:02:59. > :03:00.and blue, not breathing. She needed to be on a ventilator,
:03:01. > :03:03.and without that she would not have Like Naomi, Erin and Elliott
:03:04. > :03:07.were also born at Pilgrim Hospital. And for their parents,
:03:08. > :03:09.having care nearby was vital. When these decided they were going
:03:10. > :03:12.to come into the world, And that 15 minutes to get from one
:03:13. > :03:16.side of town to the other was as much as I really
:03:17. > :03:19.wanted to cope with, never mind having to try and get
:03:20. > :03:22.in the car and travel all the way This isn't about us wanting to keep
:03:23. > :03:26.it at our convenience. This is about us wanting to keep it
:03:27. > :03:29.because it is our local hospital. And it's not just used
:03:30. > :03:32.by the people of Boston. It's used by mums in Spalding,
:03:33. > :03:35.in Skegness, maybe even in Louth, in all these little
:03:36. > :03:37.towns and villages. Last week county councillors voted
:03:38. > :03:39.against the health plans, which could also see
:03:40. > :03:40.Grantham's A closed. I do have real concerns

:06:31. > :06:33.Around 5000 turkeys have died or been culled
:06:34. > :06:38.Our rural affairs correspondent, Paul Murphy,
:06:39. > :06:49.What has been happening there today?
:06:50. > :06:54.The focus as you say over the weekend was the culling of the
:06:55. > :07:01.turkeys. Today it is all about the investigation into the source of the
:07:02. > :07:05.virus. Lots of theories about this including the theory that it is
:07:06. > :07:10.transmitted by wild birds on their migration route over the East Coast,
:07:11. > :07:13.but it is something scientists will want to investigate. Lots of
:07:14. > :07:18.concerns about the virus because of what it is doing in Europe,
:07:19. > :07:22.spreading quickly. Just over the weekend, 20,000 turkeys had to be
:07:23. > :07:22.culled in Germany. The watchword here in Lincolnshire is
:07:23. > :07:24.bio-security. Once it's in the country
:07:25. > :07:26.it is of great concern and what we all do is we all have
:07:27. > :07:30.to up our bio-security. We keep everybody we can
:07:31. > :07:32.away from our farms, we make sure we have dedicated
:07:33. > :07:34.wellingtons for every shed, we have buckets full
:07:35. > :07:37.of disinfectant there to make sure that we can't walk
:07:38. > :07:39.it into the sheds. So we all need to be very careful,
:07:40. > :07:55.and keep it out of all the So in terms of that bio-security
:07:56. > :08:02.therein out to exclusion bones around this farm. A three climate
:08:03. > :08:07.zone prohibiting all movement of poultry, pod poultry products and
:08:08. > :08:10.wastes and a wider surveillance zone where scientists can keep an eye on
:08:11. > :08:21.neighbouring poultry units. Everything is being done to try to
:08:22. > :08:24.stop the spread. And they are very reiterating the fact that though the
:08:25. > :08:25.virus is deadly to birds it does not pose a risk to human health.
:08:26. > :08:28.People living in East Yorkshire and Hull could be forced to wait
:08:29. > :08:31.for routine operations if they smoke or are overweight.
:08:32. > :08:35.Health bosses are looking at copying similar restrictions placed
:08:36. > :08:42.The idea is designed to cut costs and reduce pressure on services,
:08:43. > :08:46.but has been criticised by slimming groups and some doctors.
:08:47. > :08:51.Margaret has had a lifelong struggle with her weight,
:08:52. > :09:03.She's concerned that delaying operations for obese people
:09:04. > :09:14.If somebody is in a lot of pain and they
:09:15. > :09:18.are overweight - I was in a great deal of pain, I was in a wheelchair.
:09:19. > :09:21.If I would have had to wait six months to
:09:22. > :09:24.try to lose weight, I would not have been able to lose the weight,
:09:25. > :09:28.The plans would affect people who need a non-emergency operation -
:09:29. > :09:33.Those with a Body Mass Index of 35 and above would be
:09:34. > :09:40.In the meantime, their surgery would be put off by six months.
:09:41. > :09:45.Health managers say the plan would help the patients recover
:09:46. > :09:49.Money is also a factor, with the NHS in this area
:09:50. > :09:53.needing to save hundreds of millions of pounds.
