from mental health problems. When the 34`year`old went to the Kent and | :01:32. | :01:35. | |
Canterbury Hospital in 2010, he was never seen by an A doctor and was | :01:36. | :01:39. | |
transferred to another unit by staff trying to meet NHS targets. Simon | :01:40. | :01:47. | |
Jones has our exclusive report. He sought help after an overdose, | :01:48. | :01:53. | |
only to be failed. He would still be here, they basically said that. If | :01:54. | :01:56. | |
they had actually bothered with him he would still be with us now. It | :01:57. | :02:01. | |
has left a massive gap in the family. An NHS investigation has | :02:02. | :02:07. | |
concluded he was inadequately assessed when he arrived for | :02:08. | :02:12. | |
emergency care in Canterburx. He was transferred to a clinical ddcision | :02:13. | :02:15. | |
unit simply to avoid reaching the four hour waiting time targdts in | :02:16. | :02:18. | |
A and was not adequately monitored. You cannot be more in | :02:19. | :02:31. | |
need and more vulnerable whdn you are suicidal. You have ment`l health | :02:32. | :02:36. | |
difficulties. You are in thd heart of a fully functioning hosphtal and | :02:37. | :02:40. | |
yet you are allowed to take your own life in those circumstances. It is | :02:41. | :02:47. | |
almost unbelievable. The NHS Litigation Authority found Simon was | :02:48. | :02:51. | |
not seen by an A doctor at any point. If Mr Wilson had been placed | :02:52. | :02:56. | |
on 15 minute observations hd would not have succeeded in killing | :02:57. | :03:00. | |
himself after his transfer. But for these failings, Simon would not have | :03:01. | :03:07. | |
died. It is a problem that the police are | :03:08. | :03:14. | |
picking up the pieces after failures in the duty of care of the | :03:15. | :03:29. | |
psychiatric services. Simon had been discharged from another hospital | :03:30. | :03:34. | |
despite being at a high risk of suicide. That was just before he | :03:35. | :03:43. | |
killed himself. I have no confidence in the NHS. Not one bit. Nothing. | :03:44. | :03:50. | |
The Trust there has offered its condolences. Simon Jones johns us | :03:51. | :03:53. | |
from the Kent and Canterburx Hospital. Simon, managers are | :03:54. | :03:55. | |
insisting tonight that lessons have been learned. This letter to the | :03:56. | :04:03. | |
family from the NHS this mistake after mistake. For example | :04:04. | :04:09. | |
psychiatric nurse refused to deal with Simon because he had bden | :04:10. | :04:13. | |
drinking. Instead the nurse went home for his shift had actu`lly | :04:14. | :04:17. | |
vanished. Simon should have been accompanied when he went into the | :04:18. | :04:20. | |
toilet. But the NHS Trust s`id that things have now changed and there is | :04:21. | :04:25. | |
much better training regardhng the care of mental health patients in | :04:26. | :04:31. | |

former soldier who's now holeless and facing deportation says he feels | :04:38. | :04:40. | |
