:00:00. > 3:59:59Cabinet minister Tony Benn who's died aged 88. That's all from the

:00:00. > :00:30.BBC News at Six Tonight, baby is given a potentially

:00:31. > :00:34.lethal dose of morphine. Her lung collapsed and heard kidney failed.

:00:35. > :00:38.Also tonight ten hour stand`off comes to an end.

:00:39. > :00:40.Learning the hard way Dasher the Education Secretary in

:00:41. > :00:44.Nottinghamshire on improving schools, the teachers strikd and

:00:45. > :00:47.continuing reforms. And with the Fox's favouritd to

:00:48. > :01:00.return to top`flight football, what could it mean for Leicester City,

:01:01. > :01:05.the fans and Leicester itself? Good evening. First tonight, the

:01:06. > :01:11.hospital were a baby was given a massive overdose of morphind, 1 0

:01:12. > :01:14.times the recommended amount. Baby Ellie Prudence was just three

:01:15. > :01:19.weeks old. The blunder happdned because the proper checks wdren t

:01:20. > :01:21.done. Her parents are considering legal action against the hospital `

:01:22. > :01:25.King's Mill in Nottinghamshhre. The NHS calls it a "never event `

:01:26. > :01:30.something, as the name suggdsts that should never happen. The family

:01:31. > :01:33.has been talking exclusivelx to our Health Correspondent, Rob Shssons.

:01:34. > :01:38.It did not start off as an emergency. She had a runny nose and

:01:39. > :01:42.was not feeding welcome and she was wheezing, so I thought I wotld her

:01:43. > :01:46.to the hospital. That was King's Mill in Ashfield in

:01:47. > :01:49.Nottinghamshire. It was manic, people did not seem to

:01:50. > :01:54.know what they were doing. Struggling with peeling, Ellie

:01:55. > :01:59.should have been given 0.27 mg of morphine, and instead received 7

:02:00. > :02:03.mg, 100 times the recommenddd amount.

:02:04. > :02:07.I knew it was too much morphine I challenged the doctor on that but

:02:08. > :02:12.I am not a doctor, so I really did trust these people. It was human

:02:13. > :02:15.error. Two doctors and nurses involved

:02:16. > :02:20.without proper checks the doses was that might go the doors was given. I

:02:21. > :02:24.said I don't care, just get in there and help her.

:02:25. > :02:25.It took one hour to identifxing the mistake.

:02:26. > :02:29.Absolutely fuming. I could not understand how that

:02:30. > :02:33.could happen. After all, we were in a hospital, entrusting our daughter

:02:34. > :02:39.to that there. That's my conveyor Kear. Ellie was transferred to

:02:40. > :02:41.Sheffield hospital and intensive care.

:02:42. > :02:45.The family see the care of their was fantastic.

:02:46. > :02:52.Nobody expected her to survhve. These procedures if they had been

:02:53. > :02:56.followed, my daughter would not have been overdosed. The family's

:02:57. > :03:00.solicitor is in Nottingham. She said King's Mill had bedn open

:03:01. > :03:03.and honest about the mistakd. Michelle and Kyle have been through

:03:04. > :03:07.a harrowing experience which is not over for them yet, Ellie is not out

:03:08. > :03:12.of the woods. We do not know the indications for

:03:13. > :03:15.her long`term health. I did not start `` stop crying for a week

:03:16. > :03:20.after it happened, because we nearly lost her.

:03:21. > :03:23.An absolute miracle she is still here.

:03:24. > :03:26.I'm sure of the impact of the overdose, Ellie will have checks in

:03:27. > :03:31.hospital every few weeks. `` unsure. So gorgeous, and so

:03:32. > :03:38.ghastly for the parents. What have ice had to say? Tonight

:03:39. > :03:42.they have apologised and Serie A very sorry for what happened and

:03:43. > :03:45.lessons have been learned. What is being done to stop this

:03:46. > :03:48.being repeated? They are going to change thd

:03:49. > :03:51.labelling of drugs, procedures have been changed at the hospital and

:03:52. > :03:56.lessons will be shared across the NHS.

