:01:39. > :01:41.safety and have been put back An inspection by the Care Quality
:01:42. > :01:44.Commission highlighted "significant concerns" over urgent and emergency
:01:45. > :01:46.care, outpatients and Now a team of experts will help run
:01:47. > :02:03.the trust until it improves. Bosses have apologised
:02:04. > :02:05.for the current situation and say Our health correspondent
:02:06. > :02:08.Vicky Johnson has this report. Computer hackers, missing patient
:02:09. > :02:10.referrals and forecasts Until today these were the main
:02:11. > :02:14.issues facing managers at Grimsby, Now, though, the Northern
:02:15. > :02:17.Lincolnshire and Goole Trust has been placed into special measures
:02:18. > :02:19.after inspectors highlighted problems in urgent and emergency
:02:20. > :02:21.care, where patients were not Outpatients, where too many clinics
:02:22. > :02:27.were being cancelled... And maternity services,
:02:28. > :02:30.where there are concerns over What we want to see is trusts
:02:31. > :02:36.improving and making sure that people walking through the front
:02:37. > :02:38.doors of this organisation are receiving safe, effective
:02:39. > :02:44.and high quality care. So when that does not happen,
:02:45. > :02:47.we are always disappointed. The trust was one of 11 placed
:02:48. > :02:53.in special measures in July 2013 as part of Sir Bruce Keogh's review
:02:54. > :02:57.into high death rates. It came out of special
:02:58. > :02:59.measures nine months later, but when it was inspected again
:03:00. > :03:01.last autumn, it was Hospital managers admit
:03:02. > :03:09.they are disappointed. This is not the level of care
:03:10. > :03:14.we would like to provide. It is a report that is disappointing
:03:15. > :03:17.to the executive and the board but most importantly it will be
:03:18. > :03:20.disappointing to all of the staff in our hospitals who have worked
:03:21. > :03:23.so hard to raise standards over I think what we have got
:03:24. > :03:30.here is a real fatal state of leadership in the hospital,
:03:31. > :03:32.we now need some certainty about who is going to run
:03:33. > :03:35.the teams going forward, we need to know that the hospital
:03:36. > :03:37.has an action plan. And an action plan
:03:38. > :03:40.is what they have got. They are being given lots of help,
:03:41. > :03:43.including peer support from another health trust and direct hands-on
:03:44. > :03:48.advice from NHS experts. A quality improvement director
:03:49. > :03:52.has also been appointed to advise on the best way
:03:53. > :03:54.to run the organisation. And there is also a full
:03:55. > :03:57.check on its finances. Most of the patients we spoke
:03:58. > :04:01.to were happy with their treatment. I had my son in hospital for one
:04:02. > :04:05.week and I cannot say bad They are kind and
:04:06. > :04:14.everything like that. Hospital managers have said
:04:15. > :04:17.they are getting the urgent help they need to bring them out
:04:18. > :04:19.of special measures I spoke to the medical director
:04:20. > :04:23.of the trust, Robert Lawrence. He told me the current
:04:24. > :04:37.situation is not good enough. Well, I think on behalf of the
:04:38. > :04:43.executive team and the board of this trust, we would like to say sorry to
:04:44. > :04:47.the patients and our friends and relatives also in this area, who
:04:48. > :04:54.come to rely on our services when they are not well. This is not the
:04:55. > :05:02.position we would expect to be in, we aspire to a higher level and as I
:05:03. > :05:05.have said, we are sorry. Missing patient referrals, a huge financial
:05:06. > :05:11.deficit, now special measures, it is a bit of a disaster and very
:05:12. > :05:15.upsetting for staff, what is going wrong? I would not accept that this
:05:16. > :05:20.is a disaster, it is a challenge and the problem. In terms of missing
:05:21. > :05:23.patients, I can tell you some numbers of those patients that they
:05:24. > :05:27.did have their referrals, which did not find their way to our
:05:28. > :05:32.organisation, 19 referrals over a period of some nine months against a
:05:33. > :05:36.background of tens of thousands that are referred to as. I think most
:05:37. > :05:41.will accept that it is a very small percentage. In terms of financial
:05:42. > :05:44.special measures, yes, I agree that it's a challenge to others and in
:05:45. > :05:50.terms of this report, that was last November and I think we have already
:05:51. > :05:53.made great strides since then. Most patients we spoke to today were
:05:54. > :05:58.happy, do you accept that this must be down to bad management? Well, the
:05:59. > :06:03.fact that most patients are happy would indicate to us that we are
:06:04. > :06:08.doing a decent job. We did get rating as good for caring, it was
:06:09. > :06:12.particularly noticed by the CQC that we are respectful and compassionate
:06:13. > :06:16.and treat people with dignity and our organisation and that really is
:06:17. > :06:23.our greatest priority. It is a pity that in certain areas things have
:06:24. > :06:26.slipped. But the CQC want to see significant improvements to the
:06:27. > :06:29.quality and safety of patient care. Patients watching will be worried
:06:30. > :06:35.that the hospital is not safe, what can you tell them in one sentence to
:06:36. > :06:42.allay any fears? I have already said, we use it, the doctors use it,
:06:43. > :06:46.it is a good, safe place to be. But this is said every time. You were in
:06:47. > :06:49.special measures for mac years ago and you had helped them to get
:06:50. > :06:54.through that, you are back there again, why have you not sustain the
:06:55. > :06:57.improvements that you have made? We have concentrated on the areas that
:06:58. > :07:01.the CQC previously identified and done a great amount of work on
:07:02. > :07:05.improving that level of care. Finally, will you be at a special
:07:06. > :07:10.measures this time next year? It is very difficult for me to predict
:07:11. > :07:14.what will happen in terms of future CQC inspections. We are
:07:15. > :07:17.concentrating on progress in quality improvement across this organisation
:07:18. > :07:26.at the fast as possible pace. Thank you for talking to us tonight, Mr

