:01:01. > :01:03.leadership and safety means government experts are again

:01:04. > :01:05.going into Lincolnshire's hospitals Four years after inspectors

:01:06. > :01:12.first placed the trust responsible in special measures

:01:13. > :01:20.they are back in again. Bosses have apologised and said

:01:21. > :01:23.things are already improving. I'll be speaking to the man

:01:24. > :01:25.in charge shortly, but first our health correspondent

:01:26. > :01:27.Vicky Johnson is here. This is not a good report

:01:28. > :01:31.for hospital managers? This is another damning report

:01:32. > :01:34.whichever way you look at it - showing a catalogue of failings

:01:35. > :01:36.almost across the board Unsafe, poorly led by managers

:01:37. > :01:42.and 'inadequate' are the fundamental A failure to refer cancer patients

:01:43. > :01:48.to treatment on time. Thousands of outpatients

:01:49. > :01:52.appointments long overdue - And A services at both Lincoln

:01:53. > :02:00.and Boston rated as inadequate. When inspectors went on their rounds

:02:01. > :02:02.in Lincolnshire's hospitals last autumn they found much

:02:03. > :02:18.to criticise, including Some of the patient care that we saw

:02:19. > :02:23.wasn't of the required standard, so for example, we saw some patients

:02:24. > :02:27.that were not treated with compassion, we are dignity and

:02:28. > :02:29.respect was compromised. We also found some concerns in the emergency

:02:30. > :02:31.department. This is the latest in a series

:02:32. > :02:33.of poor inspections. Four years ago it was put

:02:34. > :02:36.into special measures Two years later it was taken

:02:37. > :02:39.out of special measures Last October, inspectors

:02:40. > :02:45.found more failings. have enough staff in Lincoln's A

:02:46. > :02:49.and declared a "critical incident" And now confirmation that experts

:02:50. > :03:05.have again been asked to step in. The sea QC report has made for

:03:06. > :03:08.difficult reading for many. In particular, patients not receiving

:03:09. > :03:12.medication on time when they are seriously ill as a big worry and

:03:13. > :03:17.they think it is something that has been going on for some time. As a

:03:18. > :03:22.doctor reading that report I was concerned by a number of aspects and

:03:23. > :03:26.fully understand why special measures are required. That has been

:03:27. > :03:29.the right decision but equally I would reassure the public that the

:03:30. > :03:34.vast majority are receiving very good care.

:03:35. > :03:36.The trust has always had problems recruiting enough staff.

:03:37. > :03:38.Last year it spent more than ?26 million on agency

:03:39. > :03:42.Managers fear their reputation as a failing trust could make

:03:43. > :03:53.To be absolutely honest, that is my biggest year. I would sincerely ask

:03:54. > :03:58.people not to be discouraged for this. They can come on secondment

:03:59. > :04:00.and temporary contracts, detest us out. I think they will find we are

:04:01. > :04:04.such a good team. The hospital teams certainly face

:04:05. > :04:07.a testing few months as they battle to raise standards and come out

:04:08. > :04:09.of special measures. A lot of the criticisms are broad -

:04:10. > :04:12.poor leadership, staff bullying Inspectors looked at how often

:04:13. > :04:17.people with bed sores But they found three patients who'd

:04:18. > :04:27.waited between three and almost eight and a half hours

:04:28. > :04:34.for their checks. The fourth patient had no record of

:04:35. > :04:36.being checked at all. Staff told inspectors

:04:37. > :04:38.they didn't have the time. But if there's no time

:04:39. > :04:40.for these basic tasks - then that's very telling

:04:41. > :04:51.about the organisation as a whole. We will be asking for your views on

:04:52. > :04:52.your experiences at hospitals in Lincolnshire in just a moment.

:04:53. > :04:55.Earlier, I spoke to the chief executive of the United

:04:56. > :05:01.I asked him whether he accepted the report wasn't good enough.

:05:02. > :05:03.We are very disappointed with this but of course,

:05:04. > :05:12.as ever in these reports, lots of examples of good practice.

:05:13. > :05:17.We recognise a number of scores, 12% are not adequate

:05:18. > :05:23.If half are rated good, half are rated not good,

:05:24. > :05:26.and if patients are not treated with dignity, low staff

:05:27. > :05:28.satisfaction, overall the trust is inadequate.

:05:29. > :05:30.Why should the people of Lincolnshire feel safe?

:05:31. > :05:34.Nothing in the report says we are not safe.

