:01:05. > :01:07.spent a lot of time caring for others, often to his own detriment.
:01:08. > :01:19.After 18 months he is still very much missed by us all. It h`s been
:01:20. > :01:23.hard for Katie hearing how badly her dad was let down in the two days
:01:24. > :01:27.before his death. A mix`up hn the ambulance control room meant it took
:01:28. > :01:31.six hours to get him into hospital and he did not get the fluids he
:01:32. > :01:38.needed until 14 hours after the call. He died two days later from
:01:39. > :01:41.kidney failure and other problems. The coroner outlined a serids of
:01:42. > :01:54.mistakes, errors and missed opportunities. Are She said there
:01:55. > :01:57.were serious, repeated and systemic failures as he deteriorated and
:01:58. > :02:13.those caring for him were oblivious to that. The inquest heard lany
:02:14. > :02:16.measures and improvements are in place on the ward to stop it
:02:17. > :02:19.happening again and from thd Ambulance Service there camd an
:02:20. > :02:22.apology to the family and reassurances that they have made
:02:23. > :02:26.crucial changes. All the falily of Stewart Maltby hope for now is that
:02:27. > :02:42.no`one else will have to suffer as they have. Are to trust his
:02:43. > :02:45.statement tonight. "We extend our condolences and reiterate otr
:02:46. > :02:48.unreserved apologies to Mr Laltby's family for the failings in our care

:03:07. > :03:08.of errors ` what is the famhly's feeling tonight? Some of those were
:03:09. > :03:10.very basic, failure to keep records and then over whelming feelhng that
:03:11. > :03:14.there monotonous staff on dtty. Family hope that all those `re
:03:15. > :03:19.rectified at the same time. What has been the reaction of the falily
:03:20. > :03:23.after today? They were very pleased that the extent of the enquhry by
:03:24. > :03:30.the coroner and her findings. They are relieved that that parthcular
:03:31. > :03:35.aspect of their ordeal was over As you mentioned during the case is not
:03:36. > :03:47.the first incident of its khnd. There is a record of similar
:03:48. > :03:54.feelings on that particular ward and it is hoped that improvements will
:03:55. > :04:02.go through not just that ward by the hospital and other hospitals that
:04:03. > :04:08.have those problems. What are the family going to do now? Thex will
:04:09. > :04:12.take time to consider their position but but they are coming back to see
:04:13. > :04:15.me and they will consider a legal claim against the hospital who have
:04:16. > :04:24.virtually admitted responsibility and that is encouraging. In other
:04:25. > :04:27.news tonight, a teenager who's accused of murdering the football
:04:28. > :04:31.coach Antoin Akpom has told a court that he ran for his life after being
:04:32. > :04:36.confronted by him on a Leicdster street. 19`year`old Abdul H`kim
:04:37. > :04:39.described how Mr Akpom swung a dumbbell at his head. The tdenager
:04:40. > :04:53.said he feared he'd be seriously injured. Here's our chief ndws

:06:03. > :06:06.rid of it. When he heard th`t four members of one family had bden
:06:07. > :06:10.killed in a fire attack two doors from his mother's house he said that
:06:11. > :06:13.he thought at first it was his house and they had picked the wrong
:06:14. > :06:18.placefor a revenge attack. That is when he decided to go to thd police.
