to far to find a classic example. Out of the

:02:31. > :02:36.blue Italy last year, this health board announced it would shut the

:02:37. > :02:40.doctor led maternity unit at... It blamed a severe shortage of junior

:02:41. > :02:46.doctors and led to an almighty row. Eventually, the decision was

:02:47. > :02:49.reserved, -- reversed, even though the recruitment crisis hasn't gone

:02:50. > :02:53.away. Bosses say they will listen more in future but to what extent

:02:54. > :02:59.other politicians listening? All the main parties are promising to pump

:03:00. > :03:02.extra millions into the NHS. The government have a rather unfortunate

:03:03. > :03:08.habit of finding a bit more money down the back of the sofa when the

:03:09. > :03:12.going gets tough. It doesn't help the NHS if it means those difficult

:03:13. > :03:14.decisions are avoided. I would appeal to candidates that they don't

:03:15. > :03:20.promise the earth because invariably, they promise to reduce

:03:21. > :03:24.one service in the NHS, given current constraints, means that

:03:25. > :03:27.somebody else loses out. But despite all the difficulties, most people

:03:28. > :03:32.are very happy with the care they get and the truth is, we could all

:03:33. > :03:35.help the NHS out by looking after ourselves better. But according to

:03:36. > :03:39.this leading children's doctor, there is another issue. If

:03:40. > :03:43.politicians become too preoccupied with dealing with the pressures of

:03:44. > :03:47.today, they might not spot the problems of tomorrow. We have the

:03:48. > :03:51.highest proportion of children with diabetes in Wales and we are

:03:52. > :03:58.spending millions on the management of diabetes in adults. If we spent

:03:59. > :04:03.that on prevention of obesity, you would save millions in the long run.

:04:04. > :04:07.Outlining one yet -- what the NHS in Wales might look like in 20 years'

:04:08. > :04:12.time is not easy. The politicians want your vote so they can be in

:04:13. > :04:17.power for the next five years. Addressing the challenges facing the

:04:18. > :04:19.NHS and dashed changing today's picture might take a whole lot

:05:47. > :05:50.are worried they'll be pushed out of the market,

:05:51. > :06:05.while others say they the Uber app has been a runaway

:06:06. > :06:08.success but has sparked anger in some cities.

:06:09. > :06:12.In London, cabbies protested and in Paris, things got even more heated.

:06:13. > :06:17.Today, it's launched in Cardiff but how does it work? If you want a

:06:18. > :06:24.taxi, go to the app, press the button and it tells you there will

:06:25. > :06:27.be a long very shortly. They already operate in 57 countries and 15 other

:06:58. > :07:01.the credit card details. It's good for both parties. If you travel

:07:02. > :07:05.along, you can monitor your journey with your smartphone and when you

:07:06. > :07:10.get to your destination, there is no cash to pay. You simply pay your

:07:11. > :07:15.fare with your online bank account. But the competition isn't standing

:07:16. > :07:19.still. Dragon is Wales's largest private hire firm. They've launched

:07:20. > :07:23.their own app and relish the challenge. More and more people are

:07:24. > :07:26.using applications. On that basis, we had to make sure that we were

:07:27. > :07:31.developing with the times. That doesn't mean, of course, that is the

:07:32. > :07:37.end of the telephone or even the walking. Back at Uber, they are

:07:38. > :07:40.confident Cardiff it's going to be a success. If it is, it will expand to