:05:35. > :05:40.What it identifies is a number of things that need to be put right.

:05:41. > :05:45.When the CQC came in October that while a number of actions

:05:46. > :05:49.we put immediately in place and we think if they came back today

:05:50. > :05:56.they would see many of those issues already resolved.

:05:57. > :05:58.Some of those things are long standing and one

:05:59. > :06:01.of the questions is recruitment and the the reconfiguration

:06:02. > :06:03.Those will take some time to resolve.

:06:04. > :06:06.The trust keeps blaming staff shortages but the inspectors found

:06:07. > :06:08.staffing was in line with the national average

:06:09. > :06:13.The reality is while the overall numbers may be comparable

:06:14. > :06:15.to larger hospitals, if you can imagine the ease

:06:16. > :06:18.with which Nottingham can have all staff in one place,

:06:19. > :06:20.we don't have that luxury in Lincolnshire, so those same

:06:21. > :06:23.numbers of staff have to be spread relatively thinly across our

:06:24. > :06:40.They also find hospitals are not properly equipped to deal with major

:06:41. > :06:42.incidents with equipment out of date and not working.

:06:43. > :06:54.Is that not basic Janet and John stuff you're not getting right?

:06:55. > :06:56.I think at the time there were simple wrrors not

:06:57. > :07:00.being addressed and it was more about administration.

:07:01. > :07:05.Just to reassure you and others listening we carry out exercises

:07:06. > :07:09.and we are rated highly so we are very ready for any major

:07:10. > :07:12.How long until you are out of special measures?

:07:13. > :07:16.The CQC came last October but it is not down to us.

:07:17. > :07:18.We look forward to demonstrating the improvements we have made.

:08:10. > :08:11.People living in the small village of Lutton in South Lincolnshire have

:08:12. > :08:14.been paying tribute to local school teacher and church

:08:15. > :08:19.She had been reported missing two weeks ago.

:08:20. > :08:22.It's believed her body was found along with that of her husband's

:08:23. > :08:25.The couple have not yet been formally identified.

:08:26. > :08:35.This small Lincolnshire village has been left in shock by the discovery

:08:36. > :08:37.of two bodies at this house down a quite country lane

:08:38. > :08:43.It's a pleasant village and it's just so quiet here,

:08:44. > :08:46.for something like this to happen to a family is just unbelievable.

:08:47. > :08:50.One of the bodies is believed to be that of teacher Bernice Williams

:08:51. > :08:56.who went missing more than two weeks ago.

:08:57. > :08:59.It is understood she lived here but the bodies of a man

:09:00. > :09:03.and woman were only found at the weekend.

:09:04. > :09:06.It is believed the other body is that of her husband, Lawrence.

:09:07. > :09:09.Linda Harding knew Bernice and her family from services here.

:09:10. > :09:15.Anybody who knew Bernice or knew the family will be very sad.

:09:16. > :09:23.She just was a very genuine, quiet, sweet lady.

:09:24. > :09:32.As a family, they came out as a family and supported one

:09:33. > :09:46.People I have spoken to here today have told me they hadn't seen Bernie

:09:47. > :09:48.or her husband in the village for some time.

:09:49. > :09:50.Police say they are not looking for anyone else in connection

:09:51. > :09:53.with the enquiry but tonight there are still more

:09:54. > :09:55.questions than answers as to what actually happened.

:09:56. > :10:08.When will police say more about what happened here?

:10:09. > :10:14.We expect formal identification of the bodies found here to take place

:10:15. > :10:20.later this week and then we should now a few more details about exactly

:10:21. > :10:23.what happened. Luckily, it is believed the bodies are that of

:10:24. > :10:29.Bernice Williams and her husband Laurence. All day, there has been a

:10:30. > :10:36.lot of police activity and that is continuing tonight as the police

:10:37. > :10:39.investigation continues. Thank you. More than 30 firefighters are

:10:40. > :10:42.dealing with a large woodland fire in Lincolnshire.

:10:43. > :10:46.They've been called to Laughton Woods.

:10:47. > :10:48.It is not thought anyone has been hurt.

:10:49. > :10:51.Nearly 100 council workers in East Yorkshire and Lincolnshire

:10:52. > :10:56.The Taxpayers Alliance Town Hall Rich List shows 95 senior workers

:10:57. > :10:58.in our area are earning six figures a year.

:10:59. > :11:00.Lincolnshire County Council has the most high paid workers.

