Mr Parry then turned the gun on himself,

The 50-year-old was jailed earlier this year for murder, and sentenced

Today's report by the police watchdog highlights

On May 2nd 2013, an officer spoke to Mr Parry.

Mr Parry was behaving aggressively in front of officers, but

We do know that the police and the are committed to making improvements

and we do know of the IPCC that those improvements must be made,

because it is not the first time that we have highlighted failings
