:00:21. > :00:21.following the deaths of two patients, after neglect

:00:22. > :00:26.Michael Parke from Cockermouth died at the West Cumberland Hospital in

:00:27. > :00:34.December 2012 and Amanda Coulthard from Penrith died in the Cumberland

:00:35. > :00:37.Both suffered pneumonia after feeding tubes were inserted

:00:38. > :00:41.into their lungs instead of their stomachs.

:00:42. > :00:43.Megan Paterson was at Cockermouth Coroners court for us today.

:00:44. > :00:53.Earlier I asked her what happened to these patients.

:00:54. > :00:59.Well, Michael Park and Amanda Coulthard died after what should

:01:00. > :01:03.have been very simple medical procedures went wrong and mistakes

:01:04. > :01:07.were not detected. In the most simple form, nasogastric tubes are a

:01:08. > :01:11.type of feeding tube, said in through a patient's knows, taking

:01:12. > :01:15.nutrients to their stomach. In both these cases, the tubes were put in

:01:16. > :01:19.the wrong place and instead went to the patient's lungs. Both Michael

:01:20. > :01:23.and Amanda contracted pneumonia and later died.

:01:24. > :01:27.So, what safeguards did the trust have in place?

:01:28. > :01:28.According to the coroner, the safeguards which were in place were

:01:29. > :01:31.not strong enough. In 2005, there not strong enough. In 2005, there

:01:32. > :01:36.was a death in Carlisle related to was a death in Carlisle related to

:01:37. > :01:39.feeding tubes. In 2011, the NHS issued further guidance for trusts,

:01:40. > :01:43.and here in Cumbria, the trust was supposed to be in fermenting

:01:44. > :01:47.changes, introducing further staff training and making things safer.

:01:48. > :01:51.They failed to do that quickly enough. In the weeks leading up to

:01:52. > :01:54.Michael Park's death, several e-mails were exchanged calling for

:01:55. > :01:59.greater training, but it never happened. And Michael Park's case,

:02:00. > :02:02.his doctor failed to realise the tube had been placed into Michael's

:02:03. > :02:06.long, despite looking at x-ray which showed that was clearly the case.

:02:07. > :02:10.The coroner said directors and managers at the trust were so

:02:11. > :02:12.distracted by the potential merger and acquisition by Northumbria

:02:13. > :02:18.health care trust that day-to-day governance fell by the wayside. He

:02:19. > :02:22.said they were asleep at the wheel. The coroner said Michael died as a

:02:23. > :02:25.result of systemic neglect. So had no improvements being made by

:02:26. > :02:27.the time and a coal-fired died three years later?

:02:28. > :02:32.The coroner today said improvements had been made, policy had been

:02:33. > :02:38.changed, and staff were starting to operate under the new policy.

:02:39. > :02:41.Unfortunately, in Amanda Coulthard's case, staff treating her did not

:02:42. > :02:45.argument that properly. Today, the trust which runs the hospital said

:02:46. > :02:48.it would not comment directly on the findings of the inquest, and

:02:49. > :02:52.instead, it issued this statement. I am deeply sorry about these

:02:53. > :02:55.events, and they wish to extend my deepest sympathies to the families

:02:56. > :03:02.and friends of Michael Bach and Amanda Coulthard. The trust fully

:03:03. > :03:03.takes on the inclusion is of HM senior coroner Roberts. Patient

:03:04. > :03:07.safety remains of paramount importance to the trust, and we

:03:08. > :03:13.expect high standards of patient care at all times. These were not

:03:14. > :03:18.met for Michael or Amanda. We sincerely apologise for this.

:03:19. > :03:23.And the coroner today paid tribute several times to the families of

:03:24. > :03:26.Michael Park and Amanda Coulthard, saying they have conducted

:03:27. > :03:28.themselves with utmost dignity throughout a process which has no

:03:29. > :03:29.doubt been difficult. Thank you very much.

:03:30. > :03:32.A shot was fired by a man who tried to rob a Post Office

:03:33. > :03:35.Police said they were called to Hylton Road just

:03:36. > :03:39.Two men - one carrying a firearm - demanded money from

:03:40. > :03:43.The shot was fired when he refused to hand over any cash.

