Bristol's history is entwined with the industry. It was one of the | :04:39. | :04:46. | |
major employers here at the turn of the 20th century. Until the 1980's | :04:46. | :04:50. | |
Imperial Tobacco offered free cigarettes to employees. The | :04:50. | :04:55. | |
previous ledge of smokers here has been linked to that legacy. One in | :04:55. | :04:59. | |
four people living in Bristol enjoy a smoke. That's a significantly | :04:59. | :05:02. | |
higher percentage than across the south-west as a whole. One in five | :05:02. | :05:07. | |
smokers are thought to have used illegal tobacco or cigarettes. | :05:07. | :05:13. | |
worry is that people will end up buying cheap counterfeit cigarettes | :05:13. | :05:17. | |
and people have no idea what's in them. These are coming into the | :05:17. | :05:20. | |
country by their millions and many end up in the hands of children, | :05:21. | :05:24. | |
because criminal gangs don't care who they sell to. They are quite | :05:24. | :05:28. | |
happy to sell to teenagers. those trying to win the anti- | :05:28. | :05:34. | |
smoking battle, that argument is a distraction. Big tobacco say that | :05:34. | :05:37. | |
the evidence is poor, so in countries like Spain and Italy, | :05:37. | :05:41. | |
where they have the lowest taxation, they've had some of the biggest | :05:41. | :05:44. | |
smuggling problems, so I wouldn't be worried about that. The | :05:44. | :05:49. | |
Chancellor could always rebalance it. I think it is shroud waving and | :05:49. | :05:55. | |
not really a true risk. There's a gaggle of smokers outside this pub. | :05:55. | :05:57. | |
Mr Osborne's financial penalty was convincing some to think about | :05:57. | :06:04. | |
trying to quit. I've always wanted to over an amount of years. | :06:04. | :06:12. | |
Particularly now it's gone up so much. Not very happy. I may have to | :06:12. | :06:18. | |
give it up. Because of the cost? Yeah. Basically, that's what it is, | :06:18. | :06:23. | |
cost. The rise in the cost of tobacco has been labelled as sin | :06:23. | :06:33. | |

tax by some, but of course not everyone wants to be saved. There | :06:46. | :06:52. | |
are questions about the new hospital in the area, and we have | :06:52. | :06:56. | |
this report. The brand new �45 million hospital for south Bristol. | :06:56. | :07:03. | |
It took decades of campaigning to get it built, now -- but now before | :07:03. | :07:07. | |
it has treated the first patient this academic and GP is raising | :07:07. | :07:13. | |
questions about whether it will be fully used. It provides a range of | :07:13. | :07:18. | |
services, but some will be hiefd off by competition and that will -- | :07:18. | :07:22. | |
hiveed off by competition and that will affect rural areas. Patients | :07:22. | :07:26. | |
will have more choice about where they're treated and that could mean | :07:26. | :07:31. | |
the private sector. NHS hospitals like this new community one in | :07:31. | :07:37. | |
south Bristol will be competing for �60 billion worth of funding, held | :07:37. | :07:43. | |
by the new GP commissioning groups. For someone with heart disease, | :07:43. | :07:47. | |
really there will be a growing need for general practice and hospitals | :07:47. | :07:51. | |
to work together, but the bill by introducing competition will bring | :07:51. | :07:59. | |
in many providers actually reverses that. I think potentially it takes | :07:59. | :08:04. | |
back the progress that the NHS has made over decades. He's ot the only | :08:04. | :08:09. | |
one concerned. -- not the only one concerned. They are not listening | :08:09. | :08:14. | |
to the people who voted them and it makes us angry. This woman voted in | :08:14. | :08:17. | |
favour of most reforms, even though campaigners warned it would lose | :08:18. | :08:24. | |
her support. They've made sure that people who are private providers if | :08:24. | :08:29. | |
they want to do NHS patients, they will get the NHS rate. That's quite | :08:29. | :08:32. | |
right. They shouldn't be able to make a profit. They won't get more | :08:32. | :08:38. | |
money than the NHS gets. All they do is offer more opportunity for | :08:38. | :08:43. | |
treatment and more choice for people to have treatment and faster. | :08:43. | :08:48. | |
The hospital opens its doors for business next month. How much | :08:48. | :08:52. | |
business there will be though depends on the choices patients and | :08:52. | :08:59. | |
GPs now make. Well, a short while ago I spoke to the Health Minister, | :08:59. | :09:02. | |
Simon Burns, and asked him if the new hospital was being undermined | :09:02. | :09:12. | |

by the NHS reforms. No, not at all, because doctors, with consultations | :09:12. | :09:16. | |
with their patients, will determine, as they do now, which provider is | :09:16. | :09:20. | |
going to provide the care. If the hospital that you are taking about, | :09:20. | :09:24. | |
I'm sure it's a first-class NHS hospital, and patients will want to | :09:24. | :09:29. | |
go there, then they'll choose it with their GP. If the GPs don't | :09:29. | :09:34. | |
refer there, then what happens to it? Well, it will be up to the | :09:34. | :09:39. | |
patient to exercise their choice as to where they want to go for their | :09:39. | :09:43. | |
treatment. Obviously in consultation with their GPs. If the | :09:43. | :09:47. | |
hospital, which I'm sure it will be, is providing first-class care for | :09:47. | :09:53. | |
patients, then they will want to go to their local community hospital, | :09:53. | :09:56. | |
where they leave. -- live. There is nothing to stop an NHS hospital | :09:56. | :10:03. | |
going bust if it doesn't get enough patients? The fact is, though, that | :10:03. | :10:09. | |
that situation will not arise, because you'll not see almost | :10:09. | :10:12. | |
certainly full-blown district general hospitals in the private | :10:12. | :10:17. | |
sector providing the whole range of care and treatment that NHS | :10:17. | :10:21. | |
hospitals do and the private sector is not allowed, under the | :10:21. | :10:26. | |
legislation to cherry-pick services, to precisely stop them from taking | :10:27. | :10:30. | |
work from the NHS that is the work that generates income rather than | :10:30. | :10:38. | |

We are pleased he stuck to plan A, with the deficit and for big | :12:43. | :12:47. | |
members, we are delighted they had a corporation tax cut, 30% over two | :12:47. | :12:52. | |
years, it's a huge cut, but most of the members here aren't large | :12:52. | :12:55. | |
companies. They don't pay corporation tax. They pay small | :12:56. | :13:01. | |
companies tax. They haven't had any cut at all. Worse than that, they | :13:01. | :13:07. | |
have 6% in uniform business rates starting next month, so overall not | :13:07. | :13:11. | |

Government says it will cut loads out of tax loopholes. What do you | :13:18. | :13:23. | |
