:00:00. > 3:59:59League final. That's all from us. A first look at

:00:00. > :00:29.the papers on Are And now the news for thd East

:00:30. > :00:33.Midlands, I'm Anne Davies. The family of a man who died after

:00:34. > :00:36.serious mistakes were made while he was in hospital say he'd sthll be

:00:37. > :00:43.alive if lessons had been ldarnt from other deaths on the sale ward.

:00:44. > :00:47.An inquest into the death of Stewart Maltby heard evidence that the lives

:00:48. > :00:50.of two other patients on ward D 7 at the Queen's Medical Centre were lost

:00:51. > :00:53.in a similar way. Are are In a moment I'll be speaking to the

:00:54. > :00:56.Maltby family's solicitor btt first, as Jo Healey reports, they're now

:00:57. > :01:00.considering legal action ag`inst the hospital. Are are Stewart w`s a

:01:01. > :01:04.character who made us laugh with his mischievous sense of humour. He

:01:05. > :01:07.spent a lot of time caring for others, often to his own detriment.

:01:08. > :01:19.After 18 months he is still very much missed by us all. It h`s been

:01:20. > :01:23.hard for Katie hearing how badly her dad was let down in the two days

:01:24. > :01:27.before his death. A mix`up hn the ambulance control room meant it took

:01:28. > :01:31.six hours to get him into hospital and he did not get the fluids he

:01:32. > :01:38.needed until 14 hours after the call. He died two days later from

:01:39. > :01:41.kidney failure and other problems. The coroner outlined a serids of

:01:42. > :01:54.mistakes, errors and missed opportunities. Are She said there

:01:55. > :01:57.were serious, repeated and systemic failures as he deteriorated and

:01:58. > :02:13.those caring for him were oblivious to that. The inquest heard lany

:02:14. > :02:16.measures and improvements are in place on the ward to stop it

:02:17. > :02:19.happening again and from thd Ambulance Service there camd an

:02:20. > :02:22.apology to the family and reassurances that they have made

:02:23. > :02:26.crucial changes. All the falily of Stewart Maltby hope for now is that

:02:27. > :02:42.no`one else will have to suffer as they have. Are to trust his

:02:43. > :02:45.statement tonight. "We extend our condolences and reiterate otr

:02:46. > :02:48.unreserved apologies to Mr Laltby's family for the failings in our care

:02:49. > :02:52.and for letting them and thdir father down so badly. We have learnt

:02:53. > :02:55.from this sad and tragic case and made changes to improve safdty and

:02:56. > :02:59.outcomes for our future pathents." With me now is Mr Maltby's family

:03:00. > :03:02.solicitor, Paul Halen. The hospital is quite specific about the change

:03:03. > :03:06.made and they've apologised ` is that now enough It was a catalogue

:03:07. > :03:08.of errors ` what is the famhly's feeling tonight? Some of those were

:03:09. > :03:10.very basic, failure to keep records and then over whelming feelhng that

:03:11. > :03:14.there monotonous staff on dtty. Family hope that all those `re

:03:15. > :03:19.rectified at the same time. What has been the reaction of the falily

:03:20. > :03:23.after today? They were very pleased that the extent of the enquhry by

:03:24. > :03:30.the coroner and her findings. They are relieved that that parthcular

:03:31. > :03:35.aspect of their ordeal was over As you mentioned during the case is not

:03:36. > :03:47.the first incident of its khnd. There is a record of similar

:03:48. > :03:54.feelings on that particular ward and it is hoped that improvements will

:03:55. > :04:02.go through not just that ward by the hospital and other hospitals that

:04:03. > :04:08.have those problems. What are the family going to do now? Thex will

:04:09. > :04:12.take time to consider their position but but they are coming back to see

:04:13. > :04:15.me and they will consider a legal claim against the hospital who have

:04:16. > :04:24.virtually admitted responsibility and that is encouraging. In other

:04:25. > :04:27.news tonight, a teenager who's accused of murdering the football

:04:28. > :04:31.coach Antoin Akpom has told a court that he ran for his life after being

:04:32. > :04:36.confronted by him on a Leicdster street. 19`year`old Abdul H`kim

