:00:13. > :00:25.of a pregnant woman and her two`year`old son. Why was Rachael

:00:26. > :00:30.Slack not warned that her mentally ill ex`partner posed a lethal

:00:31. > :00:35.threat. We have spent six weeks in the coroner 's court hearing of the

:00:36. > :00:42.failings of Derbyshire Police after they assessed both Rachael and Auden

:00:43. > :00:47.as being at high risk of homicide. On the day care and stabbed

:00:48. > :00:52.Rachael, Auden and himself, friends told as he could have been stopped.

:00:53. > :00:56.He was arrested for making threats that he was going to kill her, and

:00:57. > :01:02.he has been let out to do exactly that. I hope someone somewhere is

:01:03. > :01:07.accountable for what has happened, because we have lost a dear friend.

:01:08. > :01:14.Also glad, an exclusive interview with the man who blew the whistle on

:01:15. > :01:21.problems at the Al`Madinah School. There is hope for the future, but I

:01:22. > :01:26.think it needs a radical shake up. And dredging the Trent in the hope

:01:27. > :01:42.that commercial barges could soon be back in operation.

:01:43. > :01:45.Good evening. Welcome to the programme. First tonight, the

:01:46. > :01:49.long`awaited outcome of a seven`week inquest aimed at finding out how a

:01:50. > :01:52.mentally ill man was able to kill his former partner and their child `

:01:53. > :02:05.before killing himself. Here's Anne with the details.

:02:06. > :02:09.Unlawfully killed. A mother, Rachael Slack ` and her two`year`old son

:02:10. > :02:13.Auden. Unlawfully killed by the little boy's father ` this man,

:02:14. > :02:15.Andrew Cairns. Who then, in the grip of mental illness, killed himself.

:02:16. > :02:17.An inquest jury returned those verdicts today after weeks

:02:18. > :02:21.considering the killings in Derbyshire three years ago. But they

:02:22. > :02:24.also said something else. The jury decided the deaths of Rachael and

:02:25. > :02:28.Auden were "more than minimally contributed to by a failure to

:02:29. > :02:33.impress on her that she was in danger/at high risk of homicide from

:02:34. > :02:38.her ex`partner." In other words, his illness made Cairns a lethal threat.

:02:39. > :02:42.But Rachael and Auden didn't get the warning they needed. This is what

:02:43. > :02:52.Rachael's family solicitor said today after the inquest concluded:

:02:53. > :02:57.we have spent the last six weeks in the coroner's court hearing of the

:02:58. > :03:06.failings of Derbyshire Police after they assessed both Rachael and Auden

:03:07. > :03:10.as being at high risk of homicide. We have also heard concerning

:03:11. > :03:16.evidence about the failure of the police and mental health to share

:03:17. > :03:20.information. We are very grateful to the coroner and the jury for their

:03:21. > :03:23.work in ensuring that these failings have been brought to light.

:03:24. > :03:26.In a moment we'll be looking at what went wrong. First, James Roberson

:03:27. > :03:34.reports on those shocking events in the village of Holbrook.

:03:35. > :03:41.In the village of Holbrook, a bench under a shady tree reveals this is a

:03:42. > :03:47.memorial. By mid`morning on second June 2010,

:03:48. > :03:52.the whole of the centre of Holbrook was cordoned off. Eventually the

:03:53. > :03:58.horrific news emerged that inside this House, two adults and a child

:03:59. > :04:02.had been found. The man who led the inquiry believes Cairns stabbed

:04:03. > :04:07.Auden, Rachael and then himself. It would appear that Rachael probably

:04:08. > :04:14.let him in. There was no forced entry. He has got both sets of keys,

:04:15. > :04:17.he has got Rachael's phone, and then at some point he has picked up a

:04:18. > :04:21.knife. At the time Rachael's friends were

:04:22. > :04:26.appalled. This has happened because this person is mentally unstable. He

:04:27. > :04:32.was arrested for making threats that he was going to kill her, and he has

:04:33. > :04:37.been let out of to do exactly that. Rachael had met Andrew Cairns in

:04:38. > :04:43.Spain, but they both returned to her native Derbyshire. Rachael had Auden

:04:44. > :04:46.in 2008, but Cairns put `` Cairns' mental health deteriorated and the

:04:47. > :04:53.couple split up. Rachael and Auden moved into the Holbrook cottage.