:03:57. > :04:02.I should also say that "nevdr event" s do not happen all the timd, thank

:04:03. > :04:07.goodness. 140 plus last year over six months in England, things like

:04:08. > :04:13.surgery leaving bandages after operations, not all overdosds like

:04:14. > :04:16.this case. I should say that King's Mill have also been in the news for

:04:17. > :04:21.another reason today, the tragic case of another baby. You'rd

:04:22. > :04:24.absolutely right. A coroner told the grieving parents

:04:25. > :04:28.of a baby who died after behng born at King's Mill that some of the care

:04:29. > :04:32.they had received was sub`standard. The couple were told baby Rhley had

:04:33. > :04:36.died at birth, only for thel to realise he was still breathhng.

:04:37. > :04:43.After an inquest today they described how they had been through

:04:44. > :04:46.hell. Sarah Teale reports. Daniel and his partner, Jodhe,

:04:47. > :04:51.described their ordeal at Khng's Mill was utterly horrendous.

:04:52. > :04:59.Complications arose from a rare undiagnosed condition called far as

:05:00. > :05:04.a PVR, where they baby, Rildy, was starred of oxygen during birth. He

:05:05. > :05:08.arrived showing no signs of life and attempts to resuscitate him feel

:05:09. > :05:13.that he was pronounced dead. Over the next hour the couple re`lised he

:05:14. > :05:17.was still alive. Our hopes were raised that Riley would livd, but by

:05:18. > :05:20.this time he had been starvdd of oxygen.

:05:21. > :05:26.He died two days later. We went through hell being told not once but

:05:27. > :05:30.twice our baby had died. An inquest found the staff had `` had followed

:05:31. > :05:36.all guidelines. The coroner acknowledged thdre were

:05:37. > :05:39.some issues of sub optimal care given during the labour and after

:05:40. > :05:44.birth, but she said she belheved there was not one course of action

:05:45. > :05:49.that could have changed the outcome for Riley. The family say evidence

:05:50. > :05:53.heard at the inquest suggests Riley would have been in distress for some

:05:54. > :05:55.two hours before he was born, something medical staff did not pick

:05:56. > :06:00.up. That day in February 2013 should

:06:01. > :06:02.have been one of the happiest of their lives and instead enddd in

:06:03. > :06:07.tragedy. It is possible Riley's life could

:06:08. > :06:09.have been saved with altern`tive management and this could bd

:06:10. > :06:15.investigated further through a civil claim for negligence.

:06:16. > :06:18.A terrible ordeal for this couple. Even if Riley couldn't have been

:06:19. > :06:21.saved, his parents were tre`ted with sub standard care?

:06:22. > :06:28.Yes, the hospital described his death as a tragic but natur`l event.

:06:29. > :06:31.They have said there are ardas they can improve income and that has been

:06:32. > :06:36.done. They have also said they want to stress that care at the hospital

:06:37. > :06:50.remains safe and excellent for mums and babies. For nowadays,, thank

:06:51. > :06:55.you. Derbyshire's chief fire offhcer has

:06:56. > :06:58.been charged with rape. Polhce say the charge against Sean Fraxne

:06:59. > :07:01.relates to an alleged inciddnt in Etwall seven years ago. The

:07:02. > :07:04.47`year`old, who is from thd village, was arrested last lonth and

:07:05. > :07:07.charged yesterday afternoon. He is due to appear before magistrates

:07:08. > :07:09.later this month. Derbyshird Fire Service says he's been suspdnded

:07:10. > :07:11.until further notice. We have some breaking news now, we

:07:12. > :07:14.have just received pictures of a serious fire in the centre of Derby.

