:00:45. > :00:50.An inquest into Karin Cheshhre's death heard her son Jay took his

:00:51. > :00:53.life after he was investigated for an allegation of rape.

:00:54. > :00:56.The coroner said Karin never recovered from losing her son.

:00:57. > :01:02.For Karin Cheshire, the pain of losing a child was something

:01:03. > :01:06.A mother-of-two who'd grown to become the closest

:01:07. > :01:10.My mum was the most amazing mum in the world.

:01:11. > :01:13.I couldn't have asked for a better mum and I couldn't have acttally

:01:14. > :01:15.asked for a better brother, and our little unit

:01:16. > :01:22.They were just amazing, amazing people.

:01:23. > :01:25.In July this year at the Chdshire's family home in Bitterne,

:01:26. > :01:30.The inquest heard today how Karin had suffered a mental breakdown

:01:31. > :01:32.following the death of her son and had been in and out

:01:33. > :01:35.of psychiatric units run by the Southern Health Trust on five

:01:36. > :01:38.occasions over a period of six months.

:01:39. > :01:41.The Trust was criticised by the coroner for failing to pick

:01:42. > :01:46.up suicidal tendencies just three days before she hanged hersdlf.

:01:47. > :01:50.It was the second tragic ch`pter in the saddest of stories.

:01:51. > :01:53.A year before Karin took her own life, her son Jay committed suicide

:01:54. > :01:59.At the age of just 17, Jay, who had a history

:02:00. > :02:02.of depression, was the subjdct of an investigation into an alleged

:02:03. > :02:05.rape - an investigation that was dropped due

:02:06. > :02:08.to a lack of evidence, and after the police said

:02:09. > :02:11.the alleged victim wouldn't support a prosecution.

:02:12. > :02:13.Jay's own inquest heard how it had a profound effect

:02:14. > :02:20.Jay also hanged himself, but that's not all.

:02:21. > :02:24.My mum and my brother used the same rope.

:02:25. > :02:28.My mum, after receiving that back from the police,

:02:29. > :02:36.that's something that's going to stick with me

:02:37. > :02:44.I have very little family, and I'm just so angry.

:02:45. > :02:46.It's the law that alleged victims of sexual assault

:02:47. > :02:49.are guaranteed anonymity, whether the investigation ends up

:02:50. > :02:53.in court or not, unless thex choose to waive their right.

:02:54. > :02:59.My brother is gone and my mum is gone -

:03:00. > :03:03.that is two lives lost, because of a certain situathon.

:03:04. > :03:06.Today the coroner said that Karin was a special person.

:03:07. > :03:09.The Southern Health Trust apologised to her remaining family

:03:10. > :03:12.for their failure to recognise the significance

:03:13. > :03:17.Camellia clings on to the mdmories of the good times her

:03:18. > :03:27.Meanwhile, the mental health trust in Sussex is being criticisdd

:03:28. > :03:31.for underestimating the risk posed by some patients and for fahling

:03:32. > :03:36.A review of ten killings fotnd some to be preventable and predictable,

:03:37. > :03:40.including the murder of Susan Goswell in West Chhltington

:03:41. > :03:48.Nine years ago, Roger Goswell used a kitchen knife and a mallet

:03:49. > :03:51.to kill his wife Susan in their home in West Chiltington

:03:52. > :03:57.He'd been receiving mental health treatment for a number of ydars

:03:58. > :04:00.but hadn't been sectioned, despite his wife telling thd police

:04:01. > :04:07.If they predicted it, then they could've prevented it

:04:08. > :04:09.I think my mother's life, there's no question

:04:10. > :04:11.if that was the case, they could've saved it,

:04:12. > :04:16.And they have a duty of card, and as a duty of care,

:04:17. > :04:18.they should have made sure that they prevented

:04:19. > :04:25.Today's report examined the Goswell case and nine others involvhng

:04:26. > :04:27.patients under the care of Sussex Partnership NHS Foundation

:04:28. > :04:33.The trust launched the revidw after Matthew Daley killed

:04:34. > :04:37.Donald Lock on the A24 in Findon last year.

:04:38. > :04:39.The 79-year-old great-grandfather was stabbed 39 times by Daldy

:04:40. > :04:50.I think he fell to the floor and then it was just like autophlot ..

:04:51. > :04:52.Daley was convicted of manslaughter on the grounds of

:04:53. > :04:57.During the trial, the jury heard his mother had pleaded

:04:58. > :05:03.The trust admitted it should've listened to his family more closely.

:05:04. > :05:05.Another of the cases involvdd a patient who became the victim

:05:06. > :05:12.Janet Muller was burned to death in the boot of a car

:05:13. > :05:15.The report found there were similarities between

:05:16. > :05:18.the killings and the trust had severely underestimated the threat

:05:19. > :05:26.My message to the families hs one of, again, apology and condolence,

:05:27. > :05:31.but an assurance to them that we will learn the lessons

:05:32. > :05:35.for Sussex in relation to these particularly tragic events.

:05:36. > :05:38.The trust has promised families like the Goswells that in the future

:05:39. > :05:40.they will be listened to and more involved

:05:41. > :05:50.We asked the mental health charity Sane if care

:05:51. > :05:57.With the community care polhcy, people, however disturbed,

:05:58. > :05:59.are living in the community, but instead of being under strict

:06:00. > :06:03.supervision, they're under community teams and there has been a lot

:06:04. > :06:13.There have been cuts that h`ve made them are overstretched,

:06:14. > :06:17.It means that there's not one consistent person that everxbody

:06:18. > :06:19.wants and needs, who knows the history, and again and `gain,

:06:20. > :06:27.Dorset Police say no-one will be prosecuted following the de`th

:06:28. > :06:31.of a 17-year-old in a fight in Bournemouth in July.

:06:32. > :06:33.Jack Gudge was found unconscious outside a Tesco

:06:34. > :06:38.He was declared dead on arrival at hospital.

:06:39. > :06:41.Four people had been arrestdd in connection with the incident

:06:42. > :06:44.But prosecutors have advised police it's not possible for them to prove

:06:45. > :06:50.whether Jack died as the result of an unlawful act.

:06:51. > :06:52.Southern Railway train drivdrs are being balloted for strike action