:04:54. > :04:59.Neighbours were suspicious of Cairns when he visited. He looked sideways

:05:00. > :05:05.out of his eyes and I thought he was a very peculiar person. But

:05:06. > :05:09.Andrew's neighbour at his bungalow thought highly of him. Our family

:05:10. > :05:16.knew her view of Cairns and wanted us to bear it. I always found it

:05:17. > :05:26.quite urgent `` found him quite a gentleman. He was one of the noticed

:05:27. > :05:35.gentleman I knew. `` nicest gentlemen. Late in 2010,

:05:36. > :05:38.Rachael taught Cairns that she had a new relationship. Things escalated

:05:39. > :05:42.when Cairns was arrested for making threats to kill Rachael and Auden,

:05:43. > :05:47.then released on condition he kept away from them both. He was there

:05:48. > :05:52.when he was distressed, after he was told that he must not go or ring or

:05:53. > :05:59.anything to do with the boy. That is what upset him. Because that is all

:06:00. > :06:04.the words he said, I am having my little boy.

:06:05. > :06:16.Worried at how angry and `` Andrew was, Phyllis phoned the police. That

:06:17. > :06:24.then made Auden at high risk of being abducted. His risk then

:06:25. > :06:27.changed. While assessing Rachael at the time

:06:28. > :06:33.to be at high risk of homicide, the police were later unsure if they

:06:34. > :06:43.actually told her she was. Knowing Rachael was at high risk of

:06:44. > :06:46.homicide, we needed to adjust our lives. I think it would have made a

:06:47. > :06:52.difference. Derbyshire Police admitted in the

:06:53. > :06:55.inquest that a cocoon of police putting neighbours under alert would

:06:56. > :06:59.have helped as well. Especially since Rachael's neighbours spotted

:07:00. > :07:08.Andrew Cairns by the Holbrook cottage a few days before the

:07:09. > :07:12.killings. On June two, 2010, this woman heard Rachael screaming next

:07:13. > :07:16.door. She saw a shadowy figure through a window behind Rachael. By

:07:17. > :07:21.the time police and paramedics arrived it was too late. Cairns had

:07:22. > :07:25.killed Auden, a pregnant Rachael and himself. I do not think he had

:07:26. > :07:31.planned it for very long. He had been to see the doctor that morning,

:07:32. > :07:35.he had gone to the premises are unarmed. So he armed himself at the

:07:36. > :07:41.premises. He may not have gone prepared. When his attention to

:07:42. > :07:45.kill? Was it before, was it inside? I don't think anybody can say for

:07:46. > :07:49.certain. I think he was at the end of his tether, but I did not expect

:07:50. > :07:54.him to do what he did do. Rachael and Auden's death will be in

:07:55. > :08:00.absolute vain if people who are living with people with mental

:08:01. > :08:05.health problems could end up going down a similar horrific power. So it

:08:06. > :08:09.has got to change by change in procedures. Making sure things are

:08:10. > :08:11.done tighter. Making sure people have the support they deserve and

:08:12. > :08:18.they require. Why then was Rachael not warned of

:08:19. > :08:32.the danger Andrew Cairns had become? There WERE warning signs in that

:08:33. > :08:36.late spring of 2010. On the 26th of May, Cairns was detained under the

:08:37. > :08:39.Mental Health Act, assessed by a psychiatrist and released. The next

:08:40. > :08:43.day, he was arrested after threatening to kill Rachael and take

:08:44. > :08:46.Auden. However, on the 28th, he was released so further inquiries could

:08:47. > :08:49.take place ` on condition he did not contact Rachael. On June the 2nd,

:08:50. > :08:53.the killings. James Roberson was at the inquest's ending today for the

:08:54. > :08:59.reaction of both families. The results of a serious case review

:09:00. > :09:04.and an inquiry by the IP CC, best `` both yet to be published, limited

:09:05. > :09:08.Derbyshire Police's response today. I would like to express my deepest

:09:09. > :09:15.sympathy to the family of Rachael and Auden.