:07:15. > :07:17.They were taken by our reporter James Robison, they show an awful

:07:18. > :07:22.lot of flames coming from the roof of the assembly room car park. We

:07:23. > :07:26.understand that ten pumps are at the scene, but we can now speak to James

:07:27. > :07:31.on the telephone. What can xou see from where you are? Yes, I `m

:07:32. > :07:35.standing by the assembly rooms. It is a seven story building, the whole

:07:36. > :07:39.of the top of the building has been heavily on fire with flames starting

:07:40. > :07:43.at one end and a spreading right across the hall of the top of the

:07:44. > :07:48.building. As you say, there are a lot of fire engines here. They are

:07:49. > :07:53.spraying water from the grotnd, trying to get the high`rise pump up

:07:54. > :07:58.there but it has been difficult because of the seat from thd fire.

:07:59. > :08:03.So far, the top of the building is burned out. There are still cars in

:08:04. > :08:08.the floors below, but, luckhly, so far, the fire has not spread to the

:08:09. > :08:12.other floors. At the moment it is a serious situation, hundreds of

:08:13. > :08:17.people are watching this, btt it does seem at last the fire hs the

:08:18. > :08:23.mission `` diminishing. Any updates, we will of course bring

:08:24. > :08:26.you that later in the progr`mme The emergency services were called to a

:08:27. > :08:29.bus crash in Derby this aftdrnoon. The double decker was travelling

:08:30. > :08:32.down Peartree Road when it hit a wall outside a shop. The police

:08:33. > :08:36.fire service and ambulance staff were all called at around 2pm. It

:08:37. > :08:40.was carrying passengers at the time, but no`one was seriously injured.

:08:41. > :08:46.It's not yet clear how it h`ppened. You're watching East Midlands Today,

:08:47. > :08:49.and a man remains in police custody after a ten hour stand`off finally

:08:50. > :08:52.came to an end during the e`rly hours of this morning. It bdgan

:08:53. > :08:54.yesterday afternoon at a snooker hall in the centre of Huckn`ll in

:08:55. > :08:57.Nottinghamshire. Our cameraman, John Ross, c`ptured

:08:58. > :09:00.the moment two women who'd been hiding in the building were led to

:09:01. > :09:04.safety. And then further dr`matic footage of armed officers moving in

:09:05. > :09:14.to make an arrest. Simon Hare has more.

:09:15. > :09:16.Come out nice and slow, I promise late `` promise you will be arrested

:09:17. > :09:32.properly. officers fire plastic around to

:09:33. > :09:37.subdue the man and move to lake an arrest. `` officers fire a plastic

:09:38. > :09:46.round. Police were called to reports of a

:09:47. > :09:50.man acting suspiciously at ` snooker hall, further enquiries revdaling he

:09:51. > :09:54.may have had done. For a short time periods were to collect thehr

:09:55. > :10:01.children from a school withhn a chord, and residents who cotld not

:10:02. > :10:09.return home were housed at the time's leisure centre. I got home

:10:10. > :10:15.about 3:15pm and saw police setting up cones and walling off thd road.

:10:16. > :10:19.I presumed we could go up, but they would not tell me anything. I did

:10:20. > :10:23.not know where any of this was happening. The police officdrs told

:10:24. > :10:27.me it could be ten minutes or ten hours. I just want to go hole, I

:10:28. > :10:31.have been at work they had wanted to go home and relax.

:10:32. > :10:37.8:40pm and police moved in to rescue two women who had been hiding inside

:10:38. > :10:41.the club. Both were unharmed. Police say they

:10:42. > :10:47.were left extremely distressed following their ordeal. Is thought

:10:48. > :10:53.they had been taxed in relatives from inside. For hours on and police

:10:54. > :10:56.bring a 20`year`old man out of the snooker club. `` it is thought they

:10:57. > :11:04.had been text thing. He has been arrested with possessing a firearm

:11:05. > :11:10.with intent to cause fear. Police praised the community for their

:11:11. > :11:13.patience during the operation. The Education Secretary, Michael

:11:14. > :11:16.Gove, says schools in Nottingham have been too bad for too long. His

:11:17. > :11:19.criticism follows the decishon by Ofsted to put almost half the city's

:11:20. > :11:21.secondary schools into spechal measures.

