:00:43. > :00:57.the main focus is the severe gales. Welcome to the programme. The family
:00:58. > :01:00.of a woman who died last week say she was let down by doctors who
:01:01. > :01:03.failed to spot that her breast cancer had returned. Vicky Liggatt
:01:04. > :01:07.sought help from her local surgery at least five times but her parents
:01:08. > :01:11.say they didn't realise how unwell she was.
:01:12. > :01:13.Last year the same surgery in Staveley near Chesterfield was
:01:14. > :01:17.criticised by the Care Quality Commission after a number of
:01:18. > :01:20.complaints. Now NHS England is investigating Vicky's death. Our
:01:21. > :01:29.Health Correspondent Jamie Coulson reports.
:01:30. > :01:33.Vicky Liggatt died last Wednesday. The breast cancer she thought she
:01:34. > :01:38.had beaten in 2012 had returned, spreading to every part of her body.
:01:39. > :01:42.Her family believes you was let down by doctors who should have spotted
:01:43. > :01:51.it had returned. I had to watch my daughter tell her
:01:52. > :01:58.son that she was dying within two weeks. Then I had got to go and tell
:01:59. > :02:05.my grandson that his mother had died.
:02:06. > :02:10.This video was taken in the days before Becky did. She can be seen
:02:11. > :02:18.talking about the preparations for her funeral.
:02:19. > :02:26.I want to say goodbye and everything.
:02:27. > :02:30.Becky was diagnosed in 2011 but was given the all clear in 2012. ``
:02:31. > :02:46.Vicky. It was only following a routine
:02:47. > :02:53.checkup in January that it became clear the cancer had returned.
:02:54. > :02:59.The GP should have spotted that things were seriously wrong. They
:03:00. > :03:06.can't have checked back on her records.
:03:07. > :03:08.Last year, the GP practice was criticised by the Care Quality
:03:09. > :03:13.Commission about the standards of care.
:03:14. > :03:19.I am very concerned about the care that people in Staveley are
:03:20. > :03:27.receiving and this is the most serious in a number of examples. I
:03:28. > :03:36.want to see services improving in Staveley and the wider NHS community
:03:37. > :03:41.improving. The medical group were Vicky went
:03:42. > :03:47.say they cannot comment at the moment. The hospital that treated
:03:48. > :03:56.her seedy treated her symptoms and referred her back to her GP.
:03:57. > :04:03.Vicky's funeral will take place next week, he funeral her family believe
:04:04. > :04:07.should have been years away. Next tonight: Why are Yorkshire's
:04:08. > :04:10.schools bottom of the league? Teachers and education officials
:04:11. > :04:13.came together for the first ever Yorkshire Education Summit today,
:04:14. > :04:19.after the region emerged as the worst in the country in some Ofsted
:04:20. > :04:24.league tables. Joe Inwood reports. What is education for? That is the
:04:25. > :04:28.question being asked in the conference in Leeds. But it is also
:04:29. > :04:34.being considered in schools like this one in Huddersfield where they
:04:35. > :04:38.need to pass tests is balanced against a more varied approach to
:04:39. > :04:44.learning. As well as the basic skills, children have the
:04:45. > :04:49.opportunity to do her in, wrote their own vegetables and have a
:04:50. > :04:53.sense of purpose within the community.
:04:54. > :04:59.The results of that approach have worked but many of our region's
:05:00. > :05:09.schools have not been performing. In Barnsley, only 22% of schools are
:05:10. > :05:17.rated good `` 22% of children go to schools rated good.
:05:18. > :05:21.The response to that criticism from the region as the first ever
:05:22. > :05:26.Yorkshire Education Summit. Hundreds of leading figures getting together
:05:27. > :05:30.to tackle a problem they do not deny.
:05:31. > :05:35.On a whole series of indicators going back quite a few years, this
:05:36. > :05:40.region does poorly compared with other areas of the country. It is a
:05:41. > :05:46.big challenge but there are tremendous is going on. We have some
:05:47. > :05:52.tremendous teachers and the aim is to use that expertise by moving
:05:53. > :05:56.knowledge around. We want to make the best practice available to as
:05:57. > :06:02.many young people as possible. While there are no easy solutions,
:06:03. > :06:11.they hope today's conference will bring some answers.