:04:37. > :04:39.described how Mr Akpom swung a dumbbell at his head. The tdenager

:04:40. > :04:53.said he feared he'd be seriously injured. Here's our chief ndws

:04:54. > :04:56.reporter Quentin Rayner. Thd jury was told that Abdul Hakim and

:04:57. > :04:59.Hussein Hussein shared a cell at a young offenders institution. Abdul

:05:00. > :05:02.Hakim has three convictions for carrying knives and sharp

:05:03. > :05:04.implements. He said that he had never had any trouble with @ntoin

:05:05. > :05:08.Akpom before. He described `n incident on the 12th of September

:05:09. > :05:11.last year. He said that the football coach pulled up in a car and very

:05:12. > :05:15.aggressively challenged thel by shouting at him a question `s to why

:05:16. > :05:19.he was in his area. Moments later he ran back with men from a local gym.

:05:20. > :05:23.Abdul Hakim said some of thd men had covered their faces and Antoin Akpom

:05:24. > :05:39.had swung a dumbbell at his head. He was scared for his life. He later

:05:40. > :05:43.noticed that Hussein Hussein had blood on his jacket and he told him

:05:44. > :05:46.that he had stabbed Antoin @kpom because he feared he was behng

:05:47. > :06:02.attacked. Abdul Hakim denied that the knife was his and that he got

:06:03. > :06:06.rid of it. When he heard th`t four members of one family had bden

:06:07. > :06:10.killed in a fire attack two doors from his mother's house he said that

:06:11. > :06:13.he thought at first it was his house and they had picked the wrong

:06:14. > :06:18.placefor a revenge attack. That is when he decided to go to thd police.

:06:19. > :06:25.He denied that he was part of a team who were involved in murderhng

:06:26. > :06:28.Antoin Akpom. Football, and it was quite a party at the King Power

:06:29. > :06:32.Stadium as newly promoted Ldicester lost 4`1 to Brighton. Elsewhere

:06:33. > :06:51.Derby County beat Blackpool 3`1 and Nottingham Forest drew 3`3 with

:06:52. > :06:53.Sheffield. That's your news. So it's goodbye from me ` but with your

:06:54. > :07:02.weather now, here's Kaye. The breeze will ease down tomorrow

:07:03. > :07:07.and it should feel a little warmer as well. Temperatures falling quite

:07:08. > :07:10.sharp in now and we will sed some cloud increasing from the north and

:07:11. > :07:15.west as we go through the e`rly hours of the morning so that should

:07:16. > :07:26.stop the Wear close to a gr`ss frost. Clyde will increase to the

:07:27. > :07:30.diesel cloudier skies into the afternoon but it is staying dry and

:07:31. > :07:31.the breeze will be lighter with highs of around 30 degrees 04

:07:32. > :07:36.degrees. dry. Maybe a bit of light rain on

:07:37. > :07:41.Friday. A quiet spell for us. Not so on the other side of the world. As

:07:42. > :07:45.you have been hearing, there is not a great deal happening in the

:07:46. > :07:49.weather over the UK over the next couple of days.

:07:50. > :07:52.Meanwhile, we have been attacked thing -- tracking an active spring

:07:53. > :07:59.storm in the US. There have been reports of flash flooding and

:08:00. > :08:04.tornadoes. You can see a narrowband which is weakening and heading out

:08:05. > :08:07.to the Atlantic. It is the Atlantic tonight that will pick up whether

:08:08. > :08:10.funds to the north of the UK, bringing with them or cloud and

:08:11. > :08:15.outbreaks of rain. Meanwhile, pressure remains relatively high to

:08:16. > :08:19.the south. There is a small chance of patchy mist and fog and low cloud

:08:20. > :08:27.by the end of the night. But temperatures will fall away again. A

:08:28. > :08:34.chilly start to Wednesday morning. There will be a bit of a breeze

:08:35. > :08:41.blowing in the north. But it will be a fairly cloudy and wet affair first

:08:42. > :08:45.thing in the morning here. The rain will be sticking to the north-west

:08:46. > :08:46.of Scotland. Further south after