:09:16. > :09:19.He believes Derbyshire Police's actions in June 2010 did meet the

:09:20. > :09:24.national guidelines of the time. From the evidence it is clear that

:09:25. > :09:29.Rachael's report of Andrew Cairns' verbal threat to kill was taken

:09:30. > :09:33.seriously by the police. We were actively investigating that threat

:09:34. > :09:38.with a view to charging him, and we took steps to ensure that Rachael

:09:39. > :09:41.and Auden's home were secure. The family of Andrew Cairns has

:09:42. > :09:46.expressed their extreme disappointment that more was not

:09:47. > :09:54.done by the mental health trust to provide the proper level of care for

:09:55. > :09:58.him. Derbyshire health care trust say

:09:59. > :10:02.they will assess the jury's findings and the coroner's comments. Robert

:10:03. > :10:09.Barlow summed up what everyone had lost. Somebody that was so kind and

:10:10. > :10:16.so on condition with their love, and respect for life, and that is what

:10:17. > :10:20.we all lost, not just me. The family, friends, everybody. She was

:10:21. > :10:23.a very, very inwardly beautiful woman.

:10:24. > :10:25.James joins us now from the coroner's court. The coroner has

:10:26. > :10:39.three recommendations for the authorities, James. Yes, the coroner

:10:40. > :10:44.said he hoped that this would bring some closure to the families

:10:45. > :10:47.affected by the tragedy, and he has those three recommendations. He is

:10:48. > :10:53.asking the Home Secretary to look at bail conditions as the Powys police

:10:54. > :10:56.have to detain people before charges, and a possible change in

:10:57. > :11:00.the law on violent and sexual offences. Also, with two women a

:11:01. > :11:05.week being killed in the UK, better training for the police on domestic

:11:06. > :11:10.violence. Secondly he is writing to Derbyshire's Chief Constable, the

:11:11. > :11:14.Home Secretary about ensuring `` improvements police forces can make

:11:15. > :11:18.in record keeping doing domestic incidents. Finally, a joint report

:11:19. > :11:23.the mental health trust and the police to ensure `` exchange more

:11:24. > :11:27.information in the future about such incidents. Did anybody except the

:11:28. > :11:35.bigger picture, did anybody join up the dots?

:11:36. > :11:38.Next tonight, the inside story of how a headmaster turned into a

:11:39. > :11:49.whistle`blower and initiated a crisis at the controversial Islamic

:11:50. > :11:52.free school in Derby. Andrew Cutts`McKay was the Principal of the

:11:53. > :11:56.Al`Madinah School in Derby. Speaking for the first time he says

:11:57. > :12:04.his last term at the school became very difficult. And he eventually

:12:05. > :12:07.quit after receiving an anonymous threat that his children would be

:12:08. > :12:12.harmed unless he left. He's been speaking exclusively to our

:12:13. > :12:18.reporter, Simon Hare. Until now he has declined to

:12:19. > :12:22.comment, but now Andrew Cutts`McKay feels he can have his say about what

:12:23. > :12:26.happened at the Al`Madinah School in Derby. He helped to set it up and

:12:27. > :12:30.open it in September last year, but by the end of the last time he had

:12:31. > :12:36.become concerned at how the school was being managed, particularly its

:12:37. > :12:43.finances. There were fairly major concerns, and what I suggested what

:12:44. > :12:49.`` was that a meeting take place to raise our concerns, and it was then

:12:50. > :12:53.given to them to investigate. You were the whistle`blower that brought

:12:54. > :12:59.in the outside bodies to investigate the Al`Madinah School. How do how

:13:00. > :13:06.difficult was that for you? Very challenging indeed. It has been

:13:07. > :13:10.challenging for my family and my children. He handed in his

:13:11. > :13:16.resignation after an anonymous threat was made towards his family.

:13:17. > :13:21.The threat that was made was against my children, and I don't know who

:13:22. > :13:28.made it. I made this clear, I gave evidence to the Department for

:13:29. > :13:32.Education. I don't know if it was racially motivated, I don't know if

:13:33. > :13:36.it was faith motivated. I knew that I had received this, there was never

:13:37. > :13:42.any reference to faith in it, it was just very specific. Unless you do

:13:43. > :13:49.this, this will happen. What did you have to do? Unless you leave the

:13:50. > :13:53.school, this will happen. But he still hopes the school survives and

:13:54. > :13:58.can be turned around. I am hopeful that it will be `` stay open, there

:13:59. > :14:04.are wonderful staff there. The children I miss every single day,

:14:05. > :14:10.they are wonderful children. I do believe that actually there is hope

:14:11. > :14:15.for the future of the school, but it does need to change, and I think it

:14:16. > :14:19.needs a radical shake up. He himself is due to resume his career as the

:14:20. > :14:24.head of a new free school at Newark in Nottinghamshire.

