0:02:09 > 0:02:16death. Jenny Rathbone a former colleague of Carl Sargeant, a Labour

0:02:16 > 0:02:20Assembly Member, Wales this morning says she did not feel that was fair

0:02:20 > 0:02:25at a basic level. It increases the pressure and Carwyn Jones to explain

0:02:25 > 0:02:30and justify the way his sacking was handled. We have not heard from the

0:02:30 > 0:02:35First Minister yet. Chris Davies, the Brecon and Radnorshire

0:02:35 > 0:02:40Conservative MP, has called for him to resign in the past half an hour.

0:02:40 > 0:02:43He is a political opponent and we should not necessarily be surprised

0:02:43 > 0:02:48by that but it shows the angle the Conservatives are taking on this.

0:03:52 > 0:03:56including in some cases entire organs, were kept longer

0:03:56 > 0:03:59than the University Hospital of Wales had permission to do so,

0:03:59 > 0:04:01following the conclusion of investigations by

0:04:01 > 0:04:03the police or coroner.

0:04:03 > 0:04:05The Human Tissue Authority, which conducted the inspection,

0:04:05 > 0:04:08says a significant amount of work needs to be done to bring

0:04:08 > 0:04:09standards up to scratch.

0:04:09 > 0:04:12Our health correspondent Owain Clarke has been

0:04:15 > 0:04:18The report out today highlights what are described as critical

0:04:18 > 0:04:21failings in the way the mortuary in the hospital behind me worked.

0:04:21 > 0:04:24I am joined by Len Richards, chief executive of the

0:04:24 > 0:04:28local Cardiff and Vale health board.

0:04:28 > 0:04:31What in your view is the biggest failing here?

0:04:31 > 0:04:34The report highlights some critical failings and I would suggest

0:04:34 > 0:04:40they relate to we have held onto 42 organs and tissues

0:04:40 > 0:04:43for longer than we were authorised to do so.

0:04:43 > 0:04:44What went wrong?

0:04:44 > 0:04:46Did you just forget you had tissue stored somewhere?

0:04:46 > 0:04:48Did you forget you were holding onto them

0:04:48 > 0:04:50or was it record-keeping and processes?

0:04:50 > 0:04:52It is record-keeping and processes.

0:04:52 > 0:04:56We hold onto these tissues and samples under the authority

0:04:56 > 0:04:59of either the coroner or the police force under which they were seized.

0:04:59 > 0:05:04It is our duty then to liaise with those police forces

0:05:04 > 0:05:08or the coroner to understand whether the cases have been

0:05:08 > 0:05:10conducted and concluded so we can then dispose of those

0:05:10 > 0:05:13tissues sensitively.

0:05:25 > 0:05:28In 2009, the mortuary here was closed down for a period of

0:05:28 > 0:05:29time because of similar concerns.

0:05:29 > 0:05:31The focus of our detention since the report was published

0:05:31 > 0:05:34in September has been to put things right within the organisation.

0:05:34 > 0:05:39Last week, I authorised an investigation to look

0:05:39 > 0:05:43at why we had deteriorated since the 2012 position.

0:05:43 > 0:05:46Once we have the outcome of that investigation

0:05:46 > 0:05:48we will act accordingly.

0:08:48 > 0:08:53dry from much of daylight hours even though the cloud is increasing. This

0:08:53 > 0:08:57weather front is responsible for the rain that will see later on tonight.

0:08:57 > 0:09:01Temperatures getting up to about 10 Celsius. Tonight, the cloud will

0:09:01 > 0:09:05thicken. We were sea mist and fog forming. Patchy rain coming into the

0:09:05 > 0:09:14north and west and gradually pushing south eastwards. It shouldn't be as

0:09:14 > 0:09:18chilly as last night. Tomorrow we have a ridge of high pressure but
