:00:19. > :00:37.on their five week old daughter. Rocky Uzzell and Katherine Prigmore

:00:38. > :00:43.were seen to laugh at the hospital where by the week old is a

:00:44. > :00:45.bull's-eye in a critical condition. Katherine Prigmore filmed her

:00:46. > :00:51.partner violently shaking and squeezing their daughter. This case

:00:52. > :00:54.highlights major failings of Kettering Hospital. Baby Isabelle

:00:55. > :00:58.was admitted here 6 times in 5 weeks yet not once did staff even consider

:00:59. > :01:03.that her parents could be to blame for her injuries. They mist to rib

:01:04. > :01:08.fractures. Had they recognised them they could have taken steps to have

:01:09. > :01:15.her removed from her parents to a more serious injury. The moral

:01:16. > :01:19.series warning signs were missed time and time again. On March 16,

:01:20. > :01:37.2014, Isabelle was admitted with leg pain.

:01:38. > :01:43.The next day, the parents were arrested. What is of particular

:01:44. > :01:47.importance is the repeat presentation to hospital over a very

:01:48. > :01:51.short period of time, that should bring an immediate child protection

:01:52. > :01:55.flag and top diction concern that would need to be checked. It appears

:01:56. > :02:02.that didn't happen. The couple lived in this maisonette. Shocked

:02:03. > :02:06.neighbours say they really saw them. Couldn't believe it. I knew

:02:07. > :02:10.something wasn't right because I only saw the mad with a pushchair

:02:11. > :02:14.once, nobody ever saw the baby. Isabel will soon turn 3. Unable to

:02:15. > :02:19.walk, talk, C and swallow, she is with foster parents and isn't

:02:20. > :02:24.expected to live until her teens. Kettering Hospital said there was

:02:25. > :02:27.profound regret over the missed opportunities to safeguard her and

:02:28. > :02:29.said it has introduced improvements to procedure to protect infants at

:02:30. > :02:31.risk. Well the head of the

:02:32. > :02:33.Northamptonshire Safeguarding Children Board said the case of baby

:02:34. > :02:35.Isabelle was one of I asked Keith Makin how

:02:36. > :02:46.it was allowed to happen. Primarily, 2 things. There was not

:02:47. > :02:50.sufficient information sharing between agencies, although I have to

:02:51. > :02:54.stress that itself wouldn't have necessarily lead to this awful

:02:55. > :03:01.outcome. More particularly, we are very concerned that the people at

:03:02. > :03:07.the accident and emergency acidity were not curious enough about

:03:08. > :03:12.whether the reasons for admission was safeguarding or not. They were

:03:13. > :03:16.not curious about whether abuse may have taken place. 1 thing that

:03:17. > :03:20.stands out on the report is that an x-ray was done on the child's chest

:03:21. > :03:27.and broken ribs were missed, how easy is it to miss broken ribs? I'm

:03:28. > :03:32.top of the medical experts at actually, it's quite hard to tell in

:03:33. > :03:36.small babies with a rib has been broken or not -- I am told by the

:03:37. > :03:42.medical experts. But the issue as I understand it from the report is

:03:43. > :03:48.that the x-rays were not examined by a paediatric radiologist. If the

:03:49. > :03:52.examination had taken place as it should have been, it is very likely

:03:53. > :03:56.the child would not have been released from hospital, very likely

:03:57. > :04:01.wouldn't have gone back to the parents and this awful outcome would

:04:02. > :04:06.very likely that of happened. What are you recommending to stop it

:04:07. > :04:09.happening again? There is a different arrangement now for

:04:10. > :04:16.examination of x-rays in babies and young children. And the sharing of

:04:17. > :04:22.information, particularly about fathers, has been reinforced so that

:04:23. > :04:27.is all backed by training programme for all relevant staff. We as a

:04:28. > :04:31.board are monitoring and ordering the impact of that. Are you 100%

:04:32. > :04:35.sure that if a baby was brought hospital today and it was being

:04:36. > :04:43.abused by its parents that it would be picked up and acted upon? Not

:04:44. > :04:48.ever 100% guarantees in safeguarding but what we are assured of is that

:04:49. > :04:52.the Rossettini I have thought about would be enacted, questions will be

:04:53. > :04:54.asked in safeguarding would be considered as a primary issue.

:04:55. > :04:55.The television entertainer Rory McGrath has been give

:04:56. > :04:58.a suspended ten week jail sentence today after admitting harassing

:04:59. > :05:04.The court in Huntingdon was told McGrath and the married woman had

:05:05. > :05:11.been having an affair. Mike Cartwright reports.

:05:12. > :05:17.Arriving at court, his wife by his side. Rory McGrath, who the court

:05:18. > :05:24.was told pursued a course of conduct that was unstable and tempestuous.

:05:25. > :05:30.After harassing a man married mother in her 40s for more than a year. She

:05:31. > :05:34.can't be identified, the 2 having an affair for 5 years. The woman

:05:35. > :05:39.wanting to finish it. After she said he had become difficult and

:05:40. > :05:43.unpleasant company. They had shared intimate messages and texts. Rory

:05:44. > :05:47.McGrath threatening to publish glossy images of them. For months he

:05:48. > :05:52.sent her texts, e-mails, called and followed her. The judge was told of

:05:53. > :05:59.his deep remorse. Outside his wife next to him, a solicitor spoke on

:06:00. > :06:03.his behalf. This has been a dark time and thankfully it's now over. I

:06:04. > :06:08.wish to thank the judge and apologise to my wife and family, and

:06:09. > :06:16.to thank them for the incredible support during this time. I now want

:06:17. > :06:19.to move on with my life. In Cambridge Park, he tailed the woman

:06:20. > :06:23.as she went for a run with a teenage daughter. He conducted her husband

:06:24. > :06:29.and her new partner. Police say harassment cases on the rise.

:06:30. > :06:34.Encouraging victims to come forward. I would urge them to pick up the
