services, it aimed to establish greater communication between | :01:25. | :01:26. | |
different groups sought to better protect children. However, an | :01:27. | :01:32. | |
independent review led by Professor Alexis Jay said the board failed in | :01:33. | :01:35. | |
what it was asked to do. It is a highly critical report which I led | :01:36. | :01:39. | |
to the board spent too much time on wrong issues, that tensions existed | :01:40. | :01:43. | |
between those at the top and a lack of structure prevented vital | :01:44. | :01:49. | |
information being shared. On that board, if the medical profession | :01:50. | :01:51. | |
agreed to share with trusts, with everybody else, for example the | :01:52. | :01:54. | |
mental health well-being of the parents and then that parent becomes | :01:55. | :01:58. | |
unwell, that can share that information with the social worker | :01:59. | :02:01. | |
who can name, for example, take preventative action. It is that | :02:02. | :02:04. | |
critical sharing of information which makes is able to work more | :02:05. | :02:07. | |
preventative late, rather than waiting until the incident happens. | :02:08. | :02:11. | |
While the review does not say that children were harmed as a result of | :02:12. | :02:17. | |
poor practice, it does say that the proper mechanisms were not in place | :02:18. | :02:20. | |
for properly recording children's deaths. What she says in this report | :02:21. | :02:24. | |
is that one, the child death overview panel had not been set up | :02:25. | :02:29. | |
but secondly there are too many processes and a lot of confusion | :02:30. | :02:32. | |
about who gets what information when a child dies, particularly when it | :02:33. | :02:35. | |
is unexpected. That has to be streamlined and it has to be clearer | :02:36. | :02:41. | |
what happens. The review makes 11 recommendations, including that the | :02:42. | :02:45. | |
