half his body weight after paying for surgery in India, says the NHS

Thousands of people are waiting for gastric operations

because the criteria for carrying out the surgery here is stricter

:01:40.:01:41.

But, with almost a quarter of adults classed as obese, funding treatment

:01:42.:01:48.

In a moment, we'll look at those pressures.

First, Tim Rogers with Zac Smith's story.

At 50 stone, Zac Smith was eating himself into an early grave. In his

:02:03.:02:16.

30s, he relied he needed help. I was not able to walk down the aisle of

:02:17.:02:24.

the church. All I kept thinking was, you are next. He thought he would

:02:25.:02:27.

qualify for an operation to reduce the size of his stomach but was

:02:28.:02:32.

turned down under NHS rules in Wales. His only option was to go for

:02:33.:02:38.

-- the -- go to India for a private operation. The surgery has since

:02:39.:02:44.

helped him lose 27 stone. But after losing so much weight, Zac was left

:02:45.:02:49.

with three stones of loose skin. He asked the NHS for another operation

:02:50.:02:52.

to remove it, but he was again turned down. At six foot five, he

:02:53.:02:57.

was told he would effectively have to reduce his weight to 12.5 stone.

:02:58.:03:03.

I do not know how some of my size, my height, how I can get my weight

:03:04.:03:16.

down to under that. So it was back to India, where he kept a video

:03:17.:03:23.

diary, as he had another operation. Fingers crossed. The operation was a

:03:24.:03:28.

success, but what followed came as an unpleasant surprise. His surgical

:03:29.:03:31.

wound became infected and after flying home he had to spend two

:03:32.:03:36.

weeks in an isolation ward in hospital. Ironically, this came at a

:03:37.:03:41.

greater cost to the NHS than the savings they made by refusing the

:03:42.:03:45.

bariatric surgery he asked for in the first place.

:03:46.:03:49.

The Welsh Government has told us that the criteria for treatment is

:03:50.:03:53.

being revised and plans are in place to increase funding for more

:03:54.:03:59.

operations over the next five years. But doctors say without surgery

:04:00.:04:03.

sooner, their patients will put greater demand on cancer, diabetes

:04:04.:04:04.

and heart services. The term "obesity epidemic"

:04:05.:04:07.

is now a well-worn phrase here in Wales, where almost a quarter

:04:08.:04:09.

of adults are classed as obese and almost half of all men are

:04:10.:04:12.

overweight by the age of 42. Poor diet and lack of exercise are

:04:13.:04:17.

largely to blame and that's putting People who are overweight or obese

:04:18.:04:20.

face a higher risk of chronic health problems,

:04:21.:04:28.

including cardiovascular disease, The Welsh Government estimates that

:04:29.:04:33.

costs our NHS at least ?1 million a week, possibly

:04:34.:04:41.

as much as ?73 million a year. And it's not just paying for drugs

:04:42.:04:50.

and doctors - bigger patients need bigger beds, and to transport the

:04:51.:04:53.

obese, specially adapted ambulances. If we don't care about this, we are

:04:54.:05:09.

going to find our NHS falling apart within the next 15 or 20 years.

:05:10.:05:15.

Because the cost of treating the chronic diseases associated with

:05:16.:05:19.

obesity are going to spiral out of control.

:05:20.:05:20.

A stark warning from Jonathan Barry, but he could be part

:05:21.:05:23.

He runs Wales' only specialist centre

:05:24.:05:25.

for bariatric or weight-loss surgery at Swansea's Morriston Hospital.

:05:26.:05:27.

He says thousands of people could benefit

:05:28.:05:29.

from a gastric band, but in Wales the rules governing who's eligible

:05:30.:05:32.

are too stringent, despite it being a very cost effective solution.

:05:33.:05:43.

To suggest it is expensive surgery is a complete fallacy. We know these

:05:44.:05:49.

operations pay for themselves within two and a half years. To put it into

:05:50.:05:54.

context, a lot of these people have diabetes, hypertension, all those

:05:55.:05:59.

conditions go away. Those conditions are very expensive to treat over the

:06:00.:06:01.

next 30 or 40 years. In England, the National Institute

:06:02.:06:02.

of Clinical Excellence has far less stringent rules for accesing

:06:03.:06:05.

bariatric surgery, and as the crisis worsens in Wales, the Health Service

:06:06.:06:07.

here has now pledged to catch up, but they admit it's

:06:08.:06:10.

a difficult balancing act. We have to prioritise between Cancer

:06:11.:06:23.

care, care for elderly people, cardiovascular services, as well as

:06:24.:06:28.

examining what we can do for people who are obese. A difficult set of

:06:29.:06:33.

choices, but the decision has been made to do more, to go from 80

:06:34.:06:38.

operations per year to 300 per year, which would actually catch up with

:06:39.:06:43.

the current English number of operations.