:11:12. > :11:16.Don't talk to strangers because you might meet some weird people.
:11:17. > :11:22.The fact that it can be manipulated in the way it has been used by a lot

:11:28. > :11:35.Sometimes I get a friend request from people I don't know so I
:11:36. > :11:43.decline it but it is freaky how they phoned my profile.
:11:44. > :11:47.I've heard of people being bullied online but I don't get involved
:11:48. > :11:53.because it will get me into trouble. It is an ever present thing and many
:11:54. > :11:55.of my friends have been affected by it.
:11:56. > :12:01.I think it is quite depressing that someone would do that, it shows they
:12:02. > :12:07.are a coward. They let me on whenever because they
:12:08. > :12:11.know I want do stuff they don't want me to do.
:12:12. > :12:22.They know I'm not the sort of person who would go on dodgy things.
:12:23. > :12:27.Tink Palmer from North Yorkshire is one of the country's leading experts
:12:28. > :12:33.on internet safety. So does she think most teenagers are as clued`up
:12:34. > :12:38.as the youngsters we spoke to? I think they are increasingly more
:12:39. > :12:43.aware and they are pretty clued up about knowing about it but when it
:12:44. > :12:49.actually comes to being online and caught up in that situation it is
:12:50. > :12:53.different things. You might know and understand about it but it is still
:12:54. > :13:00.very easy to get caught up with people who are really intent on
:13:01. > :13:05.targeting young people. How safe are we online?
:13:06. > :13:11.What a question! I would say we are as safe as our
:13:12. > :13:17.knowledge is about how to keep safe. It is not just one person's
:13:18. > :13:25.responsibility. The people around young people have responsibility.
:13:26. > :13:30.What is safe Internet use? Talking to your children or your
:13:31. > :13:35.grandchildren about what they have been doing online since the minute
:13:36. > :13:41.they have started. Just as when my children were younger and went to
:13:42. > :13:46.the cinema or the theatre I would ask them how it was. You start the
:13:47. > :13:56.dialogue as soon as possible. How common is it for it to spiral
:13:57. > :14:02.from seemingly harmless sites like Facebook and Twitter?
:14:03. > :14:10.That is where they will meet them but then they encourage them to go
:14:11. > :14:16.to more private sites. The try and get them away from other influences.
:14:17. > :14:23.Children who are sexually harmed off`line think it very difficult to
:14:24. > :14:26.talk about it but online doubly so because if the tell someone about
:14:27. > :14:37.the images they may have sent then someone may finds the images and
:14:38. > :14:45.then see what has been going on. I would advise young people always to
:14:46. > :14:49.tell someone if they are approached.
:14:50. > :14:52.If you're a young person, parent or carer who wants to learn more about

:21:17. > :21:20.be a health or transport legacy as well, cycling clubs like this one
:21:21. > :21:24.have a huge role to play in cycling that is fun, friendly and safe. On
:21:25. > :21:29.Sunday it wasn't just the hot drinks that were steaming. While Jim Brown
:21:30. > :21:32.and his dad defrosted, I asked them if they ever felt scared riding in
:21:33. > :21:36.Yorkshire's lanes. Sometimes. When you are in a group

:23:58. > :24:04.right to the heart of communities. It has got real political and social
:24:05. > :24:08.integrity about it and a beautiful heart and comedy to it. These are
:24:09. > :24:17.real funny Yorkshire people. For the lead actor, the show is a revival.

:24:23. > :24:28.years but I forgot about what an enjoyable piece it was to do.