:11:22. > :11:24.On a visit to Nottinghamshire and Derbyshire today, Mr Gove s`id

:11:25. > :11:31.failing schools needed new leadership. Here's our Political

:11:32. > :11:36.Editor, John Hess. A poisonous wart at the end of

:11:37. > :11:40.his... These children in Hucknall were getting to know the gr`ph alone

:11:41. > :11:45.today courtesy of the Education Secretary who was reading stories to

:11:46. > :11:48.class blue that Edward prim`ry. As Michael getting to know the

:11:49. > :11:51.Gruffalo. The school itself had a bit of a scare three years `go when

:11:52. > :11:57.Ofsted inspectors judged it inadequate and put it into special

:11:58. > :12:03.measures. A new headteacher and new leadership turned it around. It is a

:12:04. > :12:13.lot of hard work, giving thd staff improve and develop their practice.

:12:14. > :12:17.The quality of education chhldren enjoy in the city has been too poor

:12:18. > :12:21.for too long. Last year, allost 2000 schools were inspected by Ofsted in

:12:22. > :12:26.the East Midlands. 16% were judged outstanding, but

:12:27. > :12:32.that is lower than the national average. 61% were viewed as good,

:12:33. > :12:39.but 19% were regarded as repuiring improvement, 3%, 68 schools, thought

:12:40. > :12:42.to be inadequate. We need to change leadership.

:12:43. > :12:45.We have to nature we have the right leadership teams and headte`chers in

:12:46. > :12:48.place. The also have to raise the levels of ambition for children

:12:49. > :12:53.This Education Secretary has struggled to win over teachdrs on

:12:54. > :12:56.the issues of academies and free schools, especially.

:12:57. > :12:59.The next strike is one week on Wednesday. We will see what

:13:00. > :13:01.happens. I sympathise with the difficulties

:13:02. > :13:06.teachers face. We are asking a lot of them and

:13:07. > :13:16.their work load is... There are pressures, absolutely.

:13:17. > :13:21.I go go's ambition is to improve school 's `` schools and for him it

:13:22. > :13:25.starts in classrooms like this. And you can see a full interview

:13:26. > :13:28.with Michael Gove on the Sunday Politics for the East Midlands. Join

:13:29. > :13:32.Andrew Neil and Marie Ashby from 11am.

:13:33. > :13:35.Now to a huge education success The head of a Nottinghamshire college

:13:36. > :13:39.has become the first Indian`born woman to be made a Dame in lore than

:13:40. > :13:42.80 years. Asha Khemka, the principal of West Notts College, recehved her

:13:43. > :13:45.honour from the Prince of W`les at Buckingham Palace earlier today She

:13:46. > :13:47.was recognised in the New Ydars Honours for her services to

:13:48. > :13:51.education. Staying with the Royal Mews, of a

:13:52. > :13:55.kind, it seems we will have to wait another few weeks to find ott if

:13:56. > :14:01.Richard III is definitely going to be reburied in Leicester Cathedral.

:14:02. > :14:04.`` staying with Royal news. After a two day judicial review, three

:14:05. > :14:07.judges have retired to conshder their verdict. A group is

:14:08. > :14:10.challenging the original decision to reinter the king in Leicestdr, where

:14:11. > :14:13.he was found. They claim evdryone, from the Queen to the gener`l

:14:14. > :14:16.public, should be consulted. From the High Court, here's our Chief

:14:17. > :14:19.News reporter, Quentin Rayndr. The judges were told it was time to

:14:20. > :14:22.let Richard III's remains bd reinterred in the beautiful setting

:14:23. > :14:25.of Leicester Cathedral and that under the license the city council

:14:26. > :14:31.had no role in determining where he should be re`buried.

:14:32. > :14:34.The Plantagenet Alliance argued that once the remains were identhfied as

:14:35. > :14:38.those of a lost King, everyone to the Queen from the Church of England

:14:39. > :14:41.to citizens of York and Leicester and distant relatives should have

:14:42. > :14:44.been consulted by the Justice Secretary about the reinterlent

:14:45. > :14:48.site. After two days, how do people think it went?

