could have been prevented. I was angry reading it. It makes me feel | :02:39. | :02:45. | |
that it was unnecessary. I truly believe, if he was monitored and had | :02:46. | :02:51. | |
been taking his medication, my sister would still be alive today. | :02:52. | :02:59. | |
It is two years since she was killed. This report was ordered as | :03:00. | :03:05. | |
Matthew Triddon had been a user of mental health services in the city. | :03:06. | :03:08. | |
It concludes the homicide could not have been predicted and if it -- it | :03:09. | :03:16. | |
was difficult to see how it could have been prevented. However, it | :03:17. | :03:21. | |
highlighted shortcomings. The report said there was a communication | :03:22. | :03:27. | |
between the mental health team and his GP. He had not taken his | :03:28. | :03:31. | |
medication for a year before the accident, but there was no system to | :03:32. | :03:36. | |
monitor whether prescriptions had been collected. The workload for the | :03:37. | :03:41. | |
psychiatrist was excessive. There was no single care coordinator to | :03:42. | :03:45. | |
guide him through the system, but instead was seen by nine separate | :03:46. | :03:52. | |
GPs. So good was the progress that this gentleman was making was that | :03:53. | :03:59. | |
he was a pathway to know medication because he was recovering. It is | :04:00. | :04:06. | |
important to say that our goal is to enable people to recover from mental | :04:07. | :04:10. | |
illness. One of the saddest things from this case is that Matthew | :04:11. | :04:19. | |
Triddon felt there was a stigma attached to mental health services. | :04:20. | :04:32. | |
The report today highlight earlier occasions where concerns were raised | :04:33. | :04:39. | |
about Matthew Triddon's actions. In 2003, a community psychiatric nurse | :04:40. | :04:43. | |
recorded there was some risk of violence to himself and others. He | :04:44. | :04:48. | |
was sectioned under the Mental Health Act on four occasions. Once | :04:49. | :04:54. | |
in 2007 after he was arrested with a loaded area pistol in a briefcase. | :04:55. | :04:59. | |
After that incident, his parents wrote to a tribunal saying they | :05:00. | :05:06. | |
would have to take civil action against all parties they considered | :05:07. | :05:12. | |
negligent. Although we could have predicted he might not take his | :05:13. | :05:16. | |
medication, again, there is no sense be good at predicting what that | :05:17. | :05:20. | |
might have led to. That is no comfort to the family, I understand | :05:21. | :05:27. | |
that. Gutted because in my eyes this was preventable. It should not have | :05:28. | :05:31. | |
happened. Now we have to live our lives without my sister. My kneess | :05:32. | :05:43. | |
