:00:17. > :00:19.contributed to the death of a teenager from meningitis.

:00:20. > :00:21.Izzy Gentry, who was 16, died last May at the BRI.

:00:22. > :00:24.She'd gone to hospital two days previously but had been discharged.

:00:25. > :00:31.Our health correspondent Matthew Hill reports.

:00:32. > :00:34.This grief is not something you get over or move on from.

:00:35. > :00:44.Today's conclusion that Izzy's death could have been prevented was both

:00:45. > :00:48.welcome and distressing to her parents.

:00:49. > :00:50.Izzy was taken by ambulance to the BRI emergency department

:00:51. > :00:55.A paramedic suspected she had meningitis as she was vomiting,

:00:56. > :00:58.had neck pains, dizziness, was lethargic and had

:00:59. > :01:04.But by 5am, she had been discharged after being told

:01:05. > :01:09.But her symptoms got worse and she later died

:01:10. > :01:16.The coroner said the junior doctor who discharged Isabel did not take

:01:17. > :01:24.She said his diagnosis of gastroenteritis was inaccurate

:01:25. > :01:27.and his decision was wrong - he should have reached a different

:01:28. > :01:33.diagnosis of sepsis or severe inflammatory response syndrdome,

:01:34. > :01:35.diagnosis of sepsis or severe inflammatory response syndrome,

:01:36. > :01:38.and that he should have referred her case up to a registrar.

:01:39. > :01:40.Yesterday the hearing was adjourned because Izzy's family were so upset

:01:41. > :01:44.about evidence they heard from an expert witness.

:01:45. > :01:47.A paediatrician said if she'd been admitted to her unit

:01:48. > :01:49.she would have been given antibiotics, which would

:01:50. > :01:53.Whilst we acknowledge that the hospital have recognised

:01:54. > :01:56.that they made serious communication errors, their initial

:01:57. > :02:00.approach seemed to be damage limitation rather than saying,

:02:01. > :02:08.a 16-year-old has died in our care, we must learn and improve.

:02:09. > :02:11.The hospital will now revise the way they train junior doctors

:02:12. > :02:17.Izzy's death was the catalyst for a study of sixth-formers to find

:02:18. > :02:18.out if it's worthwhile vaccinating teenagers

:02:19. > :02:30.We've now got to do the lab work and look to see if giving

:02:31. > :02:34.them the vaccine drove down the number of bacteria

:02:35. > :02:39.Izzy's very sad case was very much the catalyst that got us on to doing

:02:40. > :02:43.this study and got the various people involved on board,

:02:44. > :02:47.the funders, different agencies, the different health workers,

:02:48. > :02:52.Her friends have given their support throughout.

:02:53. > :02:55.There is now more pressure to reconsider if they, too,

:02:56. > :02:57.should be protected from meningitis B in the same way

:02:58. > :03:01.Police have arrested an 18-year-old man on suspicion

:03:02. > :03:06.It follows the death of 25-year-old Jordan Taylor,

:03:07. > :03:09.who was stabbed in the early hours of Sunday morning and died

:03:10. > :03:14.Earlier today we learnt that police had also arrested a 24-year-old man

:03:15. > :03:18.but he's been forensically eliminated from their investigation.

:03:19. > :03:21.A mother whose three-year-old son was killed by a two-tonne trailer

:08:22. > :08:26.rain bearing. Tomorrow night, still wet for most areas. Snow over the

:08:27. > :08:30.hills of Scotland, not much rain in the east, but still some. With the

:08:31. > :08:34.crowd and breeze, temperatures kept up at eight or nine foremost, but

:08:35. > :08:36.northern Scotland will be cold enough for icy patches in the far