:14:49. > :14:53.We have always thought we h`d a strong and convincing case. The

:14:54. > :14:56.application made clear that the plan was always to reinter in Lehcester

:14:57. > :14:59.Cathedral and that has been our position consistently and h`s been

:15:00. > :15:03.publicly our position from before the time the dig took place and we

:15:04. > :15:07.have never changed from that. That is why you think you should

:15:08. > :15:10.win? That is why. I am very confident that is the

:15:11. > :15:13.right outcome and I have he`rd nothing in court that diminhshes

:15:14. > :15:18.that confidence. But I have also always said that I am not a lawyer

:15:19. > :15:21.or the judge, it is a matter of law that it will be decided upon, so

:15:22. > :15:24.let's wait and see. Are you confident deep down?

:15:25. > :15:28.Deep down I'm waiting and sdeing. At one stage Mr Justice Ousdley

:15:29. > :15:31.interjected by saying that Richard III would have raised an eydbrow

:15:32. > :15:34.that 500 years on there would be a public consultation about hhs

:15:35. > :15:39.reburial. Kings of that era, he said, were not democrats. L`dy

:15:40. > :15:42.Justice Hallett told the QC for the Alliance, "I have found your case

:15:43. > :15:46.difficult to understand." I still think we have a good chance

:15:47. > :15:50.of winning, so I would like to think that we have a very good ch`nce of

:15:51. > :15:59.winning. A ruling is expected within the next

:16:00. > :16:04.four weeks. It goes on and on. We will bring you

:16:05. > :16:12.that news. Sport is coming up next, and a look

:16:13. > :16:15.at the new film of the East Midlands actress Vicky McClure.

:16:16. > :16:20.And in the weather, it will be dry, yes, it will be warm, and yds, it

:16:21. > :16:23.will be sunny. That is your weekend forecast in a

:16:24. > :16:31.nutshell, but I will have all the details later.

:16:32. > :16:39.She will be here. Before th`t, Colin is here with the detail in sport.

:16:40. > :16:42.It has been ten years since they were last in the top flight, but

:16:43. > :16:46.Leicester City are odds on favourites to be back there and

:16:47. > :16:51.soon. For the fans it will bring an end to a long, long wait. Btt what

:16:52. > :16:53.will it mean for the club and the city itself. Angela has been finding

:16:54. > :16:57.out. In footballing terms, it is the

:16:58. > :17:01.promised land ` a return to the Premier League would be worth at

:17:02. > :17:06.least ?60 million to Leicester City, and the windfall would not stop at

:17:07. > :17:10.the King Power Stadium. The city would reap rewards, too. We know the

:17:11. > :17:12.Premier League is a huge global product in a way the Championship

:17:13. > :17:16.isn't. It is seen in more than 200

:17:17. > :17:20.countries around the world, meaning your advertising Leicester hn a way

:17:21. > :17:26.we were not before. The brand value is enormous.

:17:27. > :17:29.Enormous indeed. When Swansda city won promotion to years ago ht

:17:30. > :17:35.generated more than ?50 million for the local economy. Could Lehcester

:17:36. > :17:39.be looking at a similar boost? In financial terms for both the club

:17:40. > :17:44.and the city, it is much better than being at the top of the

:17:45. > :17:46.Championship, being at the bottom of the Premier League. The Prelier

:17:47. > :17:50.League is such a global bushness now it is crucial that clubs and cities

:17:51. > :17:53.of this kind are playing at that level.

:17:54. > :18:00.Players who have already enjoyed promotion elsewhere are relhshing

:18:01. > :18:05.the prospect of doing it ag`in. The money it generates, the games on

:18:06. > :18:09.TV, it is a bit of a boost for everyone that plays, the fans, and

:18:10. > :18:14.everyone wants to be at the top of their game and in the Premidrship.

:18:15. > :18:17.It would be nice. It is nicd to hear Leicester being talked about in the

:18:18. > :18:20.same conversation as Man City and Arsenal.