:14:25. > :14:27.A Nottingham man and his mother arrested on suspicion of kidnap and

:14:28. > :14:33.attempted child abduction are still being questioned by the police.

:14:34. > :14:37.Two`year`old Na'ilah Khan and her ten`month`old brother Sulaimon were

:14:38. > :14:40.taken from a hotel room in Stoke on Trent on Friday. Their father Saleem

:14:41. > :14:45.Tahir and his mother Denise Smith`Sellers were arrested last

:14:46. > :14:49.night at a guesthouse in Derby. The children are safe and well. In a

:14:50. > :14:53.statement their mother said it was like a part of her had been missing.

:14:54. > :14:57.It's emerged that Government inspectors forced the closure of a

:14:58. > :15:00.care home in Nottingham. It was announced yesterday that St

:15:01. > :15:03.Andrews Lodge in Basford would shut following a number of serious

:15:04. > :15:10.concerns about the care and safety of its residents ` many of whom have

:15:11. > :15:14.dementia. The Care Quality Commission says taking action to

:15:15. > :15:16.force a closure is only ever done as a last resort.

:15:17. > :15:20.Another 1,500 school places are expected to be created in Leicester.

:15:21. > :15:24.The City Council has put forward plans to invest ?16 million in

:15:25. > :15:29.primary schools in the city. The money is likely to be used to expand

:15:30. > :15:32.schools across the city. It's the second phase of improvements aimed

:15:33. > :15:35.at meeting a growing demand for places.

:15:36. > :15:39.Care homes across Nottinghamshire will get more money if they can

:15:40. > :15:43.demonstrate a higher standard of care for people with dementia.

:15:44. > :15:50.Social services are giving the extra payments to those awarded their new

:15:51. > :15:53.quality mark ` that's 32 so far. The awards have been brought in to

:15:54. > :15:58.try to improve the care surrounding one of the biggest health challenges

:15:59. > :16:12.of our age. This report from our Health correspondent Rob Sissons.

:16:13. > :16:18.Myra recalls looking after prisoners of war in the 1940s. Her friend has

:16:19. > :16:22.dementia. Like many relatives, her daughter felt guilty when she went

:16:23. > :16:26.into a whole, but says it has worked out. I have had such a life of

:16:27. > :16:35.moving around over the place, you just adapt when you are at that

:16:36. > :16:42.particular position. You don't feel institutionalised at all.

:16:43. > :16:50.Some of the homes are quite scruffy, and they don't really have

:16:51. > :16:54.enough staff. Dementia is now one of the biggest health challenges of our

:16:55. > :16:58.age. The latest Government estimates suggest 58,000 people living with

:16:59. > :17:04.dementia in the East Midlands, and that will go up by 25% by the year

:17:05. > :17:10.2021 if National projected trends are reflected here. We are looking

:17:11. > :17:16.for a style of care that encourages people with dementia to be involved,

:17:17. > :17:26.and to communicate, and to be stimulated with a range of

:17:27. > :17:32.activities and good conversation. This home in West Bridgeford is one

:17:33. > :17:39.home awarded the Care Quality Commission quality mark. They have a

:17:40. > :17:42.background, history, and they have interests we want to continue

:17:43. > :17:46.supporting them to do. Getting residents out and about is valued as

:17:47. > :17:54.part of the quality mark, and today it is something to raise a glass to.

:17:55. > :17:57.A trip to the local wine bar. It's hoped that commercial freight

:17:58. > :18:03.barges could soon return to ferry cargo along the River Trent. The

:18:04. > :18:06.remaining half a dozen barge operators stopped working there in

:18:07. > :18:15.the summer after a quarry company said it would no longer use the

:18:16. > :18:19.river to transport materials. Well, now a million pounds is being

:18:20. > :18:22.spent on clearing the Trent's navigation routes to keep them clear

:18:23. > :18:29.enough for cargo vessels. Mike O'Sullivan has this report.

:18:30. > :18:35.On the River Trent near Cromwell in Nottinghamshire.

