:01:15. > :01:18.Flight Lieutenant Sean Cunningham's parents have used to describe his
:01:19. > :01:21.death. Today saw the end of a three`week inquest into how the Red
:01:22. > :01:26.Arrow's pilot died when his ejection seat went off at RAF Scapmton in
:01:27. > :01:29.2011. Today the coroner said Sean Cunnigham's death was preventable,
:01:30. > :01:37.reserving particular criticism for the company which made the ejection
:01:38. > :01:40.seat and the Ministry of Defence. Caroline Bilton was at the inquest
:01:41. > :01:49.in Lincoln today. Caroline, how significant were the findings?
:01:50. > :01:54.Well, Peter, he said no stone would be left unturned at the start of
:01:55. > :02:00.this inquest. All parties in this case were scrutinised. Today, we
:02:01. > :02:03.learned that mistakes were made. There were failures on many
:02:04. > :02:09.different levels. It was a job he loved, but it
:02:10. > :02:11.ultimately cost him his life. Flight Lieutenant Sean Cunningham died
:02:12. > :02:17.after his ejector seat went off during routine checks on the ground
:02:18. > :02:21.at RAF Scampton in 2011. At the inquest into his death, it was
:02:22. > :02:26.revealed the accident could have been prevented. His family have been
:02:27. > :02:30.coming here every day for three weeks to get the answers there have
:02:31. > :02:35.been searching for for two years. That weight had taken its toll when
:02:36. > :02:49.it came to giving their reaction today. Sean was blameless. Excuse
:02:50. > :02:56.me. His tragic death was preventable. We still find it
:02:57. > :03:00.difficult to accept so many people could have missed what should have
:03:01. > :03:04.been obvious to those having a duty to ensure the safety of the seat.
:03:05. > :03:35.The inquest heard about concerns over pressure timetable.
:03:36. > :03:49.Had his parachute worked, he would still be alive.
:03:50. > :03:57.Other foreign forces were informed, but not the RAF. In relation to the
:03:58. > :03:59.19 missed opportunities to notice the handle was not unsafe, the
:04:00. > :04:25.coroner said: we have made, and will continue to
:04:26. > :04:30.make, every effort to make sure such a tragic accident never happens
:04:31. > :04:35.again. With regards to this nut and bolt and Martin Baker's failure to
:04:36. > :04:40.notify the MoD of potential risks of over signing, Stuart Fisher said
:04:41. > :04:49.there was a very serious failure of communication by Martin Baker.
:04:50. > :04:58.The Red Arrows training session was given today in Sean pot`macro on.
:04:59. > :05:03.Business has resumed here. The family hope lessons learned from the
:05:04. > :05:06.death of their son mean these men and women are safer in the country
:05:07. > :05:13.`` in the future. What changes have there been since
:05:14. > :05:16.Sean's death? Martin Baker did not respond in a statement today to the
:05:17. > :05:22.criticism of their failure to communicate. The coroner in tents to
:05:23. > :05:27.write to the company about that issue. They have redesigned and
:05:28. > :05:32.developed a new type of bolt and firing handle, and they will be
:05:33. > :05:36.fitted and operational by the end of the year. The coroner will also
:05:37. > :05:44.write to the RAF. He says he still has concerns about the ongoing
:05:45. > :05:48.safety of the handle. HSE is still investigating this case and there
:05:49. > :05:51.could be a prosecution. Earlier I spoke to Andrew Brookes, a former
:05:52. > :05:58.RAF pilot specialising in flight safety, and asked him what he makes
:05:59. > :06:02.of the coroner's conclusions today. It is a sorry tale. He has
:06:03. > :06:08.highlighted the errors and omissions, and pointed the finger,
:06:09. > :06:13.and at the end of the day it is a sad indictment. It was unavoidable
:06:14. > :06:18.accident, but at the end of the day, somebody had to be dragged. The
:06:19. > :06:24.ejection seat makers have been heavily criticised. They describe
:06:25. > :06:29.the ejection seat is entirely useless. The handle had been
:06:30. > :06:42.misaligned. The safety pin was not doing its job. Why was it
:06:43. > :06:47.misaligned? Sean had something to do with it, his ground crew had
:06:48. > :06:56.something to do with not seeing it there is a link in this chain. It is
:06:57. > :06:59.so sad and so avoidable. The parachute didn't apply because
:07:00. > :07:08.somebody did not tighten it properly. You are a former pilot.
:07:09. > :07:18.How is seriously are the checks taken? Very. He would have checked
:07:19. > :07:27.it. He did not notice the problem. The ground crews checked it. It is a
:07:28. > :07:46.sophisticated piece of equipment. Everybody checks it. Is it safer now
:07:47. > :07:52.being a pilot? You can take it from the ever will make sure this never
:07:53. > :07:57.happens again. Everything has been done to stop it from happening
:07:58. > :08:01.again. The only things I take from it is these things are so rare. This
:08:02. > :08:08.wonderful piece of life`saving equipment is still there for the air