:06:44.:06:44.

The Welsh Government say bariatric surgery is not a magic bullet and

:06:45.:06:47.

They say they'll continue to tackle obesity through targeted education,

:06:48.:06:51.

encouraging the right lifestyle choices

:06:52.:06:52.

If that doesn't work, some may question whether the NHS

:06:53.:06:57.

should be funding more gastric band surgery for what is largely

:06:58.:06:59.

But others would argue the simple economics suggest

Let's talk to our political reporter Paul, Martin.

A week is a long time in politics - what's changed?

:08:20.:08:26.

It is fair to say that her assessment of the dash rang her

:08:27.:08:32.

behaviour has not changed, but other key factors have. To recap, she used

:08:33.:08:37.

the N word when trying to describe how she thinks some vulnerable

:08:38.:08:41.

people in her award, whose first language in Welsh, are treated when

:08:42.:08:45.

they go to hospital and across-the-board in England. She was

:08:46.:08:48.

clear last week she was wrong to use the word, but it was a resignation

:08:49.:08:53.

-- was not a resignation issue. The position of the Council Leader,

:08:54.:08:56.

Barry Thomas, is what has changed. He runs a group of independent

:08:57.:09:01.

councillors in Powys who do not have overall control of the council. If

:09:02.:09:05.

opposition groups are unhappy, they can cause problems for the

:09:06.:09:08.

leadership. Opposition groups have not been happy. They think it was a

:09:09.:09:14.

resignation issue, so do the Unison union and was talk of them bringing

:09:15.:09:17.

a vote of no-confidence in the leadership. Last week Barry Thomas

:09:18.:09:22.

was clear she did not need to go, today, Myfanwy Alexander has told us

:09:23.:09:26.

that following pressure from the opposition, Barry Thomas has said

:09:27.:09:30.

she does have to go. To me, resignation is an admission

:09:31.:09:36.

in some sense that is -- there is a case to answer and I do not believe

:09:37.:09:40.

there is. I used a foolish and insulting word that I should not

:09:41.:09:44.

have used. I apologise fully for that and the vast majority of people

:09:45.:09:48.

I have spoken to accept that. I can be responsible for my own actions

:09:49.:09:52.

and I can be sorry for my own actions, but the actions of

:09:53.:09:55.

opposition groups are beyond my control.

:09:56.:09:59.

What has the reaction been? Unsurprisingly, perhaps, the

:10:00.:10:03.

opposition groups have welcomed the decision, saying it was inevitable

:10:04.:10:06.

and overdue. In her resignation letter, Myfanwy Alexander made some

:10:07.:10:12.

broader points about politics in Powys. She said there is a toxic

:10:13.:10:17.

nature of division that led to the pressure on her, and she said anyone

:10:18.:10:21.

hoping to make any difference that -- to the people of Powys sit --

:10:22.:10:25.

should not bother going into politics. Barry Thomas was

:10:26.:10:32.

unavailable today, but he did say in a statement that he accepts her

:10:33.:10:33.

resignation. Police are investigating the death

:10:34.:10:35.

of a man in his 20s, thought to have been found at the Aberystwyth Arts

:10:36.:10:38.

Centre, on the university campus. His body was found on Monday night

and is being treated as unexplained. A group trying to reopen

a leisure centre in Wrexham has had its bid for a ?50,000 grant rejected

this year as part of budget cuts. The Splash Community Trust are

Cymru are certainly making their stance on the referendum known. Do

they think this Unionist move spikes the guns of the Yes campaign is Mac

we have heard all this before. They promised jam tomorrow and when it

:16:17.:16:21.

comes to it, the three main parties in London failed to deliver. The

:16:22.:16:25.

only way that people in Scotland can be sure of getting more powers is to

:16:26.:16:31.

vote yes on September the 18th. The scene is set, tonight sees the first

:16:32.:16:36.