:11:01. > :11:02.However overall councils are employing fewer people on a six

:11:03. > :11:12.Pollution along a major road in Hull is higher than the safe limit

:11:13. > :11:16.Figures show the concentration of pollutants on Castle Street has

:11:17. > :11:23.increased by more than 20% in the past year.

:11:24. > :11:25.Police in Lincolnshire are warning people who try to lie to get

:11:26. > :11:28.out of speeding tickets that they will be caught out.

:11:29. > :11:31.It comes after a man was been jailed for six months for perverting

:11:32. > :11:35.Four years ago, Rashall-ul-Hassan could have gone on a speed awareness

:11:36. > :11:38.course if he'd admitted going at 38 miles per hour in a 30

:11:39. > :11:42.Instead he tried to deceive the police and has ended up in jail.

:11:43. > :12:00.It is not the most dramatic footage of a speeding offence but then this

:12:01. > :12:09.wasn't one of the most serious speeding offences. It is street let,

:12:10. > :12:12.it is a 30 mph limit and any residential area. At eight miles per

:12:13. > :12:19.hour over the limit, the driver Rashall-ul-Hassan could have taken a

:12:20. > :12:22.speed awareness course or a ?100 fine, but four years later after

:12:23. > :12:27.trying to deny it was him at the wheel he has been jailed for six

:12:28. > :12:35.months. That was a simple traffic violation. If he paid the fine I had

:12:36. > :12:39.taken the course this would have gone away, but unfortunately he went

:12:40. > :12:45.down this route and because of that he has been imprisoned and for the

:12:46. > :12:54.rest of his life he will have a criminal record. It was just January

:12:55. > :13:00.23rd 2013 when he broke the speed limit on this road. After giving a

:13:01. > :13:04.false name and trying to deny it was him at the wheel, he was found

:13:05. > :13:07.guilty and fined but then he complained to the central ticket

:13:08. > :13:12.office and it was then investigation that found he lied all along. He

:13:13. > :13:18.faced a much more serious charge of perverting the course of justice.

:13:19. > :13:23.And that carries a much greater penalty as former Cabinet member

:13:24. > :13:26.Chris Hughton found in 2013 when he was jailed for the same offence

:13:27. > :13:33.after his wife at the time claimed she was driving to accept penalty

:13:34. > :13:37.points so he could avoid a band. Today, a strong message in

:13:38. > :13:41.Lincolnshire. If you have committed an offence is the easiest option is

:13:42. > :13:48.to just admit it and get it behind you and move on with your life and

:13:49. > :13:58.don't do something, that means you have two then tell moralise and

:16:58. > :17:00.in East Yorkshire is to close, 13 people live at Hook Hall,

:17:01. > :17:03.which is the fourth care home in the East Riding to close

:17:04. > :17:06.in the past 12 months. It comes as it is revealed more

:17:07. > :17:09.than 90 social care workers quit their jobs every day

:17:10. > :17:15.in Yorkshire and the Humber. Kate Sweeting is in

:17:16. > :17:30.Hook Hall this evening. As you can see, this is a beautiful

:17:31. > :17:32.building, and the loss of this care home has been described as a major

:17:33. > :17:42.loss to this area. It has a good reputation locally,

:17:43. > :17:46.and the 13 residents currently living here will have two move

:17:47. > :17:50.elsewhere, which will have an impact on them. The owners have told me

:17:51. > :17:56.today, that they cannot afford to continue. They cannot recruit enough

:17:57. > :18:01.staff who can be flexible for the amount of money they can pay them,

:18:02. > :18:06.and they do not get enough funding from the local authority to fund the

:18:07. > :18:11.complex needs that there are residents have. One local councillor

:18:12. > :18:23.said this. They are very undervalued, the work pays low, and

:18:24. > :18:27.they can have far better terms of employment for the same money in

:18:28. > :18:34.supermarkets. Many people had a lot of choice about when and where they

:18:35. > :18:49.work. Town-mac has an 82% employment rate, and many people choose to work

:18:50. > :18:53.in more flexible jobs. Goole has an 82% rate. This recruitment problem

:18:54. > :19:05.is a national one, having an impact on local businesses like this one.

