:00:57. > :01:00.Tonight - a 22-year-old man from Rotherham who cheated death after

:01:00. > :01:06.falling through a roof he was working on says his life will never

:01:06. > :01:10.be the same again. Michael Hawkins was working for the Mansfield based

:01:10. > :01:13.company Mechanical Solutions when the accident happened two years ago.

:01:13. > :01:23.Today the company, pleaded guilty to breaches of Health and Safety

:01:23. > :01:28.rules and received a substantial fine. Amanda Harper has more.

:01:29. > :01:34.have a big scar across my back, I had a skin graft. The scars on my

:01:34. > :01:39.hands and arms and legs and my ankle. I was in hospital for two

:01:39. > :01:46.weeks. Michael was on the first day of the job carrying out repairs to

:01:46. > :01:53.a reece macro when he fell through landing 20 ft below. I jumped up,

:01:53. > :02:01.things were falling down through the roof. I got up and ran out. I

:02:01. > :02:05.could not move, I could not move. I could see blurred down my arms.

:02:05. > :02:11.Michael's accident resulted in a two weeks in hospital visit and six

:02:11. > :02:17.operations. He was employed as a sub-contract their by mechanical

:02:17. > :02:20.solutions he were carrying out repairs in Derby in 2009. At Derby

:02:21. > :02:28.Magistrates' Court mechanical solutions pleaded guilty to health

:02:28. > :02:33.and safety rules and there were safety boards but they did not have

:02:34. > :02:39.roof lights. The company was fined �10,000 and told to pay more than

:02:39. > :02:45.�5,000 to the Health and Safety Executive in costs. 35 fatalities

:02:45. > :02:50.every year from this and Ruth workers are a good proportion.

:02:50. > :02:55.Several thousand people have life- changing injuries from falling

:02:55. > :02:59.through all off industrial groups every year. Two years on, Michael

:02:59. > :03:06.is unemployed and the injuries have left him unable to carry out the

:03:42. > :03:50.inches wide and suffered horrendous injuries. His employers Compass

:03:50. > :03:54.Engineering were ordered to pay nearly �70,000 in fines and costs.

:03:54. > :03:57.You can see how small the gap was. So that's two work place accidents

:03:57. > :04:00.we've reported on in just a week and figures from the Health and

:04:00. > :04:03.Safety Executive are even more concerning as they show the number

:04:03. > :04:09.of deaths are on the increase. Nationally the number of people

:04:09. > :04:12.killed at work rose last year from 147 to 171. Here in Yorkshire there

:04:12. > :04:22.were 23 deaths that's the highest figure in the country, matched only

:04:22. > :04:25.

:04:25. > :04:29.by the north east. So what can be done to improve safety? We have to

:04:29. > :04:34.get across the message it can happen to anyone and usually it is

:04:34. > :04:38.a simple mistake and if there is nothing to stop the mistake coming

:04:38. > :04:41.to a tragic conclusion, that is what will happen. You will end up

:04:42. > :04:47.with a dead body. A stark warning to employers and employees that

:04:47. > :04:49.safety is everyone's responsibility. Also tonight - South Yorkshire's

:04:49. > :04:54.chief constable says he doesn't believe there was any deliberate

:04:54. > :04:56.police cover-up over the Hillsborough football disaster. In

:04:56. > :04:59.an exclusive interview with our Crime Correspondent John Cundy, Med
