0:00:46 > 0:00:48He had complex mental health problems, but a new report -

0:00:48 > 0:00:51seen exclusively by BBC Wales - says he would still be alive

0:00:51 > 0:00:53had he received the care he needed.

0:00:53 > 0:00:56The charity, Hafal, is now calling on the Welsh Government

0:00:56 > 0:00:58to do more to ensure health boards comply

0:00:58 > 0:01:03with the law on mental health. India Pollock reports.

0:01:06 > 0:01:09Their little sunshine - that's what Chris Wood's family

0:01:09 > 0:01:11called him when he was a child.

0:01:11 > 0:01:13But life became very difficult when he developed

0:01:13 > 0:01:18a borderline personality disorder.

0:01:18 > 0:01:20It meant he couldn't cope with intense emotions

0:01:20 > 0:01:21and would often self harm.

0:01:21 > 0:01:24He tried to kill himself 32 times in his short life.

0:01:24 > 0:01:27In January 2013, he went missing and his parents, Debbie and Robin,

0:01:27 > 0:01:28found him in a lane.

0:01:28 > 0:01:30He had hanged himself.

0:01:30 > 0:01:38We had taken him to A so many times, but always

0:01:38 > 0:01:41brought him back with us, that there was a big part of me that

0:01:41 > 0:01:44really couldn't quite believe it. It was just utterly horrendous.

0:01:44 > 0:01:47For my lovely son, we'd fought so hard to keep safe,

0:01:47 > 0:01:50to die in the street like that...

0:01:50 > 0:01:52To try and explain that to my husband's 90-year-old parents

0:01:52 > 0:01:53was just unbelievable.

0:01:53 > 0:01:59Um, it was horrendous, it really was.

0:01:59 > 0:02:02A new report has found that Chris's death would've been prevented had

0:02:02 > 0:02:04he received a reasonable standard of care, specifically a care

0:02:04 > 0:02:10coordinator to oversee a consistent approach to his complex problems.

0:02:10 > 0:02:14Debbie said that reading this report was devastating.

0:02:14 > 0:02:17It found that there was no evidence that Chris's risk to himself

0:02:17 > 0:02:21was correctly assessed the year before he died.

0:02:21 > 0:02:24It also says that his care plan was inadequate and that there

0:02:24 > 0:02:26was grave concern about the lack of the involvement of the family.

0:02:26 > 0:02:29Now there are calls for the Welsh Government to do more

0:02:29 > 0:02:32to ensure that services comply with mental health law.

0:02:32 > 0:02:35We hear too much, "We've learned lessons from these sorts of cases."

0:02:35 > 0:02:42Enough is enough and if people don't want to deliver to the mental health

0:02:42 > 0:02:44measure, they should give up delivering

0:02:44 > 0:02:45the mental health services

0:02:45 > 0:02:48themselves and give it to a health board thats capable of doing that.

0:02:48 > 0:02:50The Welsh Government says that mental health will be

0:02:50 > 0:02:51a priority for them.

0:02:51 > 0:02:52They say that they expect services to comply with national guidelines

0:02:54 > 0:02:56and that it's introduced a measure to ensure all those in secondary

0:02:56 > 0:02:58services have a care and treatment plan.

0:02:58 > 0:03:02Cardiff and Vale University Health Board has apologised to Chris's

0:03:02 > 0:03:04family and said they've taken steps to address the issues raised

0:03:04 > 0:03:05in the report.

0:03:05 > 0:03:08It also says it's been developing services for people

0:03:08 > 0:03:10with problems like Chris'.

0:03:10 > 0:03:13It's been a three-year fight for Debbie and her family.

0:03:13 > 0:03:15They're speaking out now, because they want changes for others

0:03:15 > 0:03:17like Chris, who have complex mental health needs.

0:03:25 > 0:03:27There are plans to reduce the maximum size of infant classes

0:03:27 > 0:03:28from 30 pupils to 25.

0:03:28 > 0:03:31But an education expert says it won't have much impact on learning

0:03:31 > 0:03:34and there's also concern that already overcrowded schools will

0:03:34 > 0:03:38struggle to cope with the change.

0:03:42 > 0:03:44which saw the Lib Dems' only Assembly Member Kirsty Williams

0:03:48 > 0:03:50Personally, I wouldn't do it, because I'd use the money for,

0:03:50 > 0:03:54I think, the pressing thing - the most pressing thing of all -

0:03:54 > 0:03:58which is, independent of class sizes, we haven't developed

0:03:58 > 0:04:00the capacities of our teachers to be world-class great teachers.

0:04:00 > 0:04:06That's what I would spend my money, but then, I'm not in politics.

0:04:06 > 0:04:08You speak to any teacher, they'll tell you that,

0:04:08 > 0:04:11if they have a smaller class size, they can offer better

0:04:11 > 0:04:13services to those pupils, they can identify strengths

0:04:13 > 0:04:17or weaknesses easier. They have better time for planning.

0:04:17 > 0:04:21But I'd have some concerns about the debate being do we reduce

0:04:21 > 0:04:25class sizes or do we improve teachers' professional development?

0:04:25 > 0:04:28It shouldn't be an either or, it should be how do we ensure

0:04:28 > 0:04:33a situation where we do both of those things?

0:04:33 > 0:04:35And staying with schools - a consultation on the future

0:04:35 > 0:04:37of four Secondary Schools in Powys has closed.

0:04:37 > 0:04:39The council has plans to close Gwernyfed and Brecon schools,

0:04:39 > 0:04:42and create a single school in their place, doing

0:04:42 > 0:04:46the same at Builth Wells and Llandrindod schools.

0:04:52 > 0:04:56The number of adults in Wales who say they smoke has
