:01:10. > :01:14.Sian Hollands's children were among the family members who came to court
:01:15. > :01:17.to hear about the devastating I knew in my heart the truth
:01:18. > :01:24.and today confirmed that We as a family are
:01:25. > :01:29.absolutely heartbroken. Sian was admitted to the Durrant
:01:30. > :01:32.Valley Hospital in November 2015, complaining of chest pains
:01:33. > :01:36.and problems breathing. She had had an ectopic pregnancy
:01:37. > :01:39.three weeks earlier. But doctors put her symptoms down
:01:40. > :01:42.to the fact that she had stopped taking methadone
:01:43. > :01:44.to come off heroin. They did not consider
:01:45. > :01:54.any alternatives. The conclusion of the
:01:55. > :01:56.coroner was damning. The death of Sian
:01:57. > :01:58.Hollands was due to the failures of the doctors to examine,
:01:59. > :02:01.diagnose and treat her for a Had they done so on the balance
:02:02. > :02:06.of probability she She was a one in a million sister
:02:07. > :02:10.and I miss her terribly. All we have left of
:02:11. > :02:12.her is her children. And I just can't believe
:02:13. > :02:15.that it does come down to four lines on a piece of paper
:02:16. > :02:19.of why she is not here. The inquest heard doctors failed
:02:20. > :02:22.to read notes made by Staff failed to carry out
:02:23. > :02:30.regular observations have been told she was well
:02:31. > :02:35.enough to leave hospital. That decision was made
