:00:12. > :00:16.to the death of a prisoner at Woodhill Prison in Milton Keynes.

:00:17. > :00:18.45-year-old Michael Cameron was found hanging in his cell days

:00:19. > :00:21.after arriving at the jail - he's one of 18 prisoners that have

:00:22. > :00:26.taken their own lives in the past 4 years.

:00:27. > :00:29.The prison says changes have already been made and nobody has

:00:30. > :00:38.Kate Bradbrook has been at the inquest.

:00:39. > :00:40.Michael Cameron had been charged with serious offences

:00:41. > :00:46.He was dependent on alcohol and known to take drugs.

:00:47. > :00:48.The inquest heard while in police custody he put

:00:49. > :00:54.He was assessed as being at very high risk of self-harm.

:00:55. > :00:56.When he was transferred to Woodhill Prison,

:00:57. > :01:03.He was given a three-minute mental-health screening but never

:01:04. > :01:07.referred for an urgent mental health review.

:01:08. > :01:10.I think a mental health assessment should have been done for him

:01:11. > :01:14.and if he had seen the doctor when he was there, he never saw

:01:15. > :01:24.a doctor, I think Michael would still be with us today.

:01:25. > :01:27.The jury was told while at Woodhill Prison razor blades were found

:01:28. > :01:32.in his cell and handed to staff by his cell-mate.

:01:33. > :01:36.Prescription medication was not always given because staff

:01:37. > :01:38.were concerned he was under the influence of drugs.

:01:39. > :01:41.On the day he was found hanged, his cell-mate

:01:42. > :01:47.The inquest heard on that day Michael Cameron should have been

:01:48. > :01:48.observed by staff every 30 minutes but he was found hanging 36

:01:49. > :01:56.He died at Milton Keynes Hospital two days after being found

:01:57. > :01:59.in his cell having suffered severe brain damage.

:02:00. > :02:02.Today, the jury found there was a failure to carry out

:02:03. > :02:04.adequate procedures and reviews and each of these failures

:02:05. > :02:24.These conclusions are the same, Michael is the 14th self-inflicted

:02:25. > :02:27.death between 2013 and 2016, it is dispiriting to ask the same

:02:28. > :02:31.questions of off in the same witnesses and have the same answers.

:02:32. > :02:33.It was the conclusion Michael's mother had been hoping

:02:34. > :02:40.for but she says nothing will bring him back.

:02:41. > :02:41.Following the national cyber security attack

:02:42. > :02:44.affecting parts of the NHS, BBC South understands the health

:02:45. > :02:46.trusts in Oxfordshire, Buckinghamshire and Swindon

:02:47. > :02:53.In many parts of the country computer systems have been shut down

:02:54. > :02:55.and contingency plans have been activated.

:02:56. > :03:00.In some instances hackers have been demanding money to unlock systems.

:03:01. > :03:02.Here, our health trusts have said they are "on alert"

:03:03. > :03:09.New figures suggest more than 2-hundred cyclists have been

:03:10. > :03:12.caught riding their bikes in parts of Oxford city centre

:03:13. > :03:18.which are restricted to pedestrians in little more than a year.

:03:19. > :03:19.Under the council's public spaces protection orders,

:03:20. > :03:22.restrictions are in place in daylight hours on Queen Street

:03:23. > :03:26.Campaigners say the rules arent clear enough,

:03:27. > :03:35.Oxford is known for its high number of cyclists.

:03:36. > :03:38.But here in this part of the city centre it is a pedestrian-only zone

:03:39. > :03:43.during the day between 10 and 6, or should be.

:03:44. > :03:45.We have only been here for a few minutes but already dozens

:03:46. > :03:48.of cyclists are ignoring this ban like this one right here.

:03:49. > :03:55.Campaigners say the signage isn't clear and it is too high.

:03:56. > :03:58.They are not in the right place for where a cyclist

:03:59. > :04:02.Especially with a lot of people on foot they are trying

:04:03. > :04:08.to quite rightly make sure they don't interact with.

:04:09. > :04:12.So the signs are not in the right place to make an effective message.

:04:13. > :04:15.More than 230 cyclists have been caught breaching the rules according

:04:16. > :04:23.And it says this type of offence is much more of a problem compared

:04:24. > :04:25.to other behaviour they class as a nuisance like

:04:26. > :04:35.Do you know you are not supposed to ride your bike along here?

:04:36. > :04:41.Are you aware you're not allowed to ride your bike in this area?

:04:42. > :04:46.I guess I was aware there were not that many bikes going down here.

:04:55. > :05:04.Anyone caught flouting the ban is initially given a warning.

:05:05. > :05:10.But repeat offenders can then be given a fixed penalty notice.

:05:17. > :05:18.New recommendations at Oxford City Council include

:05:19. > :05:21.targeting groups like students who might be unaware of the rules

:05:22. > :05:29.But it's clear some people just aren't bothered.

:05:33. > :05:35.Are you aware you are breaking the rule?

:05:36. > :05:41.As long as I don't run anyone over at this speed.

:05:42. > :05:47.A week of action to target Rogue Traders has been carried out

:05:48. > :05:50.Officers have been using Number Plate recognition cameras

:05:51. > :05:52.to pull over vans and small trucks that are suspected of

:05:53. > :05:57.More than a hundred vehicles were stopped.

:05:58. > :06:00.Two drivers were found to have no licences and at least four

:06:01. > :06:04.One unlicensed scrap metal dealer was stopped and will be formally

:06:05. > :06:13.We target rogue traders which the community like.

:06:14. > :06:15.We are looking for defective unroadworthy vehicles which is good

:06:16. > :06:20.Every now and then we'll get someone who is wanted as well

:06:21. > :06:26.Long delays are expected on the roads this weekend as part
