:01:03. > :01:07.The police and the fire service have launched a joint investigation.
:01:08. > :01:11.Her teachers say Emilia Kordaszewska was a delightful, friendly
:01:12. > :01:14.and positive student who was always smiling.
:01:15. > :01:17.Two other women were in the house at the time, but managed to escape
:01:18. > :01:23.The family is Polish and tonight, there will be a special mass
:01:24. > :01:39.This tragedy has been felt very deeply in the Polish community here
:01:40. > :01:50.in Braintree and 100 or so members have gathered here at the church
:01:51. > :01:54.where a mass has just started for Alina and Emilia, a mother and
:01:55. > :01:59.daughter who could not escape a fire that torso quickly through their
:02:00. > :02:03.home. Flowers and candles for a mother and daughter who died in a
:02:04. > :02:11.fire, placed by colleagues who worked alongside Alina Kordaszewska.
:02:12. > :02:20.She was such a lovely, bubbly person, always smiling. A shame, a
:02:21. > :02:28.real shame. I literally spoke with her a couple of days ago and I asked
:02:29. > :02:37.how was she doing for Christmas. She said she was still far away, as we
:02:38. > :02:42.mentioned, they have been taken far too soon, far too soon. The fire
:02:43. > :02:47.began around 10:30pm last night and neighbours told me the house went up
:02:48. > :02:54.very quickly. There is desperate to help beaten back. Just could see a
:02:55. > :03:01.glow in the back, oh my God, the house is on fire. I husband tried to
:03:02. > :03:08.help her. She was trying to break the window. It had literally come
:03:09. > :03:15.through, iPhones the Fire Brigade and about ten minutes after, they
:03:16. > :03:21.arrived. But it is very scary to watch. The roof was alight, it
:03:22. > :03:28.really went quickly and it kept stopping and starting up again. It
:03:29. > :03:33.is a tragedy. Very, very heartfelt sympathy goes out to them all.
:03:34. > :03:38.Living down this road, it is heartbreaking that something could
:03:39. > :03:45.happen so quickly. Within hours, a fundraising page had collected
:03:46. > :03:50.thousands. The timing of this fire so close to Christmas has heightened
:03:51. > :03:54.the sense of tragedy. Kept the house, there were prayers for Alina
:03:55. > :04:01.and the media as the investigation goes on. Emilia's score have issued
:04:02. > :04:09.a tribute to her. The headteacher has expressed an article surplus.
:04:10. > :04:13.This news is all the more distressing so close to Christmas.
:04:14. > :04:14.The Fire Service still have not said how big in this fire may have
:04:15. > :04:17.started. A mental health hospital in Norfolk,
:04:18. > :04:20.which has only been open a few months, has been placed in special
:04:21. > :04:22.measures by inspectors and could be forced to close
:04:23. > :04:25.if improvements aren't made. The Care Quality Commission found
:04:26. > :04:29.a number of "serious problems" at Mundesley Hospital,
:04:30. > :04:32.including "staff not up Beds at the private hospital
:04:33. > :04:37.are often taken up by NHS patients if no provision can
:04:38. > :04:42.be found elsewhere. Mundesley Hospital wants to become
:04:43. > :04:45.a renowned centre of excellence. Its Victorian building is said
:04:46. > :04:49.to be more like a hotel than a private hospital,
:04:50. > :04:51.but the CQC has rated It was inspected after
:04:52. > :04:57.Terence O'Shea from the Campaign to Save Mental Health Services
:04:58. > :05:02.reported their concerns to both the local mental
:05:03. > :05:05.health trust and the CQC. I think it is unusual that
:05:06. > :05:08.you can register a 27-bed psychiatric hospital,
:05:09. > :05:12.start admitting patients in February, only be inspected
:05:13. > :05:16.in September and have action taken During all of that time, people have
:05:17. > :05:22.been receiving what appears to be This hospital used to be home to
:05:23. > :05:29.a drug and alcohol treatment centre. It has since been taken over
:05:30. > :05:32.by Elijah Adeyemi and Lee Allison who suffered a breakdown
:05:33. > :05:35.16 years ago. He wanted to change the way mental
:05:36. > :05:39.health is viewed and treated, but the hospital has been criticised
:05:40. > :05:42.for staff not completing Strangulation risk assessments had
:05:43. > :05:48.also not been reviewed. Amongst the criticisms in this CQC
:05:49. > :05:52.report was that the hospital never reported any serious incident
:05:53. > :05:55.since it opened despite In one case, a detained
:05:56. > :06:00.patient left the hospital, was absent without leave,
:06:01. > :06:03.the police were involved, but the CQC wasn't informed
:06:04. > :06:09.and the staff did not log it fully. In the last six months,
:06:10. > :06:12.the local mental health trust has sent 171 patients here,
:06:13. > :06:15.allowing them to be treated The trust says it has been
:06:16. > :06:24.reassured they are safe. No-one was available
:06:25. > :06:28.from the hospital to talk to us today, but in a statement,
:06:29. > :06:30.it says they are undoubtedly disappointed to read the report,
:06:31. > :06:32.but with their ongoing developments, they are in a much different place
:06:33. > :06:36.now to what they were three months The CQC says it will carry out
:06:37. > :06:42.further inspections, including unannounced visits,
:06:43. > :06:45.to ensure the necessary The north Norfolk MP Norman Lamb
:06:46. > :06:55.is a former health minister who has campaigned for better care
:06:56. > :06:58.for mental health patients. Late this afternoon,
:06:59. > :07:15.he gave me his reaction These are serious failings and the
:07:16. > :07:19.CQC has acted decisively on them. I think it is important to note that
:07:20. > :07:26.the CQC found that the quality of care given by staff was good and
:07:27. > :07:30.that is important to know. It is a very interactive environment that
:07:31. > :07:34.people are in when they are there, but they have to address these
:07:35. > :07:39.failings. They are trying to do things differently. Do you applaud
:07:40. > :07:45.that? Yes, the traditional mental health ward is often absolutely not
:07:46. > :07:53.therapeutic in any and particularly when people are held for a long
:07:54. > :07:57.time, and so treating people in a nice environment which can be
:07:58. > :08:03.therapeutic I think is incredibly important. We have to be much better
:08:04. > :08:08.at keeping people out of hospital, much better preventive care,
:08:09. > :08:13.supporting people at home but if people do need hospital care, that
:08:14. > :08:19.sort of environment is a good one. Do you think the NHS should stop
:08:20. > :08:21.sending people that? There is a responsibility on the trust to
:08:22. > :08:27.ensure they are satisfied that patients will be safe, because that
:08:28. > :08:34.is one of the critical things, ligature points which increase the
:08:35. > :08:39.risk of suicide. You cannot get more serious than that. They have to be
:08:40. > :08:44.satisfied that patients are absolutely safe, received good care
:08:45. > :08:49.and stand a chance of recovery because that is ultimately what the
:08:50. > :08:53.system should be aimed at. Does it bother you that so much time goes by
:08:54. > :09:00.from a inspection to the time we hear about this? There is sometimes
:09:01. > :09:06.a delay. These inspections are quite substantial and quite robust. I have
:09:07. > :09:14.more faith in the Care Quality Commission now and the work that
:09:15. > :09:19.they do because -- we should be reassured the findings they reach
:09:20. > :09:23.are substantial. As far as you are concerned, this centre can keep
:09:24. > :09:33.going into the New Year but needs to be checked regularly? Monitored