:03:44. > :03:46.Officers believe the men left in a silver Vauxhall Vectra

:03:47. > :03:52.No-one was hurt but police have stepped up patrols in the area.

:04:01. > :04:05.66-year-old Lisa Burbidge, from Gateshead, was among those

:04:16. > :04:19.Lisa Burbidge was among 30 British victims of a lone gunman.

:04:20. > :04:23.A widow and mother of two and with four grand children,

:04:24. > :04:27.she'd been on holiday at the resort of Port El Kantaoui near Sousse

:04:28. > :04:32.when the attack began on a beach and at a hotel.

:04:33. > :04:35.Today, families of the dead were at London's Royal Courts

:04:36. > :04:38.of Justice, where the circumstances leading up to the deaths of each

:04:39. > :04:45.of the victims will be examined over the course of the hearing.

:04:46. > :04:49.And on day one, a startling claim - that Tunisian law enforcement units

:04:50. > :04:53.deliberately delayed their arrival and that a local investigation

:04:54. > :05:03.criticised some police for wasting time.

:05:04. > :05:08.The court heard an account of how one beach guard opened fire, but was

:05:09. > :05:13.hit by a grenade thrown by the attacker. A member of the public who

:05:14. > :05:14.picked up the gun then found he was unable to work it.

:05:15. > :05:17.One aspect that will emerge are the claims of some families that

:05:18. > :05:18.travel companies had assured their relatives

:05:19. > :05:20.Sousse was safe despite previous terrorist attacks.

:05:21. > :05:26.The inquests are likely to last seven weeks.

:05:27. > :05:31.Hundreds of people gathered in York today to release balloons

:05:32. > :05:35.to celebrate what would have been the eighth birthday of Katie Rough.

:05:36. > :05:39.Katie died after being found with serious injuries last week,

:05:40. > :05:44.and a 15-year-old girl has been charged with her murder.

:05:45. > :05:50.The event followed a Facebook appeal to celebrate Katie's life,

:05:51. > :05:52.and it took place near the school she attended in the Woodthorpe

:05:53. > :05:56.Her family wanted balloons of every colour of the rainbow to represent

:05:57. > :05:58.the "exuberance" of Katie, and the hundreds who turned

:05:59. > :06:02.up celebrated her life with a respectful silence.

:06:03. > :06:09.Members of an anti-fracking group in North Yorkshire have set up

:06:10. > :06:12.a protest camp close to the site where permission has been given

:06:13. > :06:16.The camp is on private land but has attracted well

:06:17. > :06:20.over 1,000 visitors, as our Business Correspondent

:06:21. > :06:34.They call it a protection camp, but they call themselves protect doors.

:06:35. > :06:38.What they want to do is try to prevent fracking in this part of

:06:39. > :06:41.North Yorkshire. After exactly a month here, they claim is that the

:06:42. > :06:46.camp has been welcomed locally and further afield, with 1300 visitors

:06:47. > :06:50.coming to hear their message. We always knew that a camp would

:06:51. > :06:54.have some opposition. It is a scary thing to suddenly find a new

:06:55. > :06:57.community and your doorstep. But we're hoping that the people who

:06:58. > :07:01.still might have concerns will come and visit us, see that we are normal

:07:02. > :07:06.people, and we just want the same things as them, and we are the To Be

:08:46. > :08:49.pressure dominates, meaning things will stay fairly settled, lots of

:08:50. > :08:53.cloud around, and patchy rain for a time as well. But we will have frost

:08:54. > :08:58.free nights, and a milder feel generally. Cloud around tonight, and

:08:59. > :09:03.some outbreaks of rain, especially in the West and north. Fairly light

:09:04. > :09:07.drizzly rain which it shouldn't cause any problems, and the further

:09:08. > :09:10.south and east you are, the more dry you will stay. It will stay cloudy

:10:41. > :10:43.Temperatures up to 8-9 C on Thursday afternoon. Friday, perhaps a bit

:10:44. > :10:50.cooler. The cloud more broken overnight. Allowing temperatures to

:10:51. > :10:53.climb up to 6-7 for most of us on Friday afternoon. Staying fine and

:10:54. > :10:58.dry as we had through the working week. Into next weekend, we will