:18:36. > :18:39.A huge dredging operation is under way, from a floating platform

:18:40. > :18:45.supported by legs that stand on the river bed. A digger plunges beneath

:18:46. > :18:50.the water, reaching deep down 12 feet or so, taking away the material

:18:51. > :18:54.that can block the navigation routes along the river. This target takes

:18:55. > :18:58.away the full hopper. It is captained by a man who was made

:18:59. > :19:02.redundant during the summer, when the last commercial barges stopped

:19:03. > :19:07.work on the Trent. A big quarry company ended its operations. I got

:19:08. > :19:13.made redundant, so we are finding other ways `` means of work. How do

:19:14. > :19:19.you feel that there is no barge work now on the Trent? I think it is very

:19:20. > :19:24.sad. It was cheap, it was efficient, it was friendly. This

:19:25. > :19:28.dredging operation is costing around ?1 million. They are targeting

:19:29. > :19:33.pressure points along the 45 Bal stretch of the River Trent.

:19:34. > :19:37.Back in the heyday of water cargo, millions of tonnes a year were

:19:38. > :19:42.transported. Today it is less than half a % nationally. The dredging

:19:43. > :19:46.operation is being funded by the canal and River and Canal Trust,

:19:47. > :19:50.which has to maintain navigational standards. The work will help

:19:51. > :19:54.leisure craft, but it could help to bring back the barges. Should the

:19:55. > :19:58.freight operators be looking for ways to transport the materials,

:19:59. > :20:02.they can be sure that the river will be able to provide that function for

:20:03. > :20:06.them. The dredging operation should soon be over, but when will the

:20:07. > :20:09.barges be back in business? From the razzmatazz of Broadway to

:20:10. > :20:12.Shakespearean tragedies, theatre director Paul Kerryson has brought

:20:13. > :20:18.an eclectic mix of shows to Leicester.

:20:19. > :20:23.Along the way he's discovered a few stars and switched from one

:20:24. > :20:26.theatrical venue to another. In the latest of our Made in Leicester

:20:27. > :20:30.series, our arts reporter Geeta Pendse found out why Paul's plan to

:20:31. > :20:48.stay for a year in the city turned into two decades.

:20:49. > :20:56.Auditions for the latest production of Chicago coming next month, and it

:20:57. > :21:00.has special meaning for the `` artistic director who started out

:21:01. > :21:07.his career in Leicester 21 years ago with a then little`known show.

:21:08. > :21:11.What was it like starting out opening with Chicago? It was a show

:21:12. > :21:16.that nobody knew, but for me it was very important that I started well

:21:17. > :21:21.at Leicester, because I was only here on a year's contract, so I had

:21:22. > :21:25.to make my first choice a good one, and Chicago of course went down a

:21:26. > :21:31.bomb, because a lot of the critics and audiences had never seen it

:21:32. > :21:36.before, so they discovered it. It is the world of musicals that

:21:37. > :21:44.earned him critical praise, from the Haymarket days to the run of big

:21:45. > :21:48.number shows. The joy of the singing, dancing, visuals,

:21:49. > :21:52.everything coming together, it is like so many talents coming

:21:53. > :21:57.together. If it works, you cannot beat it.

:21:58. > :22:10.Along the way Paul has also spotted some future stars, like Eddie

:22:11. > :22:17.Izzard. Pas Minder started off tearing

:22:18. > :22:25.tickets, then she was in the youth theatre, then she just sort of went

:22:26. > :22:31.on and look at her now so after 21 years, what keeps him excited?

:22:32. > :22:35.I love the theatre here. I love the atmosphere here. And I am always

:22:36. > :22:38.trying to bring the best in Leicester, getting the best out of

:22:39. > :22:46.Leicester, just basically entertaining people.

:22:47. > :22:48.It is a great theatre. Another crowd`pleaser, Colin with

:22:49. > :22:50.the sport. First tonight, we understand

:22:51. > :22:53.Nottingham Forest striker Dexter Blackstock is leaving on a

:22:54. > :22:57.three`month loan deal to Leeds United. He's struggled to break into

:22:58. > :23:01.the Forest line`up this season but has attracted plenty of interest `

:23:02. > :23:05.Brighton were also interested. We understand that Blackstock chose to

:23:06. > :23:08.go to West Yorkshire. The deal doesn't allow the forward to play

:23:09. > :23:11.against Forest. And final confirmation from Derby

:23:53. > :23:55.way this weekend and just like they did with the Olympics, Loughborough

:23:56. > :23:57.University's playing a huge role. Kirsty Edwards reports.

:24:50. > :24:52.genuinely all about rugby union here in the West Midlands. Leicester