:19:06. > :19:11.How to refund this industry, and how do we keep hold of the staff? We

:20:40. > :20:42.will take part in one of the biggest Jack Harvey, who's from Bassingham

:20:43. > :20:46.near Lincoln, will race in the world It is a long way from the karting

:20:47. > :20:51.tracks of Lincolnshire, which he started racing

:20:52. > :20:53.on as a schoolboy. We have been following

:20:54. > :20:59.the career of Jack Harvey ever since he became

:21:00. > :21:02.the youngest junior British karting

:21:03. > :21:04.champion in 2006, at just 13 years I am about an inch away from sitting

:21:05. > :21:12.on the floor, and when you are going down the straight,

:21:13. > :21:15.everything passes you so fast. At 14, he won the European Italian

:21:16. > :21:18.Masters Karting Championships and At 16, he moved up

:21:19. > :21:24.to junior motor racing with Formula BMW, and by 17,

:21:25. > :21:30.was leading in the championships. At 19, he became the

:21:31. > :21:33.new British Formula 3 champion, a title previously

:21:34. > :21:48.held by Nelson Piquet, and at 21, landed

:21:49. > :21:50.place in the United States Indie Lights series, something else

:21:51. > :21:54.But now, just 11 years on from all of this, he has

:21:55. > :21:56.the chance to drive in the prestigious

:21:57. > :21:58.Indy 500 at the Indianapolis Speedway.

:21:59. > :22:11.It is the biggest race in America, with average speeds of over 230 mph.

:22:12. > :22:16.To be in the Indy 500 is not an opportunity that a lot of people

:22:17. > :22:26.have, and node to go and be a part of it, I do not have the words to

:22:27. > :22:31.explain. -- now I am a part of it, I cannot explain what that means. At

:22:32. > :22:39.his home, walls of medals are a reminder of what it has taken to get

:22:40. > :22:43.this far. From the early days of karting, he has been so focused and

:22:44. > :22:47.so dedicated, and he knows that every step he takes, he goes faster

:22:48. > :22:54.and faster, and he smiles a bit, because I know he is thinking, just

:22:55. > :23:04.support me, Mum. I have to be so dedicated, and that is what mums do.

:23:05. > :23:10.To go there and see him doing it, I am proud. A bucket list moment. Yes.

:23:11. > :23:14.The race will take place at the beginning of May.

:23:15. > :23:17.It's about what happens when the end of the world approaches, and is one

:23:18. > :23:19.of City of Culture's most ambitious events.

:23:20. > :23:21.Flood is a year-long show played out on TV,

:23:22. > :23:27.And tonight is the first of those live events,

:23:28. > :23:31.in the middle of an old dock on a housing development in Hull.

:23:32. > :23:36.Here's our culture correspondent, Anne-Marie Tasker.

:23:37. > :23:40.I can't put one step in front of the next.

:23:41. > :23:43.It's a story set in the future - asking how we'd cope if Europe

:23:44. > :23:47.flooded and migrants washed up on the shores of a floating city.

:23:48. > :23:50.And this week, almost 3,000 people will watch

:23:51. > :23:53.from the sides of Victoria Dock listening through

:23:54. > :23:57.headphones to the cast, out on the floating stage.

:23:58. > :24:00.We knew that it would be much easier to tell a flood

:24:01. > :24:03.story on water than it would to tell it on land.

:24:04. > :24:04.It does come with a series of technical

:24:05. > :24:08.challenges, but each of them allows an opportunity to create something

:24:09. > :24:11.for the audience that they would not have seen elsewhere, so it is

:24:12. > :24:16.The Leeds-based theatre company, SlungLow, spent more than two

:24:17. > :24:19.weeks building the set, and then another fortnight

:24:20. > :24:26.rehearsing what is the second part of a year-long project.

:24:27. > :24:28.Part one saw a film released online, and toured around

:24:29. > :24:35.I think it sounds like a radio play, and it looks a bit like an

:24:36. > :24:38.Eventually, in the night-time, there will be 57

:24:39. > :24:46.It feels like there is a community talking and thinking

:24:47. > :24:50.In that sense, it is really ancient Greek, so yes, it is

:24:51. > :24:52.definitely a hybrid, and it is all on water,

:24:53. > :24:56.Putting on an event like this, I have never seen anything

:24:57. > :24:58.like it, and the open air, on the water.

:24:59. > :25:01.I thought it was brilliant, the way it was all put together.

:25:02. > :25:03.It was very interesting, very thought-provoking.

:25:04. > :25:06.I do not think I breathed the entire way through.

:25:07. > :25:10.That is one of the best things I have seen in a long time.

:25:11. > :25:11.These live performances run until Saturday.

:25:12. > :25:14.The next part of this epic piece of theatre will be shown on

:25:15. > :25:17.BBC Two in the summer, before part four comes back
