:00:47. > :00:49.Today an inquest jury found that a lack of on-site support
:00:50. > :00:50.and poor police interaction exacerbated the situation.
:00:51. > :01:03.Sue Cox has spent years trying to get answers
:01:04. > :01:08.He had mental health problems and ended up in this supported
:01:09. > :01:11.accommodation in Egerton Road, Bristol.
:01:12. > :01:15.On the night he died he'd gone swimming with his stepsister,
:01:16. > :01:19.niece and nephew, his dad and the two children
:01:20. > :01:22.and when he was left at 7:30 that evening,
:01:23. > :01:30.That evening was the 9th August 2013.
:01:31. > :01:35.Robert Cox was stabbed to death by another resident, Derek Hancock.
:01:36. > :01:37.Hancock had a history of paranoia, delusional behaviour
:01:38. > :01:43.and making false allegations of sexual harassment.
:01:44. > :01:49.On the night, Derek Hancock phoned police, making false allegations
:01:50. > :01:55.The police came and spoke to Hancock, but after they'd gone,
:01:56. > :02:02.His third and final call became the subject of an investigation
:02:03. > :02:08.Call handler Susan Akerman does say police "will deal with it".
:02:09. > :02:11.But Derek Hancock goes on: "I will take the law
:02:12. > :02:15.into my own hands because the law is not doing anything."
:02:16. > :02:21."Well, that's entirely up to you Derek, OK?"
:02:22. > :02:26.She doesn't say don't do that, which I would think a call handler
:02:27. > :02:28.should be saying to anybody, mental health patient
:02:29. > :02:33.I understand the police watchdog agreed, but didn't think
:02:34. > :02:37.Ms Akerman's actions contributed to Robert Cox's death.
:02:38. > :02:41.But the watchdog did say she may have gone on to give them false
:02:42. > :02:46.and misleading information, which she denied.
:02:47. > :02:51.Ms Akerman didn't want to do an interview but has told
:02:52. > :02:54.the coroner here she'd had no training about mental health
:02:55. > :02:59.and she didn't know Hancock had such problems.
:03:00. > :03:02.Speaking about that call, she said he hadn't given
:03:03. > :03:06.the impression he was going to go on to do what he did,
:03:07. > :03:09.and that she took three or four calls a day from
:03:10. > :03:15.Police say they're committed to learning lessons.
:03:16. > :03:19.But Sue Cox has had to fight for answers.
:03:20. > :03:23.A serious case review was critical about the agencies involved.
:03:24. > :03:32.But she forced an independent investigation which found the case
:03:33. > :03:34.review wasn't sufficiently in-depth or inquisitive, and...
:03:35. > :03:37."The process floundered" and it's not surprising
:03:38. > :03:46.It makes you question whether there are other cases
:03:47. > :03:49.where the same questions, the same evidence, the same
:03:50. > :03:52.problems are arising with serious case reviews.
:03:53. > :03:54.The Bristol Adult Safeguarding Board has apologised
:03:55. > :04:02.The group which ran Egerton Road said it's carried out risk
:04:03. > :04:04.assessments and that Robert's death couldn't have been predicted.
:04:05. > :04:07.But for Sue and her family, the battle isn't over.
:04:08. > :04:19.We believe there is more work to be done to prevent future deaths.
:04:20. > :04:22.Mistakes made in the care of a new mum, with a history

:04:25. > :04:28.Charlotte Bevan was able to walk out of a maternity
:04:29. > :04:30.ward in Bristol with her four-day-old baby, unchallenged.
:04:31. > :04:32.Both mother and child would later be found dead.

:04:50. > :04:51.Charlotte had attempted suicide before, after hearing
:04:52. > :04:56.During her pregnancy she'd stopped taking the medication that
:04:57. > :05:01.controlled her schizophrenia, believing it could harm her baby.
:05:02. > :05:04.In fact, the review found there was "no definitive answer"
:05:05. > :05:07.about the safe use of medication in this case.
:05:08. > :05:11.What we will always do with patients is have an in-depth conversation
:05:12. > :05:15.with them to say your mental health needs need to be the priority
:05:16. > :05:17.because there is more risk to your child if your mental health
:05:18. > :05:20.deteriorates than of the risk of medications transferred
:05:21. > :05:26.Their bodies were found later after a search by police,
:05:27. > :05:29.the findings of today's review into her care followed closely
:05:30. > :05:33.This was a particularly difficult case to manage
:05:34. > :05:39.as long-term ill mental health and pregnancy came together.
:05:40. > :05:43.Education in schools and a general increased awareness
:05:44. > :05:52.of all mental health issues will go some way to preventing what happened
:05:53. > :05:55.It also highlighted confusion about who was responsible
:05:56. > :06:14.for monitoring Charlotte's mental health, saying...
:06:15. > :06:16.Charlotte had been gone for almost 40 minutes before
:06:17. > :06:20.At her inquest a coroner said there had been a series of failings
:06:21. > :06:23.in not recognising the symptoms of her relapse into psychosis.
:06:24. > :06:26.Today's review called for clearer rules about who was in charge
:06:27. > :06:28.for monitoring the mental health of patients like Charlotte,
:06:29. > :06:36.to stop anything similar ever happening again.
:06:37. > :06:40.Since Charlotte Bevan's death in 2014, changes have already been
:06:41. > :06:43.made to support pregnant women with severe psychiatric disorders.
:06:44. > :06:48.Bristol, South Gloucestershire and North Somerset have managed
:06:49. > :06:51.to secure ?1.2 million of government funding for a team of health
:06:52. > :06:56.visitors and nurses to give women at high risk extra support
:06:57. > :07:01.And funding has been agreed for another four-bed maternity
