0:01:04 > 0:01:07These three Army reservists, physically fit, took on that

0:01:07 > 0:01:10challenge to try and join the SAS.

0:01:10 > 0:01:12It was always going to be difficult but neglect played

0:01:12 > 0:01:15a part in their deaths.

0:01:15 > 0:01:17Their families gathered in 2015 to hear about failing after failing

0:01:17 > 0:01:23which played a part in them losing their loved ones.

0:01:23 > 0:01:25The coroner, Louise Hunt, concluded that before

0:01:25 > 0:01:27the fatal march the risk assessment was inadequate.

0:01:27 > 0:01:30It didn't consider the weather or what training the men had.

0:01:30 > 0:01:34Then during the march a GPS tracker system was supposed to show

0:01:34 > 0:01:36if a candidate was struggling, but it didn't work properly.

0:01:36 > 0:01:39There was no reference in the plans on how to spot,

0:01:39 > 0:01:44locate or retrieve struggling candidates if it was necessary.

0:01:44 > 0:01:47Even after the event she was concerned that lessons

0:01:47 > 0:01:49were not being learned.

0:01:49 > 0:01:53The Ministry of Defence was censured by the Health and Safety Executive.

0:01:53 > 0:01:56That's the most serious action it can take against the MoD because it

0:01:56 > 0:02:00has immunity from prosecution.

0:02:00 > 0:02:03Then this afternoon, the MoD published its own independent report

0:02:03 > 0:02:07and concluded that there may still be lessons to be learned four

0:02:07 > 0:02:11years after the incident on the Brecon Beacons and nearly two

0:02:11 > 0:02:16years after the conclusion of that inquest.

0:02:16 > 0:02:19The report says that as of last year, there were significant areas

0:02:19 > 0:02:22of concern and unsafe practices within the training pathway

0:02:22 > 0:02:25for the reserve units.

0:02:25 > 0:02:27As a result, training activity for those units

0:02:27 > 0:02:30was immediately stopped.

0:02:30 > 0:02:32The report's author says, "It's my opinion that these

0:02:32 > 0:02:35reservists currently remain vulnerable to a further

0:02:35 > 0:02:38incident in the future".

0:02:38 > 0:02:41The Ministry of Defence tonight says its deepest sympathies

0:02:41 > 0:02:44remain with the families of the three soldiers.

0:02:44 > 0:02:46It says it commissioned this review published today

0:02:46 > 0:02:49to learn difficult lessons.

0:02:49 > 0:02:52All of the recommendations, it says, have been or are being addressed

0:02:52 > 0:02:56to ensure that such a tragic event cannot happen again.

0:04:57 > 0:04:59You were heaping praise on the Welsh Government,

0:04:59 > 0:05:02a Labour run Welsh Government, but that is a double-edged sword

0:05:02 > 0:05:04because there are successes but there are also problems,

0:05:04 > 0:05:08especially in education.

0:05:08 > 0:05:11They have had a ?9 billion cut in their budget from the UK

0:05:11 > 0:05:15Government, they need to make sure that children get free school meals,

0:05:15 > 0:05:18they have an investment programme.

0:05:18 > 0:05:20Relations between Jeremy Corbyn and the Labour leader in Wales

0:05:20 > 0:05:22are said to be cordial but no more.

0:05:22 > 0:05:25Carwyn Jones has been luke warm at best in his support

0:05:25 > 0:05:26for the party leader.

0:05:26 > 0:05:28But for Jeremy Corbyn's supporters, they were happy

0:05:28 > 0:05:31with what they heard.

0:05:31 > 0:05:33It was solidarity, unification, and it was reaching out

0:05:33 > 0:05:36to a majority of what people actually want and what

0:05:36 > 0:05:38they actually need.

0:05:38 > 0:05:42Everything he says is practical, policies that can be put into place.

0:05:42 > 0:05:45It is no surprise that Jeremy Corbyn and Labour decided

0:05:45 > 0:05:47to come to Cardiff North to launch their campaign in Wales.

0:05:47 > 0:05:50If the party is to have any chance of forming the next UK Government,

0:05:50 > 0:05:53this is exactly the kind of seat it needs to be taking

0:05:53 > 0:05:57from the Conservatives.

0:05:57 > 0:06:00For much of the past four decades, it has been held

0:06:45 > 0:06:48ably supported by Plaid Cymru and the Liberal Democrats.
