:00:00. > :00:13.Good evening. The NHS Trust running That's all from us.
:00:14. > :00:16.Good evening. The NHS Trust running Stafford Hospital has been fined
:00:17. > :00:21.over ?200,000 following the death of a diabetic patient seven years ago.
:00:22. > :00:30.Gillian Astbury died after staff at the Mid Staffordshire trust failed
:00:31. > :00:35.to identify she needed insulin. Gillian died on the 11th of April
:00:36. > :00:41.2007, simply because she wasn't given insulin. She was failed by
:00:42. > :00:45.nurses on a shift is, conducting 11 drug rounds. Unusually, the high
:00:46. > :00:50.Sheriff sat next to the judge, who said responsibility lay firmly at
:00:51. > :00:56.the door of management at the highest level. Every hospital
:00:57. > :01:01.patient has the right to expect more. Serious safety management
:01:02. > :01:06.flaws were identified by our investigation. We expect lessons to
:01:07. > :01:12.be learnt across the NHS, to prevent this happening again. The carer
:01:13. > :01:18.fought for seven years for someone to be held responsible for taking
:01:19. > :01:21.everything from him. He has described the whole prosecution to
:01:22. > :01:27.me as Monty Python but he said after seven years of talking common sense,
:01:28. > :01:32.to those forced to defend the nonsensical, enough is enough.
:01:33. > :01:38.Ultimately, a trust which is ?21 million in the red has to pay
:01:39. > :01:44.?227,000. This was about getting messages out there, in the hope that
:01:45. > :01:50.it will send some shock waves through the boardrooms of other NHS
:01:51. > :01:59.trusts. So, why didn't the Health and Safety Executive prosecute
:02:00. > :02:06.individuals? I said my statement, thank you. The judge talked of
:02:07. > :02:08.accountability but family and friends feel no senior executive has
:02:09. > :02:11.been held accountable. Our reporter Liz Copper is in
:02:12. > :02:21.Stafford. Liz, do you expect there to be any more prosecutions?
:02:22. > :02:25.Gillian's family have made it clear. They see this as the end of a long
:02:26. > :02:29.and distressing process and in its statement, the trust said this was
:02:30. > :02:32.the final stage of the investigations into her death. As
:02:33. > :02:38.for other prosecutions, we know that prosecutors are looking at one other
:02:39. > :02:47.case, that is the name `` that is the case of ivy. They are looking at
:02:48. > :02:50.173 cases but we have no idea, if any, of those cases might lead to a
:02:51. > :02:54.prosecution. There has been a second inquest into
:02:55. > :02:59.another patient. What more can you tell us about that?
:03:00. > :03:06.That is the case of John Robinson. He was 20 when he died in April
:03:07. > :03:10.2006. The inquest into his death today, we heard from the coroner who
:03:11. > :03:15.said the findings of the first inquest into his case had been
:03:16. > :03:19.quashed and this was to be a fresh inquest, looking at fresh evidence.
:03:20. > :03:22.Tributes have been paid to two Midlands servicemen who died in a
:03:23. > :03:24.helicopter crash in Afghanistan. Lance Corporal Oliver Thomas and
:03:25. > :03:28.Flight Lieutenant Rakesh Chauhan were on board a Lynx helicopter
:03:29. > :03:31.which went down 30 miles from the border with Pakistan on Saturday.
:03:32. > :03:34.Three other servicemen on board were also killed, bringing the number of
:03:35. > :03:48.British casualties in the Afghan conflict to 453.
:03:49. > :03:50.Lance Corporal Oliver Thomas and Flight Lefttenant Rakesh Chauhan,
:03:51. > :03:54.both servicemen in the prime of their lives, and both described by
:03:55. > :03:58.the army and their families as bright, and gifted with a passion
:03:59. > :04:01.for life. Oliver Thomas was 26 and from Kington in Herefordshire, where
:04:02. > :04:05.he was head boy at Lady Hawkins school, voted into the job by both
:04:06. > :04:08.teachers and pupils. The first day of term today was quiet as teachers
:04:09. > :04:15.had planned a training day. Instead they mourned. We are absolutely
:04:16. > :04:20.devastated today. We had a break to let the staff cleared their thoughts
:04:21. > :04:26.and share their personal memories of Oliver. Oliver's former history
:04:27. > :04:29.teacher remembers several trips to the First and Second World War
:04:30. > :04:37.battlefields. You could see from an early age, he was interested in the
:04:38. > :04:43.Armed Forces. He always showed the utmost respect for the war dead. He
:04:44. > :04:46.would have a joke but he took his studies seriously. It is a double
:04:47. > :04:52.blow for the school because all the's mum is a member of staff here.
:04:53. > :04:57.I spoke to her earlier on the phone and she said a statement released
:04:58. > :05:00.sums up her son. It said, all seized every opportunity and achieved in
:05:01. > :05:09.all that he did. The pain of the Thomas family is matched by the
:05:10. > :05:15.other family. I was concerned for his safety. It overshadowed the
:05:16. > :05:22.pride we had for him and all he achieved at such a young age. And
:05:23. > :05:26.another Midlands school is left to remember a fallen people with pride.
:05:27. > :05:30.He was delighted to have been assigned the task in which he could
:05:31. > :05:35.use his intellect and training to make a real difference to the world.
:05:36. > :05:37.He was in the Royal Air Force. Oliver Thomas was in the
:05:38. > :05:42.intelligence corps. Both were passengers on the helicopter when it
:05:43. > :05:51.crashed in Kandahar. The bodies of both men are due to be flown back to
:05:52. > :05:55.the UK next week. Within the next hour MPs will vote
:05:56. > :05:57.on the proposed High Speed Rail link between London and Birmingham.
:05:58. > :06:00.Around 30 backbench Conservatives have said they'll be voting against
:06:01. > :06:03.the ?50 billion project, claiming it's too costly and will damage the
:06:04. > :06:06.environment. Among them is the Lichfield MP Michael Fabricant who
:06:07. > :06:09.was sacked as the party's Vice Chairman over outspoken opposition
:06:10. > :06:19.to HS2. The Government won't be defeated, as Labour has given its
:06:20. > :06:22.backing to the project. The remarkable fundraising campaign
:06:23. > :06:25.by cancer patient Stephen Sutton has now topped the ?3 million mark.
:06:26. > :06:28.Stephen's initial target was to raise ?10,000 for the Teenage Cancer
:06:29. > :06:30.Trust. But the Staffordshire teenager's story has received
:06:31. > :06:33.worldwide publicity and tonight the total stands at ?3,000,580.