:18:21. > :18:24.It puts us on the map. It would be brilliant for the club, the city and

:18:25. > :18:28.everyone, really. With teams like Manchester Tnited

:18:29. > :18:31.and Arsenal coming to Leicester it is bound to bring more fans to the

:18:32. > :18:35.ground and improve the season as a whole. No one is taking anything for

:18:36. > :18:41.granted, but with the team seven points clear at the top and unbeaten

:18:42. > :18:45.in 15 Championship matches, it looks like Leicester City's long wait for

:18:46. > :18:52.a return to the Premier League could soon be over.

:18:53. > :18:55.Now Derby, who sit eight pohnts away from automatic promotion after three

:18:56. > :18:59.games without scoring. So two key questions as they travel to Reading

:19:00. > :19:02.` in a moment, Craig Bryson on how to get the goals flowing, btt first

:19:03. > :19:14.Steve McClaren on whether the top two is still a target.

:19:15. > :19:17.We have got ourselves into ` good position. We are in the top six and

:19:18. > :19:21.want to stay here. That is where the pressure will come, it is already

:19:22. > :19:24.coming. We are there, we ard in contention, and that has to be some

:19:25. > :19:29.kind of a miracle compared to six months ago.

:19:30. > :19:34.We still have the same qualhty in the squad, but teams are looking at

:19:35. > :19:38.how they play, they know th`t if they let us play we will be`t them

:19:39. > :19:42.because we are a really good food bowling team with a lot of `ttacking

:19:51. > :19:53.Nottingham Forest Manager Bhlly Davies is appealling against part of

:19:54. > :19:57.his five`match touchline ban for deliberately making contact with a

:20:07. > :20:14.Kirsty Edwards has been to leet Club Captain Chris Cohen ` one of many on

:20:15. > :20:19.the sidelines. On the long road to recoverx, Chris

:20:20. > :20:23.Cohen is ahead of schedule, but his knee injury will keep him ott until

:20:24. > :20:27.next season. He is not alond in the treatment room. I cannot relember a

:20:28. > :20:31.season like it. With any team that have had such bad

:20:32. > :20:35.injuries. It is probably sedn by others as an excuse, we keep talking

:20:36. > :20:39.about this, but the difficult thing for the manager I think is that he

:20:40. > :20:43.is not able to pick a consistent team. The players that have come

:20:44. > :20:46.enough done so well for us, we have great players, but it is hard when

:20:47. > :20:52.every week someone different is playing next to you. This is the

:20:55. > :20:58.Seven players currently unavailable, all suffering long`term injtries.

:20:59. > :21:02.Every week we are coming in and there seems to be someone else.

:21:03. > :21:07.We're not talking about anything anyone can do anything about. We

:21:08. > :21:10.have fantastic physiotheraphsts and sports scientists. You are not

:21:11. > :21:15.talking about pulled hamstrhngs but serious injuries. Chris's dtties are

:21:16. > :21:19.not on hold. He is often out representing

:21:20. > :21:23.Nottingham Forest in the colmunity, like this visit to a school. My best

:21:24. > :21:27.moment was my first day of the season we got promoted on the last

:21:28. > :21:31.day of the season. Hopefully we will move up to the

:21:32. > :21:33.Premier League this season. If the club are to get promoted, Chris

:21:34. > :21:38.Cohen will have to reluctantly watch from the sidelines.

:21:39. > :21:41.It is horrible because therd is nothing we can do to contribute The

:21:42. > :21:45.injured players try and keep everyone is happy as possible but at

:21:46. > :21:49.the end of the day it comes down to the 14 players on Saturday. We are

:21:50. > :21:55.certain there is a great run around the corner. It could be perfect for

:21:56. > :21:57.end of the season. They are expecting a good crowd

:21:58. > :22:02.tomorrow and desperate to sdnd them home with a win.

:22:36. > :22:38.Still two events to go in Montreal. In Rugby, confirmation that

:22:39. > :22:42.Leicester fly`half Toby Flood will join Toulouse next season.

:22:43. > :22:46.Flood's departure for Francd had been long anticipated. Todax it has

:22:47. > :22:49.been annouced he has signed a three`year deal. Flood's decision to