TV debate between the opposing sides of the campaign. The terms and

:16:37.:16:40.

topics of the debate in Glasgow are well rehearsed, but the

:16:41.:16:42.

referendum's affect on Wales is less sure. Entrance of what additional

:16:43.:16:48.

powers are offered to Scotland, the parties do not agree so I will -- I

:16:49.:16:52.

think we will have to see the results of the General Election, if

:16:53.:16:56.

there has been a No vote, to know what this means for Wales. It is

:16:57.:17:02.

true to say that Scotland is a point of comparison for Wales. If Moore is

:17:03.:17:06.

offered to Scotland, there will be pressure in Wales to catch up. There

:17:07.:17:11.

is little over a month before Wales hosts the NATO summit and just some

:17:12.:17:15.

six weeks until Scotland decides. But the debate on the effect of the

referendum on Wales has only just begun.

The National Eisteddfod is in full swing at Llanelli.

as you can see, people are pouring through here, leaving the Eisteddfod

But as well as being a place to meet up with old friends

and enjoy performances on the stage, the Eisteddfod is a shop window

There are more than 300 stalls on the Maes,

:17:45.:17:48.

selling everything from art and books to T-shirts and jewellery.

Rhodri Lewis has been asking stallholders how important

:17:51.:17:52.

For decades, the Eisteddfod has been known for this.

:17:53.:18:04.

But nowadays it's also known for this.

:18:05.:18:07.

Small businesses from all over Wales come here every year, set up stalls

Cowbois has been part of the maes for more than 20 years.

:18:11.:18:15.

Sales during Eisteddfod week are critical to a company which,

:18:16.:18:18.

apart from its appearances here, only exists online.

:18:19.:18:26.

The Eisteddfod is very important to showcase the company and show our

:18:27.:18:32.

products. We can also introduce new products as well. We can gain a new

:18:33.:18:36.

clients. It is very important and also, it is an economic way of

:18:37.:18:40.

advertising the company throughout Wales.

:18:41.:18:41.

There are more than 300 stands at this year's Eisteddfod.

:18:42.:18:45.

They'll each have paid at least ?815 to be here.

:18:46.:18:48.

The flip side is that the festival is worth between ?6-8

:18:49.:18:50.

Crefftau y Bont is part of that economy.

:18:51.:19:04.

Based just a few miles from here in Pontarddulais,

:19:05.:19:07.

its owner says the chance to network here is too good to miss.

There is no other opportunity we would have to be able to go to the

Welsh event, meet other Welsh business people and food producers,

craft makers, and to meet local Government people who are very

:19:25.:19:25.

interested in what we can do. But it's not just about buying

:19:26.:19:27.

and selling. Amidst the jewellery and paintings,

:19:28.:19:29.

this solicitor's company is here for the first time,

:19:30.:19:31.

but what's in it for them? We have recently expanded into West

:19:32.:19:42.

Wales. Last month, we opened an office in Carmarthen. So it was an

:19:43.:19:48.

excellent decision to have the Eisteddfod here, to raise awareness.

How good is it for business? I am quite surprised we are here, I

:19:58.:20:02.

think. But it is good, people have not been aware we have opened an

:20:03.:20:06.

office here, they are interested, we are employing people from Carmarthen

:20:07.:20:10.

in the office so it is good for business.

:20:11.:20:11.

The Eisteddfod has always been a showcase for welsh culture,

:20:12.:20:13.

but these days that's expanded to give Welsh businesses a shop window

:20:14.:20:16.

In other news here, two safes holding

around ?6,000 were stolen from the visitor centre here over night.

Police search teams with dogs have been investigating.

Eisteddfod officials say the cash taken was float money,

Well, in the Pavilion today the main ceremony saw the

Daniel Owen Memorial Prize awarded for the Best Novel.

And the winner was Lleucu Roberts from Rhostryfan near Caernarfon.

She takes away ?5,000 for her book, Rhwng Edafedd.

The judges said the writer showed she has a sound

torchlight procession at the highest point in the mountain range. In

Carnarvon, there was a vigil. The faces of some of those who went

has affected me tonight. It touches the old heartstrings.

At the commemoration service, the lights of Llandaff Cathedral were

dimmed for a period of silence, leaving a single candle burning. And
