:01:20. > :01:27.death was opened in Newcastle this afternoon by a coroner who

:01:27. > :01:32.immediately criticised the lack of transparency in the case.

:01:38. > :01:44.to death by Ronald Dixon, a man so disturbed he tried to break in to

:01:44. > :01:48.Buckingham Palace to kill the Queen. This is him filmed with his dog a

:01:48. > :01:52.quarter of an hour after he killed Ashleigh. He was on the way to a

:01:52. > :01:56.police station where detectives would learn the full horror of the

:01:56. > :02:05.attack. He stabbed her with a knife and turn it snapped, then he went

:02:05. > :02:09.to fetch another, and another, and another. He has now been sent to be

:02:09. > :02:15.secure institution for life. The mental health charity where she

:02:15. > :02:24.worked was fined �30,000 for failing to ensure her safety. So

:02:24. > :02:29.why do we need an inquest? Neither of those court cases gave

:02:29. > :02:35.Ashleigh's family the chance to ask questions. Questions about why he

:02:35. > :02:40.was suitable to live alone in the community and why Ashleigh was sent

:02:41. > :02:45.on her own to visit him. It has been said that it is crucial that

:02:45. > :02:51.the family have the circumstances of the death more fully explained.

:02:51. > :02:58.The hearing is more important than the conclusion. It was not the

:02:59. > :03:08.first time Dixon's mental health problems had let to violence. There

:03:08. > :03:13.has been an NHS inquiry into the management of Dixon. A lack of

:03:13. > :03:19.transparency was found to be startling by the coroner.