:09:54. > :09:59.People in Beverley welcomed any action to tackle obesity.
:10:00. > :10:05.It costs a lot of money, health care, so why wouldn't you insist
:10:06. > :10:08.that they buy into the process and lose some weight has contribution? I
:10:09. > :10:15.don't thing is the right way to tackle it. I think they should sort
:10:16. > :10:19.of say go on an exercise regime just to lose a bit of weight and if they
:10:20. > :10:21.can't do it they should give them the operation anyway.
:10:22. > :10:24.Recently a similar idea in the Vale of York was criticised by some
:10:25. > :10:26.surgeons who said delays could may worsen health problems.
:10:27. > :10:28.The East Riding of Yorkshire Clinical Commissioning Group told us
:10:29. > :10:31.it's "looking at a number of measures to improve the health
:10:32. > :10:33.and wellbeing of patients and address the financial challenges
:10:34. > :10:37.A debate remains as to whether delaying treatment
:10:38. > :10:50.There has been much debate on this, this evening.
:10:51. > :10:53.Earlier I spoke to Ian Eardley from the Royal College of Surgeons.
:10:54. > :10:58.I asked him if delaying operations for six months is a good idea.
:10:59. > :11:03.I think people who are smokers, who are overweight,
:11:04. > :11:05.certainly should stop smoking or being overweight,
:11:06. > :11:08.but that shouldn't prevent them from seeing a surgeon
:11:09. > :11:10.who may be able to offer them an operation to
:11:11. > :11:14.But if that is a non-urgent operation, and we're talking about
:11:15. > :11:17.non-urgent, why shouldn't they cut down on smoking or eat less to make
:11:18. > :11:20.themselves more healthy and give themselves a better chance of a full
:11:21. > :11:24.Well, some of those people will be in pain, and unnecessary
:11:25. > :11:28.Some of them will have conditions that
:11:29. > :11:33.And the third group of people may not be
:11:34. > :11:36.able to lose weight because of lack of mobility, so...
:11:37. > :11:38.Are you saying that somebody who's in pain, who's
:11:39. > :11:41.got a body mass index of over 35 should get their treatment
:11:42. > :11:44.there and then and not have to wait six months?
:11:45. > :11:47.They should certainly be seen and assessed to determine
:11:48. > :11:51.whether the treatment would get rid of their pain, absolutely.
:11:52. > :11:53.If a smoker is being forced to give up
:11:54. > :11:59.before a non-urgent operation, is that fair enough or is it none of
:12:00. > :12:02.the business of the CCG if someone smokes?
:12:03. > :12:05.We would feel that they should be assessed on an individual basis.
:12:06. > :12:09.I'm sure we'd all agree that stopping smoking is a good thing to
:12:10. > :12:12.do, and the CCG rightfully has an interest in that.
:12:13. > :12:15.But to use it as a blanket ban to stop them seeing a
:12:16. > :12:19.This is being called a "health optimisation
:12:20. > :12:23.process", and the CCG says it will improve outcomes for patients
:12:24. > :12:27.Do you see this as about health care or do you
:12:28. > :12:31.see it about saving money for a few months?
:12:32. > :12:33.I think the worry is that this is rationing.
:12:34. > :12:35.There are other CCGs round the country that
:12:36. > :12:37.have done similar things, and we were always
:12:38. > :12:41.approval from NHS England that this would become an imprint for other
:12:42. > :12:47.We are worried that this is primarily about money
:12:48. > :12:48.and not about the health of patients.
:12:49. > :12:51.Maybe you are worrying more about the health of your
:12:52. > :12:56.surgeons rather than getting the people healthy.
:12:57. > :13:00.We're worried about the pain and suffering of patients who have
:13:01. > :13:03.problems that would benefit from surgical care.
:13:04. > :13:06.So is this a mistake by the CCG, just finally?
:13:07. > :13:09.We would feel this is a mistake, absolutely.
:13:10. > :13:19.We would like to know what you think about this.
:13:20. > :13:22.Do you think obese people or smokers should be denied treatment?
:13:23. > :13:25.Are you trying to lose weight or stop smoking?
:13:26. > :13:28.Or do you think people who don't look after their bodies should be
