:00:28. > :00:33.electrocuted himself while in Chelmsford prison.
:00:34. > :00:36.A two-week inquest heard he'd been let down by serious failings
:00:37. > :00:45.Dean Saunders, a loving father and much loved family man.
:00:46. > :00:49.But in December 2015, with no previous
:00:50. > :00:52.history of mental health issues, out of the blue he suffered a paranoid
:00:53. > :00:54.episode which resulted in him grabbing a knife and stabbing
:00:55. > :01:00.He was arrested for attempted murder and taken to
:01:01. > :01:07.But after a few days of being on constant
:01:08. > :01:11.observation a decision by staff was taken to reduce that monitoring.
:01:12. > :01:14.They feel that 30 minutes is sufficient and I said, "I'm telling
:01:15. > :01:18.you now, if you do not put my son back on constant watch he will
:01:19. > :01:23.You won't be able to say you didn't know, you hadn't been
:01:24. > :01:25.told, you wasn't aware, because you know.
:01:26. > :01:31.And if he kills himself it will be your fault."
:01:32. > :01:34.Failed attempts to find a place in a secure unit
:01:35. > :01:39.On January 5th he committed suicide, electrocuting himself using
:01:40. > :01:51.Today, an inquest concluded that Dean's death was
:01:52. > :01:55.contributed to by neglect and multiple failings, a key fact that
:01:56. > :01:58.his family learned was that none of the people who decided
:01:59. > :01:59.to reduce his monitoring were medically trained.
:02:00. > :02:03.I don't want another Dean or another family
:02:04. > :02:10.I'm just so grateful that someone's finally listened to us.
:02:11. > :02:13.Much too late in the day but they finally listened.
:02:14. > :02:16.Words can't describe how much we miss him, how much of a
:02:17. > :02:25.The blessing is we've got a little bit of him in his son who
:02:26. > :02:29.will grow up knowing that his dad was a proud dad.
:02:30. > :02:35.And he will, in turn, know the injustice against him.
:02:36. > :02:38.After the verdict was returned, Care UK, the private
:02:39. > :02:42.company that provided health care in Chelmsford Prison, announced that
:02:43. > :02:50.they were to end their present contract.
:02:51. > :02:52.they were to end their prison contract.
:02:53. > :02:55.In a statement they said that the level of resources the
:02:56. > :02:57.prison planned to make available is insufficient to meet
:02:58. > :03:03.For the family they're urging the coroner to issue a report
:03:04. > :03:05.highlighting the failings so a similar tragedy can never
:03:06. > :03:14.Train conductors on London Midland are to vote over strike action.

:05:16. > :05:18.why they're not investing some of that money in kit that could
:05:19. > :05:20.pre-empt what's happening beneath our feet.
:05:21. > :05:25.Last year, Thames Water made ?750 million profit.
:05:26. > :05:29.I think it's not beyond the wit of man or
:05:30. > :05:32.woman to spend some of that money on getting that
:05:33. > :05:33.kit into those pipes so
:05:34. > :05:36.we can detect the leaks as early as possible.
:05:37. > :05:40.A lot of leaks reported, a lot of them are on very small pipes
:05:41. > :05:43.The leaks on the big pipes happen very rarely but
:05:44. > :05:47.when they do they can be devastating.
:05:48. > :05:49.In each of those cases, of course we wish we'd reacted much
:05:50. > :05:54.Before we close the road we need to plan the repair work carefully.
:05:55. > :05:57.But even before that we've got to find out which pipe is
:05:58. > :06:06.actually causing the leak and that does take time.
:06:07. > :06:10.We can't just go in and dig the street up willy-nilly
:06:11. > :06:14.Thames Water is reviewing how they identify potential leaks
:06:15. > :06:17.They're due to report back to the London