:07:42. > :07:44.management and there have had to declare an internal critical
:07:45. > :07:48.incident at Lincoln's A This means that they were so short of
:07:49. > :07:53.staff they were in danger of not being able to deliver safe services.
:07:54. > :07:56.Doctor Richard Andrews who is the associate medical director of the
:07:57. > :08:01.trust is blaming recent changes to tax rules which makes it much less
:08:02. > :08:06.appealing and much less lucrative for agency doctors to come and work
:08:07. > :08:11.there. Lincolnshire are significantly dependent upon agency
:08:12. > :08:16.doctors to fill positions within A and these changes to the taxation
:08:17. > :08:19.rules have affected this agency doctors which has in turn led to a
:08:20. > :08:23.lower uptake of shifts in the middle grade work in the A departments,
:08:24. > :08:29.leaving holes on our border. There is an acute shortage of agency
:08:30. > :08:32.doctors and Lincolnshire, this is not a new problem, we have
:08:33. > :08:36.continually reported about the overnight closure at Grantham's A
:08:37. > :08:40.So what they have been doing is getting doctors from neighbouring
:08:41. > :08:44.trusts, from Nottingham and Leicestershire to support them, and
:08:45. > :08:48.they have also been getting doctors from other departments within the
:08:49. > :08:52.hospital like paediatrics and orthopaedics to come and help them
:08:53. > :08:55.out in A to get them over the shortage of agency doctors. The
:08:56. > :09:01.advice again from them is that if you are not seriously ill or
:09:02. > :09:04.injured, please stay away as possible from A and seek medical
:09:05. > :09:07.advice from elsewhere. Vicki, thank you very much indeed.

:12:54. > :12:56.bits of wood off and everything. When it is finished and will look
:12:57. > :12:58.brilliant, I must admit. The footfall will begin to come back
:12:59. > :12:59.again. The work is almost complete,
:13:00. > :13:01.but one cafe owner says she won't see any of the benefit,
:13:02. > :13:11.despite enduring the disruption... They have finished the paving which
:13:12. > :13:18.is lovely in the square and it looks amazing. Sadly, it finishes here, so

:13:24. > :13:30.absolutely dreadful. As you can see, we've got a lovely new lamp post,

:13:33. > :13:43.As you can hear, it is fairly chaotic and it has been like that
:13:44. > :13:47.any city centre for the last two years. Many of our members have been
:13:48. > :13:52.really adversely affected by these works and what we want is to be
:13:53. > :13:56.fairly compensated. There is a mechanism for the City Council to
:13:57. > :13:56.refund some of the rates they have paid.
:13:57. > :13:59.The council says it will help with any damage caused
:14:00. > :14:01.by the contractor and that businesses have already had
:14:02. > :14:05.at least a 10% reduction in their business rates.
:14:06. > :14:14.Those businesses in the city centre have saved over ?630,000 in business

:14:19. > :14:23.future is very bright. So far this year we have seen over 800,000
:14:24. > :14:27.visitors to the city, now, that is a phenomenal increase and compared
:14:28. > :14:29.with two, three or four years ago. So, as the city centre
:14:30. > :14:32.is transformed, it's hoped it Campaigners in Boston say

:16:07. > :16:10.and that readily has approved a local self defence expert to ask him
:16:11. > :16:13.to get involved and to help people defend themselves. He has offered a
:16:14. > :16:19.six weeks martial arts training from the local group, that is free of
:16:20. > :16:23.charge for people in this area. Basically, what I want to do is give
:16:24. > :16:28.people the opportunity to be able to defend themselves. Well, the police
:16:29. > :16:33.have said that crime in Boston is actually down by 11% on last year
:16:34. > :16:36.and Lincolnshire County Council have said that they will not be changing
:16:37. > :16:40.the street lighting policy at this time, although they will monitor any
:16:41. > :16:46.effects that the change to the streetlights hats. The message from
:16:47. > :16:49.people here tonight is to speak to the local MP and say that they are
:16:50. > :16:52.not happy about this, they feel unsafe and hopefully they are hoping
