:00:54. > :00:55.First tonight, the so called superhead Rachel de

:00:56. > :00:59.Souza did get an early warnhng of one Ofsted inspection

:01:00. > :01:03.but she has been cleared of similar allegations at two other schools.

:01:04. > :01:06.Schools should only have half a day's notice of an inspection

:01:07. > :01:09.But it had been claimed that three schools linked to Dame Rachdl were

:01:10. > :01:13.tipped off much earlier givhng them an unfair advantage.

:01:14. > :01:16.The schools were Ormiston Victory Academy at Costessex.

:01:17. > :01:22.Ofsted has been investigating the claims and this afternoon gave

:01:23. > :01:30.No evidence of early warning at Ormiston Victory or

:01:31. > :01:34.But Dame Rachel did know about an inspection at Yarmouth bdcause of

:01:35. > :01:37.in the words of the report, a lapse in information sharing procddures.

:01:38. > :01:51.She is a high profile super head, made a Dame in the New Year 's

:01:52. > :01:54.Honours list. At the last fdw weeks, she has been at the centre of

:01:55. > :01:58.allegations that she was tipped off about the date of Ofsted

:01:59. > :02:07.inspections. She was the exdcutive principal at the school. Last year,

:02:08. > :02:10.she moved on to become chief executive of the Inspiration Trust.

:02:11. > :02:17.She runs the two other schools that were under investigation. This

:02:18. > :02:21.afternoon, Ofsted found there was no evidence that three academids had

:02:22. > :02:27.improper notice but it did find that Rachel De Souza, who was also a

:02:28. > :02:31.part`time Ofsted inspector, was mistakenly included among the

:02:32. > :02:34.recipients of an e`mail sent to a group of new inspectors. Its report

:02:35. > :02:38.says the e`mail gave her advance notice of the inspection at grainier

:02:39. > :02:42.`` great Yarmouth primary Academy, where she is the chair of governors

:02:43. > :02:47.and the Chief Executive of the sponsor. Ofsted said as soon as it

:02:48. > :02:51.found out, it changed the d`te of the inspection and returned five

:02:52. > :02:57.weeks later. It also came to the school for an unannounced inspection

:02:58. > :03:02.this week. But Ormiston is not being we inspected. Allegations from this

:03:03. > :03:06.parent that it got advanced notice were not substantiated. Ofsted said

:03:07. > :03:11.the school was in a heightened state of readiness and had predicted the

:03:12. > :03:14.time frame for inspection. H have to believe them but there is still a

:03:15. > :03:20.suspicion that somebody knew something a little bit more specific

:03:21. > :03:25.than that. That was as much as I said to Robin. He has looked around

:03:26. > :03:31.and has not found anything tntoward. There is still that suspicion in my

:03:32. > :03:36.mind. The review found no f`ult of the Ormiston Academy's trust, while

:03:37. > :03:47.Rachel D'Souza has not responded to requests for interviews.

:03:48. > :03:50.And we did ask Ofsted to cole on the programme.

:03:51. > :03:53.A woman who died from cancer has given thous`nds

:03:54. > :03:55.of pounds to the hospital which failed to diagnose her illndss.

:03:56. > :04:00.She was repeatedly turned away by doctors in the Accident

:04:01. > :04:04.and Emergency department at Southend Hospital.

:04:05. > :04:07.She recently won compensation but made the donation to help prevent

:04:08. > :04:22.Gemma's family has been inundated with cards and flowers. Her long

:04:23. > :04:24.battle against cancer and it suddenly eight days ago when she

:04:25. > :04:31.suffered a ruptured artery. `` Jemma. Her mother had been speaking

:04:32. > :04:34.on the phone with her. You have a lovely phone call and chattdd for

:04:35. > :04:39.about half an hour about her birthday, but Christmas, thd things

:04:40. > :04:46.she wanted to do in the week. Within half an hour or 40 minutes, she was

:04:47. > :04:51.gone. I cannot read the cards because it's too upsetting. In 010,

:04:52. > :04:53.she was repeatedly turned away by doctors at the accident and

:04:54. > :04:58.emergency department of Southend Hospital. She was told nothhng was

:04:59. > :05:02.wrong, even though she was bleeding heavily. Later, she was diagnosed

:05:03. > :05:08.with cervical cancer. Earlidr this year, when she knew she was dying,

:05:09. > :05:14.she was interviewed by Look East. You are determined. I will fight as

:05:15. > :05:17.long as I can. I will never give in. Had they diagnosed you sooner,

:05:18. > :05:24.you wouldn't be in this poshtion. No, I wouldn't. That is somdthing I

:05:25. > :05:30.try to come to terms with every day. Some days it hit me hard. In the

:05:31. > :05:34.last few weeks, she was awarded more than ?300,000 compensation `nd she

:05:35. > :05:38.is giving thousands of that fact of those who helped and treated her,

:05:39. > :05:43.including Southend Hospital. Beneficiaries include the

:05:44. > :05:53.counselling service, ecologhcal support service and other therapy.

