:00:41. > :00:46.He was dependent on alcohol and known to take drugs.
:00:47. > :00:48.The inquest heard while in police custody he put
:00:49. > :00:54.He was assessed as being at very high risk of self-harm.
:00:55. > :00:56.When he was transferred to Woodhill Prison,
:00:57. > :01:03.He was given a three-minute mental-health screening but never
:01:04. > :01:07.referred for an urgent mental health review.
:01:08. > :01:10.I think a mental health assessment should have been done for him
:01:11. > :01:14.and if he had seen the doctor when he was there, he never saw
:01:15. > :01:24.a doctor, I think Michael would still be with us today.
:01:25. > :01:27.The jury was told while at Woodhill Prison razor blades were found
:01:28. > :01:32.in his cell and handed to staff by his cell-mate.
:01:33. > :01:36.Prescription medication was not always given because staff
:01:37. > :01:38.were concerned he was under the influence of drugs.
:01:39. > :01:41.On the day he was found hanged, his cell-mate
:01:42. > :01:47.The inquest heard on that day Michael Cameron should have been
:01:48. > :01:48.observed by staff every 30 minutes but he was found hanging 36
:01:49. > :01:56.He died at Milton Keynes Hospital two days after being found
:01:57. > :01:59.in his cell having suffered severe brain damage.
:02:00. > :02:02.Today, the jury found there was a failure to carry out
:02:03. > :02:04.adequate procedures and reviews and each of these failures
:02:05. > :02:24.These conclusions are the same, Michael is the 14th self-inflicted
:02:25. > :02:27.death between 2013 and 2016, it is dispiriting to ask the same
:02:28. > :02:31.questions of off in the same witnesses and have the same answers.
:02:32. > :02:33.It was the conclusion Michael's mother had been hoping
:02:34. > :02:40.for but she says nothing will bring him back.
