:00:18. > :00:21.Poppi Worthington was just 13 months old when she died.

:00:22. > :00:23.The police investigation into her death was sadly lacking.

:00:24. > :00:25.In fact, the Independent Police Complaints Commission today

:00:26. > :00:27.described it as unstructured and not fit for purpose.

:00:28. > :00:29.Cumbria's Chief Constable has apologised for his force's failings.

:00:30. > :00:37.In December 2012, an ambulance crew found Poppi Worthington collapsed

:00:38. > :00:43.She was taken to the local hospital but pronounced dead.

:00:44. > :00:50.Yet more than four years on, no one is any nearer to knowing why

:00:51. > :00:53.The investigator found evidence that there had been an unstructured

:00:54. > :00:57.investigation that essentially was not fit for purpose.

:00:58. > :01:00.The evidence that we obtained clearly suggested that very basic

:01:01. > :01:05.investigative principles were not followed.

:01:06. > :01:08.The IPCC report details a series of errors and omissions

:01:09. > :01:12.The house wasn't sealed off and potentially vital

:01:13. > :01:17.One of Poppi's worn nappies was allowed to be thrown away.

:01:18. > :01:20.A concern that she may have been seriously sexually assaulted

:01:21. > :01:24.was voiced at the postmortem, but the IPCC says detectives

:01:25. > :01:33.continued to focus on a natural death.

:01:34. > :01:36.And witness accounts were not taken until eight months after her death,

:01:37. > :01:39.At that point, her father, Paul Worthington was questioned

:01:40. > :01:41.on suspicion of sexual assault, something he denied.

:01:42. > :01:44.Both parents were later released without charge.

:01:45. > :01:47.An inquest into her death lasted just seven minutes and recorded

:01:48. > :01:58.The criminal investigation faded from public view.

:01:59. > :02:01.But at the beginning of 2016, a family court judge ruled that

:02:02. > :02:03.Paul Worthington probably did sexually abuse his daughter,

:02:04. > :02:06.something he has always strongly denied.

:02:07. > :02:08.The Crown Prosecution service looked at the case again,

:02:09. > :02:11.but ruled that the police didn't have enough evidence

:02:12. > :02:15.Today, the Chief Constable of Cumbria Police accepted the IPCC

:02:16. > :02:20.criticism and apologise to Poppi's family.

:02:21. > :02:24.We've been working very hard since 2014 to this date,

:02:25. > :02:27.to try and make sure that we put things in place to make sure

:02:28. > :02:33.And I'm confident that that is the case.

:02:34. > :02:43.Police investigating the death of a two-year-old boy in Birkenhead

:02:44. > :02:46.have arrested a man and a woman on suspicion of murder.

:02:47. > :02:48.The child was found unresponsive at a house

:02:49. > :02:53.He was taken to Arrowe Park hospital but pronounced

:02:54. > :02:59.A 28-year-old man and a woman aged 24 are in custody.

:03:00. > :03:02.Three weeks ago, Robert Chilowa was hailed a hero for saving

:03:03. > :03:06.the lives of two children in a house fire which killed both of their

:03:07. > :03:12.Now he's now facing deportation back to Zimbabwe.

:03:13. > :03:14.Robert says he fled from there 16 years ago fearing

:03:15. > :03:22.But the Home Office says it is safe for him to return.

:03:23. > :03:25.A petition has been organised urging the Home Office to think again.

:03:26. > :03:27.Network Rail says trains should be running in and out

:03:28. > :03:29.of Liverpool Lime Street by next Thursday.