:05:54. > :05:58.In a statement, the chief ntrse at Southend Hospital offered hdr

:05:59. > :06:02.condolences to her family and described her as an extremely

:06:03. > :06:07.courageous woman. The statelent praised her generosity and said the

:06:08. > :06:11.hospital was extremely gratdful for her donations. The family s`y they

:06:12. > :06:14.are proud she is being so gdnerous to the hospital that failed her

:06:15. > :06:16.We saw Jemma's mother and stepfather in that report.

:06:17. > :06:19.I spoke to them late this afternoon, and asked how they felt

:06:20. > :06:25.about her decision to give this money to the hospital.

:06:26. > :06:32.I am very proud that she gave the money. I think it was a verx

:06:33. > :06:38.charitable thing for her to do, when the department had failed hdr on so

:06:39. > :06:44.many instances. And what is the money actually going towards the

:06:45. > :06:48.hospital? She has already donated to the counselling services because

:06:49. > :06:55.they helped her immensely. @lso to the cancer charity. She was also in

:06:56. > :07:02.the process of donating to the therapy appeal, which is her

:07:03. > :07:08.oncologist's appeal. We will carry that on. Gary, it's an awful loss

:07:09. > :07:12.for both of you but does it give you any comfort that she will h`ve made

:07:13. > :07:15.such a difference in this w`y? It does because when she got the

:07:16. > :07:20.money, she didn't know what to do with it. We said, you can hdlp other

:07:21. > :07:27.people. I'm just proud that she will give it to the hospital that failed

:07:28. > :07:34.her. I don't think I could do it. We are going to give it to oncology,

:07:35. > :07:40.rather than the General Hospital. She wanted it to go to oncology to

:07:41. > :07:45.help other people there. It's a comfort to be able to do th`t. It

:07:46. > :07:51.would have been nice if she could have seen it go herself. Erhca, a

:07:52. > :07:54.terribly difficult time for you but what are your thoughts towards the

:07:55. > :08:00.hospital after all that you've been through? It's very hard for me

:08:01. > :08:08.because I actually work at the hospital. I am appalled that the

:08:09. > :08:14.treatment we had and I am jtst blessed that we were so lucky to get

:08:15. > :08:18.the treatment that she got `t oncology and radiotherapy bdcause

:08:19. > :08:24.they are absolutely outstanding especially the doctor who w`s hurt

:08:25. > :08:28.oncology doctor. She found ht very difficult to trust doctors `fter all

:08:29. > :08:37.of the instances. When she finally got her doctor, she felt trtly lucky

:08:38. > :08:41.to have him. Gary, we heard Jemma and she was such a fighter. She was

:08:42. > :08:49.because she could have given up three years ago. She kept going and

:08:50. > :08:52.going. She was planning for Christmas and we always knew it

:08:53. > :08:58.could come an end. It was r`ther sudden at the end. She wouldn't let

:08:59. > :09:03.it get her down. She got tired at two speaking for an hour. She would

:09:04. > :09:07.go on, have her friends arotnd as if nothing had happened but it would be

:09:08. > :09:14.obvious to us that she was tired. She was a fighter, yes, verx much

:09:15. > :09:17.so. She talked to us about how an earlier diagnosis might havd made

:09:18. > :09:22.all the difference. Can you cope with the fact that it might have

:09:23. > :09:26.done? It's very hard becausd nobody can say for definite but her first

:09:27. > :09:30.diagnosis, they did say, if we'd have got this six months ago, like

:09:31. > :09:35.all cancers, the earlier yot get them, the more chance you'vd got of

:09:36. > :09:38.survival. You cannot say shd definitely would have survived but

:09:39. > :09:42.the odds would have been in her favour because they've got far

:09:43. > :09:47.better treatments now. By the time they had founded, it was too late

:09:48. > :09:51.the chilled the first one btt this was a secondary cancer, which can

:09:52. > :09:59.always happen when the earlx one is not found. It was very hard because

:10:00. > :10:02.when I did question the varhous doctors at accident and emergency, I

:10:03. > :10:08.was basically told that if H carried on, I would be barred from the

:10:09. > :10:14.Department and they wouldn't admit Jemma. I was in a strange shtuation.

:10:15. > :10:17.You want to kick off becausd that is your daughter lying there, bleeding

:10:18. > :10:20.and in pain, but when you do go out approach them, that is what they

:10:21. > :10:24.tell you. They are not going to treat your daughter and you will be

:10:25. > :10:28.barred from the accident and emergency department. We hope that

:10:29. > :10:32.something is set up. The people are not specialists but we hope that

:10:33. > :10:37.they do, in future, get told that if someone comes in like this,

:10:38. > :10:40.referring to a specialist and the specialists have to realise that not

:10:41. > :10:44.everyone is going to be a c`ncer case but it's better to be safe than

:10:45. > :10:50.sorry. We hope in future thdy refer people. Rather that than go away and

:10:51. > :10:57.see your GP. That is what Jdmma wanted. She wanted to know that

:10:58. > :11:01.people would be helped in the future. She knew it was too late to

:11:02. > :11:06.help her but she wanted to help other people. Thank you verx much

:11:34. > :11:37.information but does relate to offending between the 1970s and
