give them that legitimacy, they shouldn't exist in my opinion. The | :02:09. | :02:13. | |

heart attack. His family say they have just wanted the truth. We never | :03:50. | :03:55. | |

job, over what has happened. To our dad. But we felt that the standard | :04:01. | :04:07. | |
of dad's care over those two days was obviously extremely poor. What | :04:08. | :04:12. | |
we would like to see is that for the doctors and nurses who were | :04:13. | :04:19. | |
involved, in dad's case, where their performance was less than competent, | :04:20. | :04:22. | |
there should be a note on their record, to say what has happened and | :04:23. | :04:26. | |
that they have within involved in this It is a statutory deucety to | :04:27. | :04:31. | |
report such deaths to coroner. It took the trust six months to report | :04:32. | :04:36. | |
this death. Held later the inquest revealed a catalogue of failings. | :04:37. | :04:42. | |
The family received an apology, the trust's medical director said their | :04:43. | :04:46. | |
father had died as a result of mistakes made by clinical staff of | :04:47. | :04:50. | |
the hospital. He also apologised in his words, for the major mistake he | :04:51. | :04:54. | |
had made in advising that there was no need to inform the coroner about | :04:55. | :04:59. | |
