:00:17. > :00:20.after Irene Simister was moved twice in the last half hour of her life.

:00:21. > :00:29.The hospital has now apologhsed as Cathy Killick reports.

:00:30. > :00:33.It's just five days since the death of his mother Irene,

:00:34. > :00:37.and David Simister is at her flat to start sorting out her affairs.

:00:38. > :00:40.His grief is still very raw and it's compounded by the man

:00:41. > :00:44.of his mother's death at Harrogate Hospital.

:00:45. > :00:47.In her dying moments, a clinical bed manager insisted

:00:48. > :00:50.Irene was moved to make way for another patient.

:00:51. > :00:52.It was very upsetting because we had prepared ourselves for our lother

:00:53. > :00:56.to pass away in the room shd had been in for almost a week.

:00:57. > :01:00.She had cards from well-wishers there.

:01:01. > :01:04.She had flowers, and she had a photograph of her and my father

:01:05. > :01:06.on their wedding day on the table in front of her.

:01:07. > :01:10.In the melee that was to get her into this other room for her death,

:01:11. > :01:14.Some other stuff was left in the corridor.

:01:15. > :01:17.We were left with our mother dead in another room

:01:18. > :01:23.What was the reaction of the nurses when they werd told

:01:24. > :01:29.It was very clear that they were doing it under sufference.

:01:30. > :01:32.When my mother did die, the nurses were crying.

:01:33. > :01:34.These are nurses who see death on a daily basis

:01:35. > :01:39.But they weren't used to this particular death.

:01:40. > :01:41.Irene's family had been impressed by the care that hospital g`ve

:01:42. > :01:43.their mother until her last half-hour.

:01:44. > :01:46.But those highly emotional and precious final minutes

:01:47. > :01:51.are, for them, forever tainted by what happened.

:01:52. > :01:53.The hospital has apologised for moving her.

:01:54. > :02:13.I want to ensure that what happened to my mother and what myself

:02:14. > :02:16.and my family are experienchng does not happen to anybody else.

:02:17. > :02:28.End of life care which we dhdn't experience in the last moment

:02:29. > :02:36.For David, that means seeing protocols established to ensure

:02:37. > :02:38.dying patients aren't moved in future.

:02:39. > :02:43.So they're granted the dignhty of dying in peace.

:02:44. > :02:45.Well, Toby Scott is from the Dying Matters Coalition,

:02:46. > :02:48.a charity which aims to help people to become more comfortable

:02:49. > :02:55.How does he think the end of someone's life should be managed?

:02:56. > :02:58.The best way for someone's and of life care is for thel to get

:02:59. > :03:00.the care that they previously expressed a wish to receive.

:03:01. > :03:03.Or if they're still consciots, to get what they're asking

:03:04. > :03:11.What that means in practice is people looking for a space

:03:12. > :03:15.that's calm, that's comfort`ble that's not flustering for ehther

:03:16. > :03:16.the patient the individual concerned, or their family.

:03:17. > :03:19.Is that achievable in a hospital, or is it better

:03:20. > :03:25.You can receive excellent care in all places.

:03:26. > :03:27.Most of the time, hospitals are providing absolutely

:03:28. > :03:33.Even under the constraints that the NHS is operating under

:03:34. > :03:35.But within that average, there are examples of where the care

:03:36. > :03:39.dips below what you would w`nt and you don't get the end of life

:03:40. > :03:42.That concerns us as an organisation and I think it concerns

:03:43. > :03:46.We all of us deserve first-class end of life card.

:03:47. > :03:51.The challenge is making sure that it happens.

:03:52. > :03:55.You say that we know how to deliver it, but is there enough trahning

:03:56. > :03:57.for staff in hospitals on how to do the best practice?

:03:58. > :04:00.The interesting thing is that the front-line staff have

:04:01. > :04:03.usually got sufficient training and are doing an excellent job,

:04:04. > :04:11.A lot of the time it might be an issue for other members of staff.

:04:12. > :04:15.We are part of the group that is pushing what's calldd

:04:16. > :04:18.the ambitions framework and one of the parts of that is the phrase

:04:19. > :04:25.The idea is that anyone involved in end of life care is responsible

:04:26. > :04:27.for each individual's quality of end of life care.

:04:28. > :04:31.It might be someone who's got nothing to do with that indhvidual

:04:32. > :04:34.on a day-to-day basis, yet an action they take or fail

:04:35. > :04:37.to take might have a big impact on the care received.

:04:38. > :04:40.What should relatives look out for when it gets to this st`ge?

:04:41. > :04:42.Quite often it's a taboo subject, death in our country.

:04:43. > :04:45.But what should relatives ask for when a loved one is reaching

:04:46. > :04:52.A good early conversation with medical staff in terms

:04:53. > :04:55.of what care is being provided, what the signs might be.

:04:56. > :04:57.We published a resource on our website called

:04:58. > :05:00.What To Look For When Someone You Care For Is Dying.

:05:01. > :05:03.It talks you through the different physical changes you can expect

:05:04. > :05:07.Which can often be unsettling for the family.

:05:08. > :05:10.Most of the time, those physical changes and causing any discomfort.

:05:11. > :05:18.Talk to the doctors and nurses, have them explain what's happening

:05:19. > :05:20.so that you understand and the family understands

:05:21. > :05:22.and the person who is nearing the end of their life understands

:05:23. > :05:27.Very briefly, if there's ond thing that makes all the difference,

:05:28. > :05:32.The one thing we can do as individuals that make

:05:33. > :05:34.a difference is actually have conversations, think for ourselves,

:05:35. > :05:36.have conversations with the family about what care we

:05:37. > :05:40.Where would we like to be when we die.

:05:41. > :05:43.What would want to receive `nd what would we not want to receivd.

:05:44. > :05:45.If we've have those convers`tions and written those thoughts down it

:05:46. > :05:48.makes the whole process easher for everyone because then pdople

:06:10. > :06:16.Neighbours tried to rescue the family when the fire broke out

:06:25. > :06:33.My dad came out and tried to get people out alive.

:06:34. > :06:40.As he's gone to go into the house, he was too scared to open the door

:06:41. > :06:46.He was scared he was going to make it worse.

:06:47. > :06:52.Her 35-year-old mother and a 13-year-old sister

:06:53. > :06:54.were taken to hospital, but their injuries

:06:55. > :06:59.They've been released and are now being looked after by relathves
